National High Blood Pressure Education Program Working Group report on ambulatory blood pressure monitoring.

Blood pressure measurements obtained in a physician's office may not represent a patient's blood pressure during an entire day. Ambulatory blood pressure monitoring (ABPM) provides multiple readings over time. This review of the literature on ABPM addresses the current state of ABPM methods, normal blood pressure profiles, the clinical and research uses of ABPM, cost considerations, and recommendations for use of ABPM in selected circumstances. Current ABPM devices use either auscultatory or oscillometric methods to determine blood pressure. A rigorous comparison of these methods is needed to

Framework for patient-ventilator asynchrony during long-term non-invasive ventilation.

Episodes of patient-ventilator asynchrony (PVA) occur during acute and chronic non-invasive positive pressure ventilation (NIV). In long-term NIV, description and quantification of PVA is not standardised, thus limiting assessment of its clinical impact. The present report provides a framework for a systematic analysis of polygraphic recordings of patients under NIV for the detection and classification of PVA validated by bench testing. The algorithm described uses two different time windows: rate asynchrony and intracycle asynchrony. This approach should facilitate further studies on prevalen

A multi-modal wearable dataset for cognitive attention and task-based stress analysis.

Stress is a natural physiological response that often adversely affects physical and mental health. Wearable devices that offer real-time acquisition of physiological signals provide opportunities for personalised monitoring and effective management of stress. We present, CATSA, a multimodal dataset comprising physiological signals, including blood volume pulse, tri-axis acceleration, electrodermal activity, and average heart rate, collected from 50 participants for monitoring different types of artificially induced stresses from wearable devices. A structured protocol was established to induc

Deep Learning-Based Prediction of Heart Failure Readmission Using Longitudinal Cardiac Device Data.

BACKGROUND: Heart failure (HF) is a leading cause of hospitalization and readmission. Cardiac implantable electronic devices (CIEDs) continuously capture physiologic diagnostics that change during and after HF decompensation and may enable early post-discharge risk stratification. OBJECTIVES: To predict 60-day HF readmission using deep learning models trained on post-discharge temporal behavior of CIED diagnostic parameters. METHODS: We performed a retrospective analysis of patients with implantable cardioverter defibrillators (ICDs) or cardiac resynchronization therapy defibrillators (CRT-D

A Comprehensive Review of the Approach to Ultrasonography in Neonatal Shock: The Partnership Between Targeted Neonatal Echocardiography and Point-of-Care Ultrasound.

The neonatal cardiovascular system is dynamic and influenced by multiple factors, which makes neonatal shock a challenging early clinical diagnosis. Real-time monitoring has been limited to indirect measurements of hemodynamic well-being, such as heart rate, blood pressure, capillary refill time, and biochemical markers of adequate oxygen delivery and oxygen consumption. Bedside diagnostic ultrasound to guide decision-making has become increasingly integrated into modern neonatal care. It allows clinicians to enhance physiologic monitoring, differentiate among diverse cardiovascular phenotypes

Oxygen Targeting in Extremely Low Birth Weight Infants.

The use of supplemental oxygen plays a vital role in the care of the critically ill preterm infant, but the unrestricted use of oxygen can lead to unintended harms, such as chronic lung disease and retinopathy of prematurity. An overly restricted use of supplemental oxygen may have adverse effects as well. Ideally, continuous monitoring of tissue and cellular oxygen delivery would allow clinicians to better titrate the use of supplemental oxygen, but such monitoring is not currently feasible in the clinical setting. The introduction of pulse oximetry has greatly aided the clinician by providin

Gastroesophageal reflux disease in 2006. The imperfect diagnosis.

There continues to be significant controversy related to diagnostic testing for gastroesophageal reflux disease (GERD). Clearly, barium contrast fluoroscopy is superior to any other test in defining the anatomy of the upper gastrointestinal (UGI) tract. Although fluoroscopy can demonstrate gastroesophageal reflux (GER), this observation does not equate to GERD. Fluoroscopy time should not be prolonged to attempt to demonstrate GER during barium contrast radiography. There are no data to justify prolonging fluoroscopy time to perform provocative maneuvers to demonstrate reflux during barium con

Outpatient Administration of Chimeric Antigen Receptor T-Cell Therapy Using Remote Patient Monitoring.

Chimeric antigen receptor T-cell (CAR-T) therapies are standard of care for the treatment of several hematologic malignancies. Although patients receiving CAR-T therapies are frequently hospitalized given risks of cytokine release syndrome (CRS) and immune effector cell-associated neurotoxicity syndrome (ICANS), there is increasing interest and evidence for the safety of their outpatient administration. We review various models of care and provide operational considerations for centers that are interested in developing outpatient CAR-T programs, with a particular emphasis on using remote patie

Remote patient monitoring system combining hardware and artificial intelligence based software.

