"Like a weight off my shoulders": Participating in peer support from the perspectives of young people experiencing homelessness.

The aim of this study is to explore how young people experiencing homelessness or housing instability experience participating in peer support from paid peer support specialists with shared lived experiences. The present study used participatory qualitative methods, specifically a photovoice project with young people (N = 8) experiencing homelessness or housing instability, to understand their experiences of being in a relationship with a peer support specialist. This study found that young people experiencing homelessness or housing instability experienced peer relationships as uniquely carin

Spatially confined sub-tumor microenvironments in pancreatic cancer.

Intratumoral heterogeneity is a critical frontier in understanding how the tumor microenvironment (TME) propels malignant progression. Here, we deconvolute the human pancreatic TME through large-scale integration of histology-guided regional multiOMICs with clinical data and patient-derived preclinical models. We discover "subTMEs," histologically definable tissue states anchored in fibroblast plasticity, with regional relationships to tumor immunity, subtypes, differentiation, and treatment response. "Reactive" subTMEs rich in complex but functionally coordinated fibroblast communities were i

Well being, work environment and work accidents.

We examine factors that influence accident proneness among employees. We agree that the determinants of accident proneness include organizational, emotional and personal factors. Using logistic regression we estimated three models, and their predictability for accident proneness among sample of 200 injured workers interviewed upon entering hospital emergency wards in Israel. Work injuries were not contingent on age, religion, nor education. The effects of gender were strong but non-significant. Subcontracted and higher-paid workers are more likely to get repeat injuries. Prior injury experienc

Modeling dopamine system dysfunction in experimental animals.

Quite a substantial number of human disorders have been associated with a primary or a secondary impairment of one or several of the dopaminergic pathways. Among disorders associated with a primary impairment of dopaminergic transmission are Parkinson's disease, striatonigral degeneration, progressive supranuclear palsy, and possibly schizophrenia. Diseases of secondary dopamine dysfunction are chiefly represented by Huntington's disease in which dopaminergic transmission is being interrupted by progressive loss of the striatal neurons bearing the postsynaptic D1- and D2-dopamine receptors. A community-focused rural academic health department model in North Carolina: elevating community engagement, rural public health practice and research.

INTRODUCTION: In decentralized public health systems, rural public health departments have fewer economic resources and higher needs for public health programs and services. This case study describes the evolution of a rural district health department's community and academic partnerships across three phases. Throughout this process, partnerships grounded in community engagement informed the local public health workforce, translated research into practice, and supported the design, implementation, and evaluation of interventions in a rural setting. METHODS: Our approach is guided by three est

Interoperability of Health Information Systems in Low and Middle-Income Countries (LMIC): Implementation of Cluster Care System.

Interoperability of Health Information Systems (HIS) is critical for improving healthcare delivery, decision-making, and resource management in Low- and Middle-Income Countries (LMICs). However, challenges such as fragmented systems, limited digital infrastructure, and the absence of standardized health records hinder progress. This study proposes the Cluster Care System (CCS), a modular, cluster-based framework designed to address these challenges. Its architecture includes local Cluster Information Systems, a Cluster Master Patient Index for unified patient identification, and a Cluster Ente

A mixed methods approach to exploring the moderating factors of implementation fidelity of the integrated chronic disease management model in South Africa.

BACKGROUND: Chronic care models like the Integrated Chronic Disease Management (ICDM) model strive to improve the efficiency and quality of care for patients with chronic diseases. However, there is a dearth of studies assessing the moderating factors of fidelity during the implementation of the ICDM model. The aim of this study is to assess moderating factors of implementation fidelity of the ICDM model. METHODS: This was a cross-sectional mixed method study conducted in two health districts in South Africa. The process evaluation and implementation fidelity frameworks were used to guide the

Serving transgender people: clinical care considerations and service delivery models in transgender health.

The World Professional Association for Transgender Health (WPATH) standards of care for transsexual, transgender, and gender non-conforming people (version 7) represent international normative standards for clinical care for these populations. Standards for optimal individual clinical care are consistent around the world, although the implementation of services for transgender populations will depend on health system infrastructure and sociocultural contexts. Some clinical services for transgender people, including gender-affirming surgery, are best delivered in the context of more specialised

[Ebola virus disease in West Africa and Germany : clinical presentation, management and practical experience with medevacuated patients in Germany].

