Surgical management of bilateral mandibular angle fractures: A systematic review.

Bilateral mandibular angle fractures (BMAF) are rare and complex maxillofacial injuries. This review describes the patient factors, procedural factors and post-procedural outcomes of BMAF management. PubMed and Scopus were searched for English papers from 1993 to 2023 using the strategy: (bilateral AND angle) AND (mandib* OR bmaf) AND fracture AND (management OR repair OR surgery). Four papers were eligible for inclusion, representing 198 cases. The mean age of patients was 25 years, with 94.9% of cases male. Interpersonal violence was the leading cause of injury (76.8%). Transbuccal approach …

Deadly cerebral ischemia due to carotid stenosis following a facial shot with a pellet gun: An autopsy case report.

INTRODUCTION: Facial gunshot wounds have devastating functional and aesthetic consequences for the patient. If associated with penetrating craniocerebral injuries, the prognosis is rather compromised even with appropriate medical and surgical treatment. Chop-off injuries with penetrating wounds constitute a challenging situation for the facial reconstructive surgeon in facial trauma. OBSERVATION: This case involved a 49-year-old man who sustained an accidental facial shot from a pellet gun. Radiological and clinical investigations revealed complex ballistic trauma to the maxillofacial region,

Mantle of forensics in child sexual abuse.

Child sexual abuse (CSA) is defined as inappropriate adolescent or adult sexual behavior or contact with a child. Sexual abuse may be committed by any person including those under the age of 18 years when that person is either significantly older than the victim or is in a position of power or control over the child. Detecting CSA requires a high incidence of suspicion and familiarity with physical, behavioral, and verbal indications of abuse. Shame and guilt often may have discussion difficult. Studies have shown that approximately 60% of abused children have injuries to head, face, and mouth

Can a Facial Injury Severity Scale Be Used to Predict the Need for Surgical Intervention and Time of Hospitalization?

PURPOSE: We aimed to evaluate whether the Facial Injury Severity Scale (FISS), proposed by Bagheri et al in 2006, can predict intervention needs in the operating room (OR), length of hospital stay, and need for support by other specialties. PATIENTS AND METHODS: Data were collected from the medical records of trauma patients from a public tertiary hospital between January 2009 and December 2015, related to age, gender, comorbidities, habits, history of maxillofacial trauma, etiology, and presence and location of fractures and lacerations, in addition to type of treatment performed and period

Oral lesions due to child abuse. Identification of the problem and role of the professional.

Violence in the family setting is a serious social problem of obliged concern to all health professionals. The victims are mainly children, women and the elderly. Due to the defenselessness characterizing childhood and the requirements of Spanish law, the dentist must be aware of the magnitude of the situation in order to act in favor of the affected child. The problems encountered in this aspect comprise physical abuse, sexual abuse, physical neglect, emotional abuse and emotional neglect. Any of these problems can have irreversible consequences for the psychological and physical development

Patterns and characteristics of maxillofacial fractures in women.

PURPOSE: Facial trauma in women is complex with physical, psychosocial, and cultural influences impacting clinical presentations. Although multifactorial, assaults and falls are principally reported as the main causes. METHODS: A retrospective review was conducted from January 2012 to January 2017 at the Women and Children's Hospital and Royal Adelaide Hospital, Adelaide. All maxillofacial fractures in women that attended or were referred to the unit were included in this study. The primary objective was to analyse epidemiological trends of facial fractures and clinical outcomes in the South

Unusual dental injuries following facial fractures: report of three cases.

We report 3 cases of unusual dental injuries following facial fractures. The first patient sustained intrusion of a maxillary incisor into the nasal cavity following a mandibular fracture. The tooth dislocated into the pharynx and was found lodged in the piriform fossa during surgery. The second patient sustained intrusion of molars into the maxillary sinus following maxillary and mandibular fractures. His treatment was delayed due to life-threatening hemorrhage. The third case involved ingestion of multiple avulsed teeth into the alimentary tract following severe maxillofacial fractures. Alth

Bilateral microneurosurgical reconstruction of inferior alveolar nerves via autogenous sural nerve transplantation.

