The effect of Medicaid payer status on patient outcomes following repair of massive rotator cuff tears.

BACKGROUND: The influence of socioeconomic status and insurance type has not been studied extensively for RCR, particularly not in the high risk massive RCT population. The purpose of this study is to identify relationships between Medicaid payer status and patient outcomes following massive RCR. METHODS: A retrospective review of shoulder surgery database identified 29 patients undergoing massive rotator cuff repair. Patients were stratified based on insurance type into two cohorts, Medicaid (14 patients) and non-Medicaid (15 patients). Missed routine follow-up appointments and comorbidities…

Potential for Medicaid Savings: A State and National Comparison of an Innovative Neonatal Abstinence Syndrome Treatment Model.

In recent years, neonatal abstinence syndrome (NAS) rates have increased rapidly across the United States, rising from 1.2 (2000) to 5.8 (20 … se mothers used long-acting opioids. Given that 81% of NAS cases in the United States are funded by Medicaid programs and that the cost burden is rising rapidly, researchers considered the potential saved charges associated with implementing the same program in other hospitals state- and nationwide. Researchers used regression models to project state and n …

Satisfaction and sense of well being among Medicaid ICF/MR and HCBS recipients in six states.

Self-reported satisfaction and sense of well-being were assessed in a sample of 1,885 adults with intellectual and developmental disabilities receiving Medicaid Home and Community Based Services (HCBS) and Intermediate Care Facility (ICF/MR) services in 6 states. Questions dealt with such topics as loneliness, feeling afraid at home and in one's neighborhood, feeling happy, feeling that staff are nice and polite, and liking one's home and work/day program. Loneliness was the most widespread problem, and there were also small percentages of people who reported negative views in other areas. Few…

Racial and ethnic disparities in prenatal syphilis screening among women with Medicaid-covered deliveries in Florida.

OBJECTIVES: Black and Hispanic infants are 19.9 and 10.3 times more likely, respectively, than white infants to develop congenital syphilis (CS), a disease that is preventable with timely prenatal screening and treatment. We examined racial/ethnic group differences in prenatal syphilis screening among pregnant women with equal financial access to prenatal care through Medicaid. METHODS: We used Florida claims data to examine any, early, and repeat screening among non-Hispanic white, non-Hispanic black, and Hispanic women with Medicaid-covered deliveries in FY1995 (n=56,088) and FY2000 (n=54,0…

Impact of a statewide Medicaid managed care system on healthcare utilization and outcomes for people living with HIV.

BACKGROUND: Medicaid managed care (MMC) systems provide insurance for many persons living with HIV and AIDS (PLWH). This study sought to assess the impact of a statewide MMC system (TennCare) on healthcare utilization and outcomes for PLWH. METHODS: A retrospective longitudinal analysis of trends in population characteristics was performed. The study population included all Tennessee PLWH identified by State Health Department, enrolled for 1 year or longer in Medicaid (1992-1993) or TennCare (1994-1997). Main outcome measures included health care utilization, incidence of opportunistic infect…

A New Approach to Identifying Avoidable Emergency Department Visits Among High-Need High-Cost Medicaid Patients.

Patients with high need and high costs (HNHCs) are frequent users of emergency departments (EDs), and some of their visits may be avoidable with better access to primary care. A better understanding of the reasons for ED use of HNHC patients can inform cost containment and quality improvement efforts. Our goal was to identify reasons for ED visits based on categories of services received by HNHC patients during the visit and to examine the likely role of different categories of primary care providers (PCPs) in reducing potentially preventable visits. We used the 2018 California Medicaid enroll…

Medicaid/CHIP Income Thresholds And Health Services Use Among Children In Low-Income Households, 2016-23.

Children in low-income households face health care barriers shaped by state Medicaid and Children's Health Insurance Program (CHIP) income eligibility thresholds. Using 2016-23 National Survey of Children's Health data (N = 124,250) merged with state data, we assessed associations between income thresholds and binary outcomes: insurance status, preventive care use, and unmet medical needs. A 1-standard-deviation increase in a state's income threshold (mean: 260 percent of the federal poverty level) was associated with a 1.10-percentage-point increase in consistent insurance and 0.78-percentage…

An Innovative Approach to Enhanced Care Management for High-Need Pediatric Medicaid Members: Retrospective Cohort Study.