Ebolaviruses are the causative pathogens of a severe form of viral haemorrhagic fever with cytokine induced shock and multi-organ failure and a high case fatality rate in humans (50-90 %, more than 70 % in the beginning of the current outbreak), designated Ebola haemorrhagic fever or Ebola virus disease (EVD). Ebola is endemic in regions of Central and West Africa. Ebolavirus Zaire (EBOV) is the most aggressive Ebola virus species and is causing the current epidemic. Currently, beginning in late 2013, an unprecedented epidemic with several thousand cases and deaths (as per WHO report 24.12.201

Community-based care coordination: practical applications for childhood asthma.

Care coordination programs have been used to address chronic illnesses, including childhood asthma, but primarily via practice-based models. An alternative approach employs community-based care coordinators who bridge gaps between families, health care providers, and support services. Merck Childhood Asthma Network, Inc. (MCAN) sites developed community-based care coordination approaches for childhood asthma. Using a community-based care coordination logic model, programs at each site are described along with program operational statistics. Four sites used three to four community health worker

Creation of nutrition support centers of excellence: is it truly an excellent idea?

In response to external pressures and events that threatened to adversely affect the field of bariatric surgery, the American Society for Metabolic and Bariatric Surgery created a mechanism for certifying surgeons and their facilities to guarantee that bariatric surgery would be performed with the greatest degree of patient safety. Those surgeons and facilities that satisfied the requirements were designated as Bariatric Surgery Centers of Excellence (BSCOE). That process involved creating an independent, not-for-profit corporation, establishing the best practice criteria for certification, an

In the shelter of each other: Respite Care for Students as a partnership model.

Many of today's nursing students live in the margin of society without family support. When this is compounded by demanding family responsibilities, the personal and academic consequences for students can be harsh. Although numerous student retention strategies have been clearly documented, Respite Care for Students is offered as a unique partnership model to sustain these students' enrollment, provide adequate time for their learning, and enhance their graduation rates.

Barriers and facilitators to implementation of evidence-based task-sharing mental health interventions in low- and middle-income countries: a systematic review using implementation science frameworks.

BACKGROUND: Task-sharing is a promising strategy to expand mental healthcare in low-resource settings, especially in low- and middle-income countries (LMICs). Research on how to best implement task-sharing mental health interventions, however, is hampered by an incomplete understanding of the barriers and facilitators to their implementation. This review aims to systematically identify implementation barriers and facilitators in evidence-based task-sharing mental health interventions using an implementation science lens, organizing factors across a novel, integrated implementation science fram

Scaling Up Caregiver Support for Autism in Public Health Care: Adoption, Implementation Pace and Sustainability of a Train-the-Trainer Model for the WHO Caregiver Skills Training in Italy.

Train-the-Trainer (TTT) models can scale parent-mediated interventions (PMIs) in public health care, yet evidence on large-scale implementation remains limited. This study reports the implementation of an adapted TTT model for the WHO Caregiver Skills Training (CST) within the Italian National Health Service, evaluating adoption, acceptability, feasibility and sustainability. Twenty-eight clinicians from 10 Regions undertook the four-phase TTT. A mixed-methods design combined surveys with focus groups, analyzed using the Consolidated Framework for Implementation Research (CFIR). Twenty trainee

Establishment of a microwell-array-based miniaturized thymic organoid model suitable for high-throughput applications.

T cell development depends critically on the thymic stroma-in particular, the diverse array of functionally distinct thymic epithelial cell (TEC) types. However, a robust in vitro thymus model mimicking the native thymus and compatible with medium-/high-throughput analyses is currently lacking. Here, we demonstrate a high-density microwell-array-based miniaturized thymus organoid (mTO) model that supports T cell commitment and development, possesses key organizational characteristics of the native thymus, and is compatible with live imaging and medium-/high-throughput applications. We establis

Caregiving and developmental factors differentiating young at-risk urban children showing resilient versus stress-affected outcomes: a replication and extension.

This study tested hypotheses from an organizational-developmental model for childhood resilience. In this model resilience reflects a child's mastery of age-salient objectives, in the face of substantial adversity, by drawing on internal and external resources that enhance processes of adaptation specific to each developmental stage. Interviews were conducted with parents of 122 7- to 9-year-old urban children exposed to multiple risk factors, 69 classified as resilient and 53 as maladjusted. Consistent with predictions generated by the model: (1) characteristics of a child's caregiving system

From stochastic systems to governed intelligence: a neurosymbolic path for pharma.

INTRODUCTION: Artificial intelligence is being deployed across the pharmaceutical value chain, yet returns have been inconsistent. One of the explanations for this inconsistency is the organizational focus on models rather than the data foundations needed to support reliable use. AREAS COVERED: This Critical Perspective argues that pharma's AI challenge is not primarily model capability, but rather representation and governance. The authors examine why model-centric deployment strategies underdeliver in semantically weak and operationally complex setting. The authors follow this by outlining
