Ictal piloerection is associated with high-grade glioma and autoimmune encephalitis-Results from a systematic review.

PURPOSE: To comprehensively analyze ictal piloerection (IP) in a large number of subjects. METHODS: We performed a systematic review on cas … n among males (median:3/day, IQR:1-11) (p = 0.017). The two most common underlying pathologies were limbic encephalitis (23%) and astrocytoma (23%, among them 64% WHO III-IV astrocytoma). CONCLUSION: IP was particularly associated with autoimmune encephalitis and high-grade glioma, suggesting IP's particular clinical importance in directing diagnostic work-up.

Neuronal antibodies in patients with suspected or confirmed sporadic Creutzfeldt-Jakob disease.

OBJECTIVES: There have been reports of patients with antibodies to neuronal antigens misdiagnosed as sporadic Creutzfeldt-Jakob disease (sCJD). Conversely, low levels of antibodies to neuronal proteins have been reported in patients with sCJD. However, the frequency of misdiagnoses, or of antibodies in patients with subsequently confirmed sCJD, is not clear. METHODS: We reviewed 256 consecutive cases of sCJD seen in the National Prion Clinic, of whom 150 had sera previously referred for selected antibody tests. Eighty-two available samples were retested for antibodies to N-methyl-d-aspartate

In vivo visualization of activated glia by [11C] (R)-PK11195-PET following herpes encephalitis reveals projected neuronal damage beyond the primary focal lesion.

A major challenge in the assessment of brain injury and its relationship to the ensuing functional deficits is the accurate delineation of the areas of damage. Here, we test the hypothesis that the anatomical distribution pattern of activated microglia, a normally dormant population of resident brain macrophages, can be used as a surrogate marker of neuronal injury not only at the primary lesion site but also in the antero- and retrograde projection areas of the lesioned neurones. Two patients with asymmetrical herpes simplex encephalitis were serially scanned 6 and 12 months after the acute i

Gliomatosis cerebri with blindness: A case report with literature review.

Cerebral gliomatosis (GC) is a rare diffuse infiltrative growth pattern of glioma with nonspecific clinical manifestations like visual impairment that may involve bilateral temporal lobes. Herpes simplex encephalitis (HSE) and limbic encephalitis (LE) can also lead to temporal lobe involvement. Differentiating these entities is necessary for patients with misleading presentations and imaging findings. To the best of our knowledge, this is the third case of GC presenting with blindness. The patient was a 35 years-old male in a drug rehabilitation center for heroin addiction. He presented with a…

Immune-mediated inflammatory conditions in epilepsy: role of autoimmunity.

Several studies have demonstrated the relationship between epilepsy and neuroinflammation; syndromes such as Rasmussen syndrome and paraneoplastic limbic encephalitis associated with epilepsy and some cases with refractory status epilepticus are prominent examples of this relationship. However, seizures are one of the symptoms that patients with autoimmune encephalitis frequently present with or are at increased risk for epilepsy, hence the confusion with associated autoimmune epilepsy. Moreover, the etiology of one-third of all epilepsies remains unknown, and it is estimated that approximatel…

Mechanisms of Blood-Brain Barrier Disruption in Herpes Simplex Encephalitis.

Herpes simplex encephalitis (HSE) is often caused by infection with herpes simplex virus 1 (HSV-1), a neurotropic double-stranded DNA virus. HSE infection always impacts the temporal and frontal lobes or limbic system, leading to edema, hemorrhage, and necrotic changes in the brain parenchyma. Additionally, patients often exhibit severe complications following antiviral treatment, including dementia and epilepsy. HSE is further associated with disruptions to the blood-brain barrier (BBB), which consists of microvascular endothelial cells, tight junctions, astrocytes, pericytes, and basement me

Autoimmune encephalitis related to LGI1 antibodies with negative MRI study: Description of two cases.

BACKGROUND: Leucine-rich glioma inactivated 1 (LGI1) antibody-related autoimmune encephalitis is easily misdiagnosed clinically because of its complex and diverse clinical manifestations. We present two cases of LGI1 antibody-related encephalitis with negative imaging findings and perform a literature review on this disease entity. CASE DESCRIPTION: The first case was that of a 60-year-old man who presented with involuntary movement of the paroxysmal right limb. The second case was that of a 66-year-old man who presented with hearing hallucinations, involuntary shaking of the right limb, and

Neuroprotective Drug Discovery From Phytochemicals and Metabolites for CNS Viral Infection: A Systems Biology Approach With Clinical and Imaging Validation.

BACKGROUND: Recent studies have reported that pulmo-neurotropic viruses can cause systemic invasion leading to acute respiratory failure and neuroinfection. The tetracycline class of secondary metabolites of microorganisms is effective against several migrating neurotropic viral disorders, as Japanese-Encephalitis (JE), Severe-Acute-Respiratory-Syndrome Coronavirus-2 (SARS-COV2), Human-Immunodeficiency-Virus (HIV), and Simian-Immunodeficiency-Virus (SIV). Another microbial secondary metabolite, cephalosporin, can be used for anti-viral combination therapy. However, a substantial public health

Encephalitis Induced by Immune Checkpoint Inhibitors: A Systematic Review.

