We studied load-related predictors (handedness, occupation and workload history) of cortical bone mass as estimated by the metacarpal cortical index (MCI). OBJECTIVE: To study the effect of two different absorbable intramedullary nails in treatment of metacarpal and phalanx fractures.
METHODS: From January 2002 to December 2002, open reduction and internal fixation were done with two different absorbable intramedullary nails in 60 cases of metacarpal and phalanx fractures (52 cases of open fractures and 8 cases of closed fractures). Carpometacarpal dislocations are rare. In most cases, the dislocation is caused by a violent injury, e.g. the crash of a motorcyclist or a fall from great height. A considerable swelling of the back of the hand may mask the characteristic lump at the root of the hand. The diagnosis can be easily missed due to serious associated injuries. Twelve metacarpals and two phalanges in 14 patients (nine males and five females) were lengthened between August 1992 and March 1999 by the callus distraction technique using a small external fixator (Orthofix). The indications were traumatic amputation (10 cases), aplasia (three cases) and hypoplasia owing to premature epiphyseal closure of the metacarpal (one case). A patient with multiply recurrent giant cell reparative granuloma of the third metacarpal is reported. Three prior excisions failed to prevent recurrence. A wide resection and replacement with a nonvascularized fibular bone graft resulted in elimination of the tumor at the 7-year follow-up visit.

The concept of early motion is valuable in the treatment of fractures of the hand and wrist. It is a means to an end, however, and should be employed principally when the benefits outweigh the risks. The goal is a hand that functions as near to normally as possible, considering the injury. All treatments need to be tailored to the individual patient.

A thermoplastic fracture-brace was compared to closed reduction with ulnar gutter splinting in the treatment of 38 isolated fractures of the fifth metacarpal with angulation more than 40 degrees at the neck or 25 degrees in the diaphysis. To evaluate the effect of human growth hormone (hGH) on the growth of children with achondroplasia, three patients with achondroplasia and one patient with hypochondroplasia were treated with 0.5 IU/kg/W of pituitary-extracted hGH for 6-12 months. Seventeen patients had 21 bone graft operations as part of the overall delayed primary management of hand and wrist wounds. All bone graftings were performed within 10 days of injury. Internal or external fixation was used in all cases, and all wounds healed without infection. Complications included a fibrous union at one bone graft juncture and one malunion. From an experience of 120 fractures of the base of the first metacarpal bone, the authors evaluated the functional results of in 40 patients followed-up. Most of these patients had been treated by Bunden's method of longitudinal pinning. If one eliminates the comminuted fractures and the Bennett's fractures with a single large fragment treated by screw fixation the method of longitudinal pinning restores normal function to the first interdigitalspace Furthermore, the development of secondary arthrosis of the trapezo-metacarpal joint is rare.

Deformities of the hand and wrist in 26 upper limbs of 20 patients with ulnar deficiency were studied. In eight limbs the ulna was totally absent. In the remainder it was severely or moderately hypoplastic. Abnormalities of the digits were present in all affected limbs. There was ectrodactyly in 25 hands. Pacman dysplasia has been previously reported to be a lethal skeletal dysplasia with epiphyseal stippling and osteoclastic overactivity. We report on a sibling of a fetus previously reported as Pacman dysplasia. This infant has a clinical course consistent with mucolipidosis type II (I-cell disease) along with confirmatory biochemical, cytologic, and radiographic evidence. The deep branch of the ulnar nerve may be injured at the same time as fracture dislocations of the fifth metacarpal. We describe injuries to that nerve in two patients with dislocated fractures of the second or third and fourth metacarpals. The fractures were treated by open reduction and fixation with plates and screws, and it is possible that the repositioning and fixing caused the injury.