Microneurosurgical operative techniques permit satisfactory restoration of sensation in many lesions of the inferior alveolar nerve. Therefore, restoration of the sensory deficit is becoming increasingly more important in the total functional rehabilitation of individuals with mandibular continuity defects involving transection of or permanent damage to the inferior alveolar nerve. This article reviews the case history of a young man who underwent bilateral osseous mandibular reconstruction and microneurosurgical reconstruction of his inferior alveolar nerves following severe maxillofacial tra

Patient-Reported Outcome Measures in Maxillofacial Trauma: A Scoping Review.

OBJECTIVE: The majority of maxillofacial trauma research is limited to objective and clinician-rated outcome metrics. This review was designed to characterize topic domains of patient-reported outcome measures (PROMs) previously used in the field. DATA SOURCES: PubMed, Cochrane, Scopus, and CINAHL. REVIEW METHODS: Publications through March 2025 reporting at least one PROM in maxillofacial trauma patients were included. Study metadata was recorded, and survey instruments were classified as established PROMs or "custom surveys." Each instrument was analyzed, and key topic domains were charact

A Prospective, Randomized Comparison of the Classical Altemir's Method With the Newer Seldinger's Technique of Submental Intubation.

BACKGROUND: Submental intubation performed using the classical Altemir's technique is a well-accepted, safe technique for providing optimal operating field to the maxillofacial surgeon, in cases where either nasotracheal or orotracheal intubation is impossible. We propose a new, percutaneous Seldinger's technique of submental intubation as an interesting alternative to the classical Altemir's technique, wherein a percutaneous dilatational tracheostomy kit is used to dilate the submental tract, instead of bluntly dissecting it. We hypothesized that Seldinger's technique would be associated with

Submental intubation in complex maxillofacial trauma: Pilot balloon protection.

AIMS: This study aims to describe our refined technique of submental intubation to avoid the recorded intraoperative complications related to tube passage and pilot balloon rupture. CASE SERIES: This is a retrospective case series of 21 patients with complex maxillofacial trauma who underwent submental intubation from January 2019 to January 2023. All the patients underwent to the same procedure with a new technique of pilot balloon protection: the pilot balloon was not deflated because, once the connector was removed, only the tube was curved and passed through the incision extraorally while

An epidemiologic retrospective study of maxillofacial traumatology in the elderly.

INTRODUCTION: The elderly population, which is more active than before, is increasingly suffering from trauma (loss of reflexes and systemic pathologic conditions). Surgical management may be more controversial due to the potential consequences of general anesthesia and the sometimes negligible consequences of functional management. The main objective of this study was to analyze the causes and location of facial fractures in subjects older than 65 years. The secondary objective was to evaluate the management of these fractures (surgical or functional) according to comorbidities. MATERIAL & M

Do the Benefits of Systemic Corticosteroids Outweigh Adverse Effects During Maxillofacial Trauma Surgery? A Systematic Review and Meta-Analysis.

PURPOSE: Perioperative systemic corticosteroids are widely used in maxillofacial trauma surgery to prevent postoperative complications, but potentially perceived benefits are speculative rather than evidence-based. The purpose of our systematic review and meta-analysis was to assess the effects of perioperative systemic corticosteroids on clinically significant outcomes in patients undergoing maxillofacial trauma surgery. METHODS: We searched Medline, Embase, CENTRAL, Clinical trial registry, and grey literature as well as references of included trials. Our primary outcomes were facial edema

Orbital corrections with the use of alloplastic material.

After a severe trauma of the face, e.g. fractures involving the zygoma, the upper jaw or other orbital bone alteraions and deviations of the bony orbital contours and also of the orbital contents can subsist, even after primary operative correction. The patients have functional as well as cosmetic complaints. In consequence of eyeball dislocation and incarceration or fibrosis of the external eye muscles, the patients also complain of diplopia in one or more directions. Due to the bony orbit enlargement and reduction of the orbtial contents, the eye is moved inferiorly and backwards, also causi
