Cryoglobulinemia manifested by gangraene of almost all fingers and toes.

Cryoglobulinemia is a rather rare condition accompanying quite a broad spectrum of different states and diseases. Mixed or polyclonal cryoglobulins can be seen in patients with autoimmune disorders, chronic infections and lymphoproliferative disorders. Monoclonal cryoglobulins are often revealed in patients with multiple myeloma or Waldenström's macroglobulinemia. Cryoglobulinemia is in most cases asymptomatic. Cryoglobulinemic vasculitis is an immune complex-mediated systemic disorder involving mostly small, but sometimes also larger vessels. In this report, we describe a case of a patient pr

Risk for Acute Care Utilization in Housing-Insecure Adults: A National Study of a Hot Spotter Program in the US Veterans Health Administration.

BACKGROUND: To identify patients at the highest risk for acute care utilization, health care systems have developed "hot spotter" programs. Homelessness is a robust social determinant of acute care utilization. OBJECTIVES: To describe the prevalence, patterns, and correlates of meeting criteria for a hot spotter program among housing-insecure adults in the US Veterans Health Administration (VHA). RESEARCH DESIGN: Among veterans on the VHA Homeless Registry in Fiscal Years 2018-2022 (N=1,469,893), we identified those who met criteria for a Hot Spotter Report [ie, ≥1 hospital admissions and/or

A Community-Based Cross-Sectional Study to Assess the Prevalence of Polypharmacy and Potentially Inappropriate Medications among Elderly Patients in Urban Areas of Western Rajasthan.

BACKGROUND: The elderly population is increasing worldwide, including India. This population has multiple chronic conditions, leading to the prescription of multiple drugs called polypharmacy. This increases the risk of potentially inappropriate medications (PIMs). This study aimed to assess the prevalence of polypharmacy and PIM use among elderly individuals in urban western Rajasthan and identify associated factors. METHODS: A community-based cross-sectional study was conducted from April to June 2024 in Jodhpur, Rajasthan. A total of 265 elderly individuals (aged 60 and above) were selecte…

Secondary messenger signalling influences Pseudomonas aeruginosa adaptation to sinus and lung environments.

Pseudomonas aeruginosa is a cause of chronic respiratory tract infections in people with cystic fibrosis (CF), non-CF bronchiectasis, and chronic obstructive pulmonary disease. Prolonged infection allows the accumulation of mutations and horizontal gene transfer, increasing the likelihood of adaptive phenotypic traits. Adaptation is proposed to arise first in bacterial populations colonizing upper airway environments. Here, we model this process using an experimental evolution approach. Pseudomonas aeruginosa PAO1, which is not airway adapted, was serially passaged, separately, in media chemic

Early-life impairments, chronic health conditions, and income mobility.

Individuals who have congenital conditions or become disabled early in life tend to have poorer educational and occupational outcomes than non-disabled individuals. Disability is known to be a complex entity with multiple causations, involving, inter alia, physiological, social, economic, and cultural factors. It is established that social factors can influence educational and occupational attainment for disabled people, and current disability policy in many countries, particularly in the Global North, stress the importance of equality of opportunity. However, there is a scarcity of research t

Statistical models for the control phase of clinical monitoring.

The rise in the prevalence of chronic conditions means that these are now the leading causes of death and disability worldwide, accounting for almost 60% of all deaths and 43% of the global burden of disease. Management of chronic conditions requires both effective treatment and ongoing monitoring. Although costs related to monitoring are substantial, there is relatively little evidence on its effectiveness. Monitoring is inherently different to diagnosis in its use of regularly repeated tests, and increasing frequency can result in poorer rather than better statistical properties because of m

Income Inequalities and Risk of Early Rehospitalization for Diabetes, Hypertension and Congestive Heart Failure in the Canadian Working-Age Population.

OBJECTIVES: In the Canadian context of universal health-care coverage, income inequalities are understudied as potentially predictive of the timings and patterns of repeat hospitalizations for diabetes, despite this condition requiring self-care practices entailing appreciable out-of-pocket expenses in daily life. In this study, we examined the relationships between income disparities and risk of earlier readmission for diabetes and commonly comorbid chronic conditions in the working-age population. METHODS: The cohort study exploited 2006 population census data linked longitudinally to 3 yea

Oesophageal Obstruction from a Pharmacobezoar Resulting in Death.

Formation of an intestinal pharmacobezoar is a rare condition. It may form after intake of various oral pharmaceutical preparations of drugs, both as a result of an acute overdose and through chronic use of therapeutic doses of a drug. We report a case with a patient presenting with an oesophageal pharmacobezoar and complete obstruction of the oesophagus and severe toxic symptoms and death related to oral ingestion of multiple drugs.

[A case of liver abscesses in a bull].

