Since 1981, we have used the stapes allograft, singly or in combination with homograft incus, in 20 cases of tympanoplasty and in 7 cases of fixed stapes. All of the allograft stapes discussed were procured, processed and stored in 1980 by the Hyogo Ear Bank, the first to be established in Japan. The stapes allograft was used in three situations: an intact posterior canal wall, an open mastoidectomy cavity, or a fixed stapes (postinflammatory, congenital or otosclerotic). Two modalities of positioning of the donor stapes were attempted: (1) the allograft stapes was placed in the normal positio…

INTRODUCTION: For pediatric patients, bone-conduction hearing devices (BCHD) have demonstrated excellent outcomes. Unique to this population, BCHD implant surgeries can be technically challenging in children due to thinner, developing bone and syndromes with atypical anatomy. Image-guided surgical navigation (IGSN) clarifies underlying skull structure, potentially improving outcomes. IGSN is commonly used in otorhinolaryngologic surgeries, but current use in BCHD placement surgeries remains unprecedented. We report favorable results of IGSN in BCHD implantation for three children with complex

Cochlear implantation (CI) in radical cavities remains challenging due to the risk of postoperative electrode extrusion. We report a single-stage approach employing piezoelectric instrument (PEI) to optimize cavity osteoplasty, atraumatic cochlear access, and electrode stabilization in the setting of partial scala tympani ossification. A 72-year-old female with left profound sensorineural hearing loss and prior canal wall (CW)-down mastoidectomy underwent CI with posterior CW reconstruction. Angled and straight PEI inserts facilitated controlled bone removal over the facial nerve and creation …

Temporal bone encephalocele (TBE) has become less common as the incidence of chronic mastoid infection and surgery for this condition has decreased. Due to its declining incidence, the diagnosis of TBE may be delayed and result in the development of serious complications such as cerebrospinal fluid leak, meningitis, epidural or subdural abscess. Six cases of large (>1 cm) TBE of diverse etiology are described. Two patients had suffered previous temporal bone fractures, two had had prior mastoidectomy, and two patients had long-standing chronic mastoiditis. Two patients had undergone prior unsu…

BACKGROUND: Atlanto-axial rotatory fixation is a persistent deformity of the C1-2 vertebral relationship caused by subluxation of the articular surfaces, and can occur after positioning for ENT procedures where the head is rotated - for example to access the ear or posterior triangle of the neck. If promptly recognised, it can usually be managed successfully with conservative methods, without long-lasting sequelae, but delayed or inappropriate management may lead to permanent neck deformity, neurological problems and pain.
METHOD: Case review.
CASE REPORT: Two children with atlanto-axial rot

OBJECTIVE: The purpose of this study was to evaluate how various sociodemographic factors such as rurality, age, race, body mass index, and sex affect the surgical management of cholesteatoma. Canal wall-down (CWD) tympanomastoidectomy and longer surgical times were used as potential proxies for increased severity of disease.
STUDY DESIGN: Retrospective.
SETTING: A tertiary academic center.
METHODS: All patients who underwent primary tympanomastoidectomy for cholesteatoma at the University of Kentucky from January 2008 through September 2023 were reviewed in this study. The rurality of the …

BACKGROUND: Tuberculous otitis media is a chronic Mycobacterium tuberculosis infection of the middle ear tissues. Diseases with varied and insidious clinical features can make diagnosis difficult and delay treatment.
CASE PRESENTATION: Here, we document a case of tuberculous otitis media in a 46-year-old ethnic Han woman that manifested as nonspecific chronic otitis media. A mastoidectomy and tympanoplasty were performed for the initial diagnosis of cholesteatoma. The histopathology of the tissue specimen revealed granuloma formation with necrosis. OBJECTIVE: To determine how the ear-canal sound pressures generated by earphones differ between normal and pathologic middle ears.
DESIGN: Measurements of ear-canal sound pressures generated by the Etymtic Research ER-3A insert earphone in normal ears (N = 12) were compared with the pressures generated in abnormal ears with mastoidectomy bowls (N = 15), tympanostomy tubes (N = 5), and tympanic-membrane perforations (N = 5). Similar measurements were made with the Telephonics TDH-49 supra-aural earphone in normal ears (N = 10) and abnormal ears with mastoidectomy bowls (N = 10), tympanostomy t…

Perioperative anaphylaxis is a rare but potentially fatal event, with an incidence ranging from 1:300 to 1:20,000 surgeries and a mortality rate of 3-9%. Its intraoperative occurrence is diagnostically challenging due to patient sedation and overlapping drug effects. Crystalloids like Ringer's Lactate are rarely implicated. A 27-year-old American Society of Anesthesiologists grade I female undergoing mastoidectomy developed intraoperative hypotension, tachycardia, and generalized erythema after induction with standard anesthetics and Ringer's Lactate infusion. Anaphylaxis was suspected, and Ri…

Background: Cholesteatoma-related posterior mastoid plate defects may provide a route for recurrent otogenic meningitis. Case Presentation: A 32-year-old man with chronic right otorrhea and two prior bacterial meningitis episodes had HRCT evidence of mastoid cholesteatoma and a focal posterior mastoid plate defect with dural exposure. A retroauricular canal-wall-up mastoidectomy was performed; matrix was dissected from intact posterior-fossa dura. The defect was repaired using a three-layer onlay construct of temporalis fascia, ipsilateral conchal cartilage, and free temporalis muscle. Histopa…

Endolymphatic duct blockage is a relatively new treatment option for Ménière's disease, aiming to reduce vertigo attacks while sparing hearing and equilibrium. After a regular mastoidectomy, the posterior semicircular canal is identified, and Donaldson's line is determined. This is a line through the horizontal semicircular canal, crossing the posterior semicircular canal. The endolymphatic sac is usually found at this site under the posterior semicircular canal. The bone of the endolymphatic sac and the dura are thinned until the sac is skeletonized, after which the endolymphatic duct is iden…

OBJECTIVE: This study aimed to determine anatomical landmarks for accurate and safe middle turn cochleostomy on cadaveric temporal bones.
METHODS: In 17 cadaveric wet adult temporal bones, cortical mastoidectomy was performed, followed by extended posterior tympanotomy through which a middle turn opening was created anterior to the stapes footplate. Micro-measurements of various lengths were taken from the cochleostomy to normal middle-ear anatomical landmarks using a digital microscope.
RESULTS: The mean length from the middle turn cochleostomy to the processus cochleariformis was 1.8 ± 0.3…

An elderly African American woman presented to our clinic following 9 months of right-sided unilateral headache, otorrhoea and progressive hearing loss. Despite treatment with topical and oral antibiotics, her clinical condition worsened, and imaging showed mastoid coalescence with an associated subgaleal abscess. She underwent right mastoidectomy and was discharged 3 days later on broad-spectrum intravenous antibiotics despite negative operative cultures. Six weeks later, she was hospitalised with diplopia secondary to a right lateral rectus palsy. Imaging showed abscess resolution but progre…

The middle fossa approach was used in 11 patients with profound bilateral hearing loss for insertion of a cochlear implant. Fibroadhesive otitis media (n = 1), bilateral cavity radical mastoidectomy (n = 1), autoimmune inner ear disease (n = 2), previous cranial trauma (n = 1), genetic prelingual deafness (n = 5), and otosclerosis (n = 1) were the causes of deafness. A cochleostomy was performed on the most superficial part of the basal turn, and the electrode array was inserted up to the cochlear apex. Speech perception tests (1-9 months after cochlear implant activation) yielded better resul…
