Evaluating the quality and reliability of youtube videos on tympanostomy tubes: a comprehensive analysis for patients and parents.

BACKGROUND: The increasing use of online platforms as sources of medical information has led to a growing concern regarding the accuracy, reliability, and quality of the content available. YouTube, one of the most widely accessed platforms, serves as a prominent source for health-related information, including topics like tympanostomy tubes. However, the variability in the quality of uploaded videos and the lack of standardization raise questions about their utility for patient education and decision-making. This study aims to evaluate the quality and reliability of the information provided by

Myringotomy tube placement: understanding the impact of the coronavirus disease 2019 pandemic.

OBJECTIVE: Otolaryngologists perform bilateral myringotomy and tube placement for surgical management for otitis media with effusion. This retrospective study aimed to address the extent to which the coronavirus disease 2019 pandemic and season impact the number of bilateral myringotomy and tube placement procedures performed at a tertiary care centre. METHODS: A total of 1248 charts of children who underwent bilateral myringotomy and tube placement from January 2018 through February 2021 were reviewed. RESULTS: The cohort included 41.6 per cent females and 58.4 per cent males, with 63.7 per

Need for Tympanostomy Tubes in Children With Recurrent Acute Otitis Media Without Middle Ear Effusion.

OBJECTIVE: Children with recurrent acute otitis media (RAOM) presenting without middle ear effusion (MEE) do not meet indications for surgical intervention as outlined by Clinical Practice Guidelines (CPGs). The objective of this study was to determine which patients presenting with RAOM without MEE ultimately received tympanostomy tubes. STUDY DESIGN: Case series. SETTING: Single academic pediatric otolaryngology clinic. METHODS: Children (0-12 years) presenting with RAOM and no MEE were identified from October 2017 to December 2019. As per CPGs, no surgery was offered initially. Patients

[Effectiveness of baloon dilation of Eustachian tube in children with otitis media with effusion].

UNLABELLED: Otitis media with effusion is one of the most common ENT diseases in childhood. Absence of acute symptoms, prevalence of pathology among preschoolers, who often cannot complain on discomfort in the ears and hearing loss, lead to late diagnosis and treatment. Standard surgery is highly effective, but it is not able to help all patients. A new, minimally invasive technique of surgical treatment of otitis media with effusion - balloon dilation of the Eustachian tube provides additional opportunities in solving these problems. OBJECTIVE: To evaluate the effectiveness and possibilities

Tympanostomy tubes in the age of quarantine.

OBJECTIVES: After state-mandated COVID-19 quarantine measures were lifted in 2020, pediatric otolaryngologists noticed that many children who were previously scheduled for tympanostomy tube (TT) placement for indications of acute otitis media (AOM) or chronic middle ear effusions (OME) no longer required surgery. This study aims to describe the effect of home-quarantine on pediatric patients with recurrent AOM and OME to increase our understanding of these conditions. METHODS: This was a retrospective review of pediatric patients that were originally scheduled for TT for recurrent AOM and/or

Middle ear effusion prevalence at time of tympanostomy before and during COVID-19 pandemic.

BACKGROUND: Management of pediatric otitis media with effusion (OME) and recurrent otitis media typically includes observation up to 3 months. Bilateral myringotomy and tube (BMT) placement is performed due to persistent effusion with associated symptoms such as decreased hearing. With the COVID-19 pandemic and mandatory stay at home orders (MSHO), children were quarantined at home and many remained home after MSHO. We reviewed the prevalence of middle ear effusion (MEE) at the time of BMT during similar time periods in the year before, during and after MSHO in this pandemic year. STUDY DESIG

A high-resolution computed tomography based scoring system to predict ease of electrode insertion in cochlear implantation.

OBJECTIVE: This study aimed to formulate a scoring system based on high-resolution computed tomography scans to predict ease of electrode insertion during cochlear implantation via posterior tympanotomy in paediatric patients. METHOD: A scoring system Cochlear Implantation Radiological Assessment Score (CIRAS) was formulated based on six parameters. This score was correlated with intra-operative findings, and receiver operating characteristic analysis was performed to determine the optimal cut-off score to predict difficulty of surgery and to establish the inherent validity of the scoring sys

Otolaryngeal and Oropharyngeal Conditions: Common Ear Conditions.

Ear pain (otalgia) is a common reason for visits to family physician offices and emergency departments. Otalgia is primary when the pathology originates from the ear, and is secondary for disease processes associated with otalgia and an ear examination with normal findings. The most common diagnosis related to otalgia in children and adults is acute otitis media (AOM). It is characterized by an erythematous, bulging, and cloudy tympanic membrane. Otitis media with effusion is the presence of fluid behind the tympanic membrane without signs of inflammation. Chronic middle ear effusion is manage

Change in Initial Tympanostomy Tube Placement Timing Yields Decreased Burden of Care Without Increased Complications in Patients With Cleft Lip and Palate.