BACKGROUND: The California Advancing and Innovating Medi-Cal (CalAIM) initiative supports Enhanced Care Management (ECM) for high-need pediatric populations but published evidence of the impact of ECM in pediatric populations is lacking. OBJECTIVE: We evaluated a novel multidisciplinary care model (Pair Team) for delivering ECM services, focusing on implementation and early outcomes for children and adolescents enrolled in California's Medicaid program (Medi-Cal). METHODS: We conducted a retrospective, observational cohort study of Medi-Cal-enrolled children and adolescents who enrolled in P…

GLP-1 Receptor Agonist Coverage for Obesity in Medicaid and Total GLP-1 Receptor Agonist Utilization.

IMPORTANCE: Glucagon-like peptide-1 receptor agonists (GLP-1 RAs) are effective for treating obesity, but few Medicaid programs cover them due to cost concerns with accelerating utilization. However, potential substitution effects between obesity-labeled GLP-1 RAs and diabetes-labeled GLP-1 RAs, which have identical active ingredients and which all states cover, are not well understood. OBJECTIVE: To determine whether Medicaid coverage of obesity-labeled GLP-1 RAs is associated with obesity-labeled, diabetes-labeled, and total GLP-1 RA utilization. DESIGN, SETTING, AND PARTICIPANTS: This cro…

Medicaid reimbursement for community violence intervention and prevention (CVI): a multi-state policy implementation case study.

BACKGROUND: Violence has fluctuated in the United States in recent years. Additionally, policing practices have been challenged, especially in neighborhoods of color. Community Violence Intervention (CVI) programs have emerged as an effective policy to address violence through neighborhood-centered resources, trauma-informed care, and credible messengers, without full reliance on law enforcement officials. However, inconsistent funding challenges the feasibility and sustainability of these programs. In 2021 several states introduced policies to allocate Medicaid reimbursement for CVI services …

U.S. Federal and State Medicaid Spending: Health Policy Patterns by Political Party Leadership and Census Demographics.

Medicaid is a vital public health program, serving over 70 million Americans from many backgrounds. Understanding how Medicaid spending varies by political leadership and demographic factors can inform policy discussions and advocacy efforts. We conducted a cross-sectional descriptive analysis of state Medicaid spending using publicly available data. Our findings show that individuals identifying as White comprise the largest single racial group of Medicaid beneficiaries both nationally and in most states. While the 2020 Census Diversity Index correlated strongly with total Medicaid spending, …

Pain disparities among Medicaid beneficiaries with chronic low back pain: The differential role of psychological distress.

This study examined associations between Medicaid status and pain-related experiences in adults with chronic low back pain (cLBP), and examined whether the association attenuated after adding psychological distress. This cross-sectional analysis used baseline data from 751 adults with cLBP (112 Medicaid, 639 non-Medicaid) from a multi-site clinical trial across three healthcare systems. Linear regressions were used to assess associations between Medicaid status and pain outcomes (intensity, physical function, interference), controlling for demographic and clinical covariates including race, fo…

Changes in coverage, access, and health status among adults with cardiovascular disease after medicaid work requirements.

Policymakers have intensified calls to expand work requirements in Medicaid across the United States, which could have implications for low-income adults who experience a high burden of cardiometabolic risk factors and disease. In this difference-in-differences analysis, we found that the implementation of Medicaid work requirements was associated with decreased health insurance coverage, no change in employment status, and a trend towards worse access to care. Our findings suggest that the expansion of work requirements could have major implications for the cardiovascular health of working-ag…

Does Medicaid cover artificial urinary sphincter and male urethral sling surgery?-a state-by-state analysis.

BACKGROUND: Information regarding the Medicaid coverage of artificial urinary sphincter (AUS) and male suburethral slings (MS) placement in the United States (US) is not readily available. In this manuscript, we seek to elucidate the state-by-state Medicaid coverage of these two procedures in the US. METHODS: State Medicaid websites were utilized to access publicly available physician fee schedules for the calendar year 2020. Fee schedules were searched for current procedural terminology (CPT) codes. CPT codes representing insertion of tandem cuff, insertion of AUS, removal of AUS, removal an…
