BACKGROUND: India has few mental health professionals to treat the large number of people suffering from mental disorders. Rural areas are particularly disadvantaged due to lack of trained health workers. Ways to improve care could be by training village health workers in basic mental health care, and by using innovative methods of service delivery. The ongoing Systematic Medical Appraisal, Referral and Treatment Mental Health Programme will assess the acceptability, feasibility and preliminary effectiveness of a task-shifting mobile-based intervention using mixed methods, in rural Andhra Prad

In Canada, there are currently no guidelines at either the federal or provincial level regarding the provision of kidney transplantation services to foreign nationals (FN). Renal transplant centres have, in the past, agreed to put refugee claimants and other FNs on the renal transplant waiting list, in part, because these patients (refugee claimants) had health insurance through the Interim Federal Health Programme to cover the costs of medication and hospital care. However, severe cuts recently made to this programme have forced clinicians to question whether they should continue with transpl

Dens invaginatus is a dental anomaly resulting in an abnormal tooth morphology, most commonly affecting the permanent dentition. An associated infolding of the outer enamel organ results in structural pocketing and increased susceptibility to dental disease. Clinical sequalae of affected teeth may include dental caries, pulpal pathology, devitalisation, discolouration, localised periodontal pocketing or increased tooth mobility. Early diagnosis is therefore essential in the delivery of effective prevention and informed treatment planning. Clinicians should be aware of the diverse clinical and

A Public Health Emergency of International Concern (PHEIC) has been declared in the Democratic Republic of Congo (DRC) due to Ebola disease from Bundibugyo virus. Non-health-focused factors should be fundamental to how DRC and its partners respond to and recover from the current emergency. Filovirus emergencies are violent in every sense-the disease experience of patients and their caregivers, the tumult in impacted communities, sometimes the physical manifestations of frustration against healthcare services, the fits and starts of concerted response, and the milieu of conflict in which this e

On January 12, 2024 the safety committee of the European Medicines Agency (EMA) recommended precautionary measures over a potential risk of neurodevelopmental disorders in children born to men treated with valproate. These new measures recommend patient supervision by a specialist in the management of epilepsy, bipolar disorder, or migraine. In the United Kingdom, the Medicines and Healthcare products Regulatory Agency (MHRA) issued a far more stringent precaution, warning against prescribing valproate to anyone under 55 years of age. We, members of the European Network of Teratology Informati

Brain abscesses represent areas of localised infection of the brain parenchyma. Those confined to the brainstem are rare and usually fatal if untreated. Streptococcus intermedius is a common causative organism of brain abscesses and is associated with significant morbidity. We describe a case report of medullary abscess secondary to dental infection in a 68-year-old patient. The patient presented with headaches and flu-like symptoms progressing to left hemiparesis and reduced consciousness/bulbar function. The patient underwent emergency posterior fossa craniectomy and drainage with subsequent

This paper describes and analyzes the legal and normative framework guiding the use of mobile units in Portugal, United States and Brazil, which seek to improve access and continuity of care for people in homelessness. We used a comparative analysis through literature and documentary review relating three categories: context (demographic, socio-economic and epidemiological), services system (access, coverage, organization, management and financing) and, specifically, mobile units (design, care and financing model). The analysis was based on the theory of convergence/divergence between health s

The objective of this study was to evaluate community-based outpatient mental health services for young adults. Participants were interviewed at ages 21, 24, 27, and 30. Outcomes included: (1) symptoms of depression, generalized anxiety, social phobia, dysthymia and post traumatic stress individually and as a global scale; and (2) a dichotomous diagnosis variable inclusive of all above disorders. Treatment was indicated by an outpatient visit to a psychiatrist or other professional. Treatment did not reduce mental disorder or symptoms. Substance use, violence, poverty, community disorganizatio

OBJECTIVE: The emergency physician should be familiar with the wide spectrum of pediatric mental health emergencies because they are commonly encountered in emergency medical practice.
METHODS: A review of the literature was done in order to develop an approach for dealing with children presenting with mental health disorders in the emergency department (ED).
RESULTS: Children' mental health emergencies have a wide spectrum from behavioral disturbances to major depression. An approach to the issues involved in caring for these patients is discussed which acknowledges the essential role of th

Both general practice and homeopathic consultations are organized around the key task of treating patients' health-related problems. Despite their different theories of healing, interactions between professionals and patients in both share many features, though there are also clear differences in the ways in which patients and professionals go about the process of problem solving. This paper compares the ways in which a specific activity, the delivery and reception of the reason for the visit, is managed in these two institutional environments. Through the comparison, it discusses ways in whic

This paper examines whether traditional medicine promotes biopsychosocial fulfillment in African health and argues that every society has its own method of managing illness and of controlling the environment. The paper shows how each society functions within the constraints of its own unique system of social structure, language and communication, belief and customs, attitudes, behaviour and 'cultural facts' upon which health services are based. The concept, meaning and uses of traditional medicine are also discussed. It shows how traditional medicine can be used as a 'psychological opium' as a

Community-based organizations (CBOs) played a key role in providing health and safety net support to low-income Latino communities during the COVID-19 pandemic; yet we know little about how organizations navigated challenges during this time. Understanding how CBOs navigate challenging periods is critical to ensuring a robust safety net for Latino and other underserved populations. We examined the factors that allowed CBOs to address Latino communities' needs and the potential long-term impact of the pandemic on the sustainability of their health programs. Using a case study approach, we condu

Coronavirus disease (COVID-19) has engulfed the whole world, and India has been the second worst-hit nation. Organ transplant services were halted in both the public and private care sectors of India, with public care sectors more adversely affected. Deceased donations were disproportionately more affected, with unfavorable rates at the peak of the pandemic. Mortality outcomes of COVID-19 among different organ transplant recipients in India have been lower compared with the Western world, with younger age and less comorbidities among Indian populations partly responsible for the lower mortalit

Continuous improvement of practices is a constant challenge for healthcare organizations, managers, and decision-makers. Despite knowledge of evidence-based data and recommended health practices, integrating best practices into care and services remains difficult and requires tailored support approaches. Over the past 30 years, several initiatives have been implemented to support the continuous improvement of practices in the field of mental health in Quebec. Within these continuous improvement processes, close collaboration with stakeholders at all stages of the process, under collaborative l

BACKGROUND: Polysubstance use (i.e., using ≥2 psychoactive substances concomitantly) is associated with increased morbidity and mortality and complicates drug treatment needs among people who inject drugs (PWID). We explored patterns, contexts, motivations, and perceived consequences of polysubstance use among PWID in small cities and towns in the U.S. Northeast.
METHODS: Between October 2018 and March 2019, we conducted semi-structured interviews with 45 PWID living outside of the capital cities of Rhode Island and Massachusetts recruited online and through community-based organizations. Wri