IMPORTANCE: Encephalitis is a severe immune-related adverse event secondary to treatment with immune checkpoint inhibitors (ICIs). The spect … checkpoint inhibitors may induce mainly 2 different encephalitic syndromes: a focal limbic or extralimbic encephalitis and a meningoencephalitis. Immune checkpoint inhibitor-induced encephalitis is associated with an overall favorable outcome, with a low rate of fatal events. An undetected preexisting paraneoplastic encephalitic syndrome may be triggered by ICIs, an …

Imaging biomarkers in antibody-mediated autoimmune encephalitis.

BACKGROUND: Imaging, particularly multimodal magnetic resonance imaging (MRI), serves as an essential auxiliary examination for diagnosing autoimmune encephalitis (AE). The diversity of autoantibodies complicates the imaging presentation of AE, exhibiting both common and individual features across different subtypes of AE. Currently, there is a lack of comprehensive studies on the imaging features of different subtypes of AE. The study aimed to explore imaging biomarkers for AE mediated by various subtypes of antibodies and clarify their significance in disease severity, treatment response, an

Clinical specificities and outcome of LGI1-antibody encephalitis according to age, sex and HLA.

BACKGROUND: Patients with leucine-rich glioma-inactivated 1 antibody (LGI1-Ab) encephalitis are typically elderly men that often carry human leucocyte antigen (HLA)-DRB1*07:01 (≈90%). Herein, we aimed to investigate whether patients with atypical demographic profiles or not carrying DRB1*07:01 have distinct clinical manifestations and outcome. METHODS: Retrospective chart review of LGI1-Ab patients diagnosed at the French Reference Centre and three other European centres. RESULTS: Among 238 patients included, median age at onset was 66 years (IQR: 60-73), 65% were male, 89% carried DRB1*07:0

Clinical and neuropsychological course of young adult patient with anti-GAD-65 autoimmune encephalitis: A case report.

Objective: Among autoimmune limbic encephalitis syndromes, glutamic acid decarboxylase 65 (GAD65) antibody-associated syndromes represent a relatively rare and clinically heterogeneous subtype characterized by prominent temporal lobe involvement and diverse neurological manifestations. GAD65 is an intracellular antigen involved in gamma-aminobutyric acid (GABA) synthesis, and associated autoantibodies have been identified across several autoimmune and neurological conditions with limited diagnostic specificity and variable neurological and neuropsychological presentations. Method: This case re…

Anti-Ma2 Encephalitis as a Treatable Cause of Narcolepsy Type 1: A Case Report.

BACKGROUND Secondary narcolepsy type 1 (NT1) is an uncommon but increasingly recognized disorder, often associated with autoimmune etiologies that disrupt hypothalamic hypocretin/orexin-producing neurons. Anti-Ma2 encephalitis, a paraneoplastic syndrome commonly linked to testicular tumors, typically involves the limbic system and diencephalon, leading to diverse neuropsychiatric manifestations. While hypersomnia has been reported in a subset of these patients, full syndromic NT1 - characterized by cataplexy, hypocretin deficiency, and objective sleep study abnormalities - remains underrecogni

Patient-derived monoclonal LGI1 autoantibodies elicit seizures, behavioral changes and brain MRI abnormalities in rodent models.

OBJECTIVE: Limbic encephalitis with leucine-rich glioma inactivated 1 (LGI1) protein autoantibodies is associated with cognitive impairment, psychiatric symptoms, and seizures, including faciobrachial dystonic seizures (FBDS). Patient-derived LGI1-autoantibodies cause isolated symptoms of memory deficits in mice and seizures in rats. Using a multimodal experimental approach, we set out to improve the validity of existing in vivo rodent models to further recapitulate the full clinical syndrome of anti-LGI1 antibody mediated disease. METHODS: A monoclonal anti-LGI1 antibody (anti-LGI1 mAb) deri…

Paraneoplastic Neurological Syndromes Associated With Parotid Cancer Expressing the Zic4 Antibody: A Case Report.

Paraneoplastic neurological syndromes (PNS) are rare, immune-mediated disorders associated with cancer, often presenting with various neurological symptoms, such as encephalomyelitis, limbic encephalitis, and rapidly progressive cerebellar syndrome. A 66-year-old woman presented to our department with rapidly increasing swelling in her right parotid gland. Fine needle aspiration cytology confirmed parotid cancer, and a right parotidectomy and dermectomy were planned. Preoperatively, she experienced symptoms of nausea and vertigo and was later diagnosed with PNS based on the presence of Zic4 an…