OBJECTIVE: To report a practice audit of the consequences of a change in protocol in the timing of placement of tympanostomy tubes in infants with cleft lip and palate. PARTICIPANTS: All children with a diagnosis of cleft lip and palate, treated between November 1998 and May 2006 under the old protocol, and between December 2012 and July 2016 under a new protocol. Under the old protocol, tympanostomy tubes were first inserted at the time of lip repair at around age 2 months. Under the new protocol, tympanostomy tubes were deferred until the time of palate repair around the age of 9 months. Ch

Barriers to timely tympanostomy tube placement in trisomy 21.

OBJECTIVES: Trisomy 21 is the most common chromosomal abnormality in the United States with an incidence of 1 in 700 in live-born children. Children with trisomy 21 are at increased risk for chronic ear disease, as well as other medical issues that may complicate healthcare access. In this study, we investigate whether children with trisomy 21 experience delays in tympanostomy tube insertion, and if there are any factors that contribute to a delay in care. METHODS: We performed a retrospective analysis on patients with trisomy 21 undergoing tympanostomy tube insertion at Boston Medical Center

Differentiating Acute Otitis Media and Acute Mastoiditis in Hospitalized Children.

Acute otitis media is a common infection in children. Most acute otitis media episodes can be treated at an outpatient setting with antimicrobials, or only expectant observation. Hospital treatment with parenteral medication, and myringotomy or tympanostomy, may be needed to treat those with severe, prolonged symptoms, or with complications. The most common intratemporal complication of acute otitis media is acute mastoiditis. If a child with acute mastoiditis does not respond to this treatment, or if complications develop, further examinations and other surgical procedures, including mastoide

Quality of life after surgery for recurrent otitis media in a randomized controlled trial.

BACKGROUND: Tympanostomy with or without adenoidectomy is effective in preventing recurrences of acute otitis media (RAOM), but little is known about the effect of these operations on the quality of life (QOL). We evaluated the efficacy of insertion of tympanostomy tubes with and without adenoidectomy for improving QOL in young children in a controlled, randomized trial. METHODS: QOL was evaluated in the 159 children aged 10 months to 2 years participating in our larger study in which children with RAOM were randomly assigned to receive tympanostomy tubes, tympanostomy tubes with adenoidectom

[Grommets (ventilation tubes) for recurrent acute otitis media in children--assessment of a Cochrane review].

A recently published Cochrane review on grommets (ventilation tubes) for recurrent acute otitis media in children included only two studies involving 148 children. The review showed that ventilation tube insertion led to a mean reduction of approximately one episode of acute otitis media in the first six months after treatment. We examined three excluded studies and found that their results were comparable even if all five studies had different designs. Clinicians should consider the possible adverse effects of grommet insertion before performing the surgery.

Anesthetic management of a patient with asymptomatic ventricular tachycardia.

There are many causes of ventricular arrhythmias in pediatric patients, even those with structurally normal hearts. However, in young patients with 'normal' hearts, sustained ventricular arrhythmias are relatively rare. The primary concern of the physician is to identify which patients have benign ventricular arrhythmia patterns and which are at risk for sudden cardiac death. Even in asymptomatic patients, the choice of anesthetic agents may be important to minimize precipitation of episodes of tachyarrhythmias. This clinical report describes the anesthetic considerations for an asymptomatic c

Surgical strategies for otitis media.

OBJECTIVE: As we enter the new millennium, Otitis Media remains the most common cause for an acute care visit to the pediatrician. This paper discusses the various surgical approaches to children who do not respond to medical management for acute otitis, recurrent acute otitis and for chronic otitis media. In addition to discussing the relevant alternatives of intervention, attention is directed to special populations of patients who are likely to demand a great deal of care, the problems that are encountered with typanostomy tubes and the concept of a new era of a "post-tympanostomy tube lega

The mini-grommet and tympanosclerosis: results at two years.

One hundred and sixteen children with otitis media with effusion (OME) underwent surgery with grommet insertion. A conventional Shah grommet was used in one ear, and a Mini-Shah grommet in the other. Final review of the subjects two years after surgery revealed a significantly lesser degree of tympanosclerosis in the ear into which the Mini-Shah grommet had been inserted. This benefit might have resulted from the lesser mass of the mini-tube or its shorter duration in situ.

Bacteriological aspects of swimming with grommets.

A prospective bacteriological and symptomatic study was carried out on 50 swimmers and 50 non-swimmers who had grommets inserted for secretory otitis media. The risk of developing otorrhoea with positive bacterial pathogens was similar for swimmers (7.9%) and non-swimmers (8.0%). Subdividing the non-swimmers into 'wetters' and 'strictly dry' there was a similar risk of ear symptoms and pathogens. All children who swim before the insertion of grommets should continue to do so postoperatively with no added risk of ear infections.
