ON THE MECHANISM OF MILK SECRETION : THE INFLUENCE OF INSULIN AND PHLORIDZIN.

The four types of experiments on milk secretion herein described really fall into one general class so far as the physiological effects produced are concerned. Starvation lowers the blood sugar and raises the osmotic pressure of the blood. The experiment using parathyroid hormone with or without starvation may have its effects interpreted as simply due to starvation since 1000 units of this hormone produced no visible effects on the blood calcium or milk constituents different from those of starvation. Since insulin produces a marked and rapid drop in blood sugar it too may be looked upon as a…

IgA nephropathy-associated breast milk B cell alterations.

BACKGROUND: IgA nephropathy (IgAN) is a mucosal immune-associated disease characterized by the overproduction of galactose-deficient IgA1 (Gd-IgA1) and formation of nephritogenic immune complexes. Epstein-Barr virus (EBV), which preferentially infects IgA+ B cells, has been implicated in IgAN pathogenesis, although its role remains unclear. Given mucosa involvement, we characterized breast milk B cells as available representatives of mucosal tissue to better understand the pathogenesis of IgAN. METHODS: We performed a phenotypic analysis of B cells (CD19+), plasmablasts (CD38+), and CD138+ ce…

Sirolimus excretion in breast milk: a preliminary investigation.

BACKGROUND: Sirolimus is a mammalian target of rapamycin inhibitor that has been used in solid organ transplantation and is now increasingly being administrated to treat various manifestations of Tuberous Sclerosis (TSC). Lack of data regarding its safety during breastfeeding has led to its contraindication by pediatric and breastfeeding guidelines. METHODS: Levels of sirolimus were evaluated in breast milk samples collected during 24 h on the 10th and 15th day postpartum, by a lactating TSC patient chronically medicated with 4 mg of sirolimus per day. Controls were healthy lactating women. S…

Antacid Abuse - A Case of Milk-Alkali Syndrome.

PRESENTATION: A 42-year-old man presented with a four-week history of nausea, vomiting, and abdominal pain, ultimately attributed to chronic overuse of calcium carbonate tablets for gastroesophageal reflux disease (GORD). DIAGNOSIS: Diagnostic evaluation revealed hypercalcaemia, metabolic alkalosis, and acute kidney injury, necessitating intravenous fluid administration and cessation of calcium carbonate tablet consumption. Comprehensive investigations excluded alternative aetiologies, confirming Milk-Alkali Syndrome (MAS). TREATMENT: Management involved cessation of calcium carbonate intake…

Infantile Colic: When to Suspect Cow's Milk Allergy.

Background/Objectives: Worldwide, an estimated 20-30% of infants suffer from infant colic (IC), with excessive crying and unsettled behavior, during the first three months of life. These infants are often referred for a medical evaluation, but the pathogenesis of IC remains poorly understood. The aim of this narrative review is to critically appraise the available literature regarding the relation between IC and cow's milk allergy (CMA). Methods: A literature search using the search strings cow's milk allergy [MeSH Terms] OR food allergy [MesH Terms] AND colic [MeSH Terms] OR crying [MeSH Term…

Human Breast Milk Extracellular Vesicles Mitigate Endothelial Dysfunction.

Background: Endothelial cell (EC) dysfunction is an early sign of compromised vascular integrity and is associated with various cardiovascular diseases (CVDs). Activation of Toll-like receptor 4 (TLR4) plays a central role in this process. Extracellular vesicles (EVs) derived from milk have known anti-inflammatory properties, particularly in suppressing TLR4 activation. This study investigates the therapeutic potential of human breast milk-derived EVs (HBM-EVs) in mitigating EC dysfunction related to CVDs. Methods: HBM-EVs were isolated from the breast milk of healthy nursing mothers using ult…

Dysphoric Milk Ejection Reflex: Prevalence, persistence, and implications.

BACKGROUND: Dysphoric Milk Ejection Reflex (D-MER), characterized by an abrupt wave of negative emotions during milk-letdown, affects a relevant proportion of lactating parents and may significantly impact feeding choices, but remains poorly understood. METHODS: We conducted a cross-sectional study using an online survey of lactating parents of children 18 months and younger who were nursed for any period of time. RESULTS: The prevalence of D-MER in our cohort was 14.2 % (n = 209/1469). Among the 209 women who reported having experienced D-MER, the most frequently selected descriptions were:…

Neon Pink Breast Milk Stain in the NICU.

Pink discoloration in breast milk is a rare phenomenon arising from various benign or pathological causes. Serratia marcescens, a gram-negative bacterium known for its characteristic red pigment prodigiosin, is a notable cause. This case report examines the clinical approach, diagnostic workup, microbiological findings, and management strategies for a neonate presenting with neon pink stains in breast milk. A female neonate born at 30+5 weeks gestation via repeat cesarean section was admitted to the neonatal intensive care unit (NICU). On day of life (DOL) 28, bright pink staining was observed…

Unique Aspects of Managing Milk and Egg Allergies.

Milk and egg allergies are of critical importance due to their high prevalence and rising epidemiology. Although they are commonly outgrown, they can be severe, and the foods are difficult to avoid. Nutritional status is compromised by avoiding milk. There are important differential diagnoses for milk allergy, including nonimmunoglobulin E-mediated allergies and lactose intolerance. Milk and egg allergies can be managed by allowing baked forms of these proteins into diets with attention to risks and benefits. Milk and egg can be in various medications although for most products their safety in…