This study details the development of a remote patient monitoring system with a primary focus on a novel, customized Deep Neural Network (DNN) for arrhythmia detection. The system integrates hardware for real-time data collection from biomedical sensors, where IoT-based sensor data is collected and encrypted in a central database for subsequent analysis. The novelty of the work lies in the proposed AI-based software component rather than the hardware assembly, which utilizes accessible components. The developed system is designed to function as a decision support system for healthcare personne

Quantitative Pupillometry for Intracranial Pressure (ICP) Monitoring in Traumatic Brain Injury: A Scoping Review.

The neurological examination has remained key for the detection of worsening in neurocritical care patients, particularly after traumatic brain injury (TBI). New-onset, unreactive anisocoria frequently occurs in such situations, triggering aggressive diagnostic and therapeutic measures to address life-threatening elevations in intracranial pressure (ICP). As such, the field needs objective, unbiased, portable, and reliable methods for quickly assessing such pupillary changes. In this area, quantitative pupillometry (QP) proves promising, leveraging the analysis of different pupillary variables

Continuous In-Bed Monitoring of Vital Signs Using a Multi Radar Setup for Freely Moving Patients.

In hospitals, continuous monitoring of vital parameters can provide valuable information about the course of a patient's illness and allows early warning of emergencies. To enable such monitoring without restricting the patient's freedom of movement and comfort, a radar system is attached under the mattress which consists of four individual radar modules to cover the entire width of the bed. Using radar, heartbeat and respiration can be measured without contact and through clothing. By processing the raw radar data, the presence of a patient can be determined and movements are categorized into

TBscore: Signs and symptoms from tuberculosis patients in a low-resource setting have predictive value and may be used to assess clinical course.

We developed a clinical score to monitor tuberculosis patients in treatment and to assess clinical outcome. We used the WHO clinical manual to choose signs and symptoms, including cough, haemoptysis, dyspnoea, chest pain, night sweating, anaemia, tachycardia, lung-auscultation finding, fever, low body-mass index, low mid-upper arm circumference giving patients a TBscore from 0 to 13. We validated the score with data from a cohort of 698 TB patients, assessing sensitivity to change and ability to predict mortality. The TBscore declined for 96% of the surviving patients from initiation to end of

Measurement reliability of cardiovascular reactivity change scores: a comparison of intermittent and continuous methods of assessment.

Cardiovascular reactivity testing is a widely used measure of risk for hypertension and coronary heart disease. Evidence for the reliability of the measures, however, is weak. If the measures (usually change scores) are unreliable, the usefulness of testing is limited, since relationships with other measures, such as disease outcomes, may be obscured. This study examines the reliability of blood pressure change, using both a traditional method of assessment (Colin ABPM) which provides intermittent readings, and a non-invasive method that samples the complete distribution of pressures (Ohmeda F

Patient and environmental factors affecting ambulatory blood pressure monitoring.

Several patient and environmental factors may limit the applicability of ambulatory BP monitoring. In children and in elderly patients, erroneous readings may occur more frequently. Comparison of 25 elderly (greater than 65 yr) vs 25 young subjects recently monitored for clinical reasons in our hypertension clinic showed slightly higher error rates (19% vs 15% of readings) in the elderly but similar willingness to undergo monitoring again (96% vs 84% of subjects, respectively). The technique has not been well validated in obesity. Arrhythmias may prevent currently available ambulatory BP machi

Clinical use of home and ambulatory blood pressure monitoring.

Blood pressure measurements made in the physician's office with a mercury-column sphygmomanometer traditionally have been the standard for diagnosis of hypertension and determination of the efficacy of antihypertensive agents. The utility of this measurement is limited, however, by the characteristic variability of blood pressure; office blood pressure readings are not always reliable indicators of pressures occurring throughout the course of the day. Therefore, blood pressure measurements performed by patients or family members at home and automatic ambulatory blood pressure monitoring are tw

Acute effects of nimodipine on cerebral vasculature and brain metabolism in high grade subarachnoid hemorrhage patients.

BACKGROUND: Nimodipine is the only medication shown to improve outcomes after aneurysmal subarachnoid hemorrhage (SAH). Preliminary theories regarding the mechanism by which it prevents vasospasm have been challenged. The acute physiologic and metabolic effects of oral Nimodipine have not been examined in patients with poor-grade SAH. METHODS: This is an observational study performed in 16 poor-grade SAH patients undergoing multimodality monitoring who received oral Nimodipine as part of routine clinical care. A total of 663 doses of Nimodipine were observed. Changes in physiologic measuremen
