Fournier's or genito-perineal gangrene of the male genitalia typically spares the testis, urethra and deep penile components. It usually is polymicrobial and synergistic. To our knowledge, this is the first reported case in the literature with severe, deep, full penile thrombosis that caused a severe urethral stenosis and required later surgery. Only Proteus mb. could be isolated and gave rise to a disseminated intravascular coagulation syndrome that has probably accentuated the thrombotic penile phenomenon. The process was secondary to urethral lithiasis.

An overview of inherited disorders of purine metabolism, concentrating on well established enzyme defects is given. Included are HPRT and the LNS, APRT and 2,8-dihydroxyadenine lithiasis, hyperactivity of PRPP synthetase, ADA and PNP and immunodeficiencies. Emphasis is put on underlying molecular mechanisms on the gene-, enzyme-, or metabolite level for a better understanding of the events leading from the genotype to the clinical phenotype. Finally some aspects of extracellular purine nucleotide metabolism catalyzed by cell surface-bound ectoenzymes are discussed.

This kind of patient has allowed further development of laparoscopic surgery in urology derived from the addition of reported conditions: obstructive prostate carcinoma, juxta-vesical gross lithiasis imbedded for 2 years, complexity of ureteroscopic access, large volume for extracorporeal lithotripsy and the need to know the extend of nodular affectation.

We present the case of an 82-year-old woman with history of right colon adenocarcinoma in whom a routine control scan is performed using a dual-layer detector spectral CT scanner. Conventional CT scan images show a dilated cystic bile duct with no apparent cause of obstruction, finding also no lithiasis inside the gallbladder. Non-calcified gallstones are composed mainly of cholesterol and can have the same radiographic density as bile, making them hard or impossible to spot on conventional CT.
