Synovial mesenchymal stem cells effectively alleviate osteoarthritis through promoting the proliferation and differentiation of meniscus chondrocytes.

OBJECTIVE: To investigate the relationship between the meniscal defect area and OA progression and explore the effect and mechanism of SMSCs cell therapy in knee osteoarthritis (OA) rat model. MATERIALS AND METHODS: For animal experiments, knee osteoarthritis (OA) model was constructed in Sprague Dawley (SD) rats by removing the medial meniscus of the right knee. Synovial mesenchymal stem cells (SMSCs) were engrafted by injecting into the right knee cavity. For in vitro experiments, CCK-8 assay was performed to evaluate the proliferation and differentiation of BMSCs and ATDC5 cells after co-c…

Biomechanical characterization of a novel collagen-hyaluronan infused 3D-printed polymeric device for partial meniscus replacement.

The menisci transmit load by increasing the contact area and decreasing peak contact stresses on the articular surfaces. Meniscal lesions are among the most common orthopedic injuries, and resulting meniscectomies are associated with adverse polycaprolactone contact mechanics changes and, ultimately, an increased likelihood of osteoarthritis. Meniscus scaffolds were fabricated by 3D-printing a network of circumferential and radial filaments of resorbable polymer (poly(desaminotyrosyl-tyrosine dodecyl ester dodecanoate)) and infused with collagen-hyaluronan. The scaffold demonstrated an instant…

Biomechanical comparison between suture anchor and transtibial pull-out repair for posterior medial meniscus root tears.

BACKGROUND: Posterior medial meniscus root (PMMR) tears have a serious effect on knee joint biomechanics. Currently used techniques for refixation of the PMMR include the transtibial pull-out repair (TP) and suture anchor repair (SA). These techniques have not been compared biomechanically. HYPOTHESIS: The SA technique provides superior biomechanical properties compared with the TP technique. STUDY DESIGN: Controlled laboratory study. METHODS: A total of 24 fresh-frozen porcine tibiae with attached intact medial menisci were used. The specimens were randomly assigned to 3 groups (8 specimen…

TIMP2 deficient mice develop accelerated osteoarthritis via promotion of angiogenesis upon destabilization of the medial meniscus.

Vascular invasion into the normally avascular articular surface is a hallmark of advanced osteoarthritis (OA). In this study, we demonstrated that the expression of tissue inhibitor of metalloproteinases-2 (TIMP2), an anti-angiogenic factor, was present at high levels in normal articular chondrocytes, and was drastically decreased shortly after destabilization of the medial meniscus (DMM). We also investigated the anti-angiogenic properties of TIMP2 via knockout. We hypothesized that the loss of TIMP2 could accelerate osteoarthritis development via promotion of angiogenesis. Loss of TIMP2 led …

Results of open meniscus repair. Long-term follow-up study with a matched uninjured control group.

We have followed for 13 years a consecutive series of 31 patients who had open repair of a torn meniscus. They were between 13 and 43 years of age at the time of operation and all had intact stabilising ligaments. Comparison was made with a matched group of normal subjects of similar age and level of activity. The total rate of failure after meniscal repair was 29%; three of the repaired menisci did not heal and six reruptured during the follow-up period. At follow-up 80% of the patients had normal knee function for daily activities. Radiological changes were found in seven. Two had reduction …

Comparable Midterm Survival After Medial Meniscus Root Repair for High and Low Intraoperative Outerbridge Classification Grades.

BACKGROUND: While medial meniscus posterior root repairs (MMPRRs) have been shown to reverse harmful biomechanical effects of medial meniscus posterior root tears, it is questionable whether meaningful improvement occurs in patients with advanced chondral loss. PURPOSE/HYPOTHESIS: The objectives of this study were to compare midterm survival of MMPRR and to determine patient-reported outcomes (PROs) after MMPRR based on intraoperative Outerbridge grades (0-2 vs 3 and 4). It was hypothesized that patients undergoing MMPRR with Outerbridge grades 3 and 4 would have a decreased rate of MMPRR sur…

Arthroscopic Medial Meniscus Posterior Root Repair and Centralization With Medial Capsulodesis Using a Knotless Soft Anchor.

Meniscal extrusion after posterior root tears of the medial meniscus disrupts the knee hoop-stress mechanism and increases joint contact pressure, accelerating osteoarthritis. Even with an anatomic root repair, extrusion often persists. Meniscal centralization has been proposed as a complementary strategy to restore joint biomechanics and reduce stress on the repaired root. This technique combines transtibial pull-out repair of the medial meniscus posterior root with meniscal centralization and medial capsulodesis. A tibial tunnel is created for root fixation using a FiberWire suture, and cent…

Item-Specific Knee Injury and Osteoarthritis Outcome Score Characterization of Patients With Medial Meniscus Root Tear.

BACKGROUND: Most studies regarding medial meniscus posterior root tear (MMPRT) report total Knee injury and Osteoarthritis Outcome Score (KOOS) subscale values as important patient-reported outcomes, but there are few symptom-specific characterizations of patients with MMPRT. PURPOSE/HYPOTHESIS: The purpose of this study was to characterize the prevalence and severity of symptoms and functional limitations among patients with MMPRT based on item-level KOOS responses. It was hypothesized that patients with MMPRT would show similar symptoms to those of other meniscal tear types, with items from…

How to Avoid Iatrogenic Saphenous Nerve Injury During Outside-In or Inside-Out Medial Meniscus Sutures.

Medial meniscal sutures using outside-in or inside-out techniques are very popular. Nevertheless, a saphenous nerve entrapment may occur with these techniques. This Technical Note describes a modified outside-in medial meniscus suture without any additional skin incisions. By dissecting the space between the subcutaneous and the capsule, which is performed by grasping and knotting the sutures through the anteromedial portal, iatrogenic saphenous nerve injury is avoided.