During a farm visit a breeding bull was presented with signs of anorexia, somnolence, and loss of condition. These signs were chronic and slowly progressive. Clinical examination revealed an enlarged liver and subsequent ultrasonography revealed multiple abscesses in the liver. This was confirmed during post-mortem examination. Ultrasonography, proved to be of great diagnostic value, especially because it is a quick and non-invasive procedure.

Comorbid extra-intestinal central sensitization conditions worsen irritable bowel syndrome in primary care patients.

BACKGROUND: Irritable bowel syndrome (IBS) is characterized as a central sensitization syndrome (CSS), a group of conditions including fibromyalgia, chronic fatigue, and restless leg syndrome (RLS) among others with frequent comorbidities of anxiety, depression, and chemical sensitivity. The prevalence of comorbid conditions and their impact on IBS symptom severity and quality of life in rural community populations has not been described. METHODS: We administered a cross-sectional survey to patients with a documented CSS diagnosis in rural primary care practices to evaluate the relationship b

[Hidradenitis suppurativa and psoriasis. Therapeutic response to secukinumab].

Hidradenitis suppurativa and psoriasis are chronic inflammatory systemic dermatoses. The immune system plays a fundamental role in their pathogenesis, as each involves an immunological imbalance and the participation of numerous cytokines, including interleukin-17. The coexistence of these conditions can significantly impair quality of life, thus requiring integrated and targeted therapeutic approaches. We report the case of a 47-year-old woman diagnosed with hidradenitis suppurativa who developed psoriasis lesions over time. After receiving multiple therapeutic regimens without response, she

Use of a Sheath and Stylet for a Difficult Dorsal Root Ganglion Stimulation Lead Extraction: A Case Report.

Dorsal root ganglion stimulation (DRG-S) is a relatively new neuromodulation technique that has shown promising results in the treatment of chronic pain conditions. We present a case of a difficult lead extraction during the explantation of a DRG-S device. The lead was unable to be removed despite multiple attempts until a sheath and stylet were used to facilitate extraction. As DRG-S utilization becomes more widespread, DRG-S device explantation will inevitably become more common. The technique described in this report may be beneficial in certain cases of difficult DRG-S lead extraction.

When the Gut Tells a Story: Bezoars in a Neglected Autistic Child.

This case study delves into the unique presentation of bezoars in a 14-year-old autistic female who exhibited chronic diarrhea and abdominal pain. While trichobezoars, masses formed from ingested hair, are rare, they are predominantly seen in young females and are associated with psychiatric conditions. Through rigorous diagnostic procedures, including a computed tomography imaging of the abdomen and pelvis (CTAP) scan, fecal impaction, and multiple bezoars, including hair and non-biological items, were identified. The background revealed significant neglect, emphasizing the importance of a co

Myeloid-derived suppressor cells exert immunosuppressive function on the T helper 2 in mice infected with Echinococcus granulosus.

Cystic echinococcosis (CE) is a worldwide hazardous zoonotic parasitosis caused by Echinococcus granulosus. CE development involves complex immunological mechanisms, including participation of multiple immune cells and effector molecules. Myeloid-derived suppressor cells (MDSCs) are known to be involved in chronic and acute inflammatory conditions. In this study, we aimed to characterize the immune function of MDSCs in CE to improve the understanding, prevention and treatment of CE. Our results indicated that MDSCs overexpressing Ly6C and Ly6G inhibit the formation and activity of T helper 2 c

Update on medications with adverse skeletal effects.

Patients rely on their primary care physician to manage multiple, often chronic medical conditions that require prescription medications. Balancing the risk to benefit of treatments can be challenging and requires that the primary care physician stay abreast of new information regarding risks and benefits. The number of medications with reported adverse effects on skeletal health is expanding. This review focuses on medications recently added to the list of "bad to the bone" drugs and on recent advances in management of glucocorticoid-induced osteoporosis. A practical approach to assessing and

Acquired reactive perforating collagenosis associated with papillary thyroid carcinoma: a paraneoplastic phenomenon?

Acquired reactive perforating collagenosis (ARPC) is an uncommon dermatosis characterized by transepidermal elimination of altered collagen. It is commonly seen in patients with diabetes mellitus and/or chronic renal insufficiency. Rarely, it has been reported in association with malignancy and other conditions. We report a 30-year-old woman with insulin-dependent diabetes mellitus who presented with multiple, discrete, violaceous, hyperkeratotic papules on the extensor aspects of both legs, characteristic of ARPC. Four months later, metastatic papillary thyroid cancer was diagnosed. This case

An unusual patient with hypothyroidism, tophaceous gout, and marked joint destruction.

This report describes a 75-year-old Caucasian man with extensive urate deposits and severe gouty arthropathy that confined him to a wheelchair. Since age 50, he suffered multiple acute gout flares and progressive deformities in his hands, feet, knees, and elbows (tophi). Serum creatinine was 1.4 mg/dL and serum urate 9.4 mg/dL. Conditions known to increase uric acid production (psoriasis, chronic bronchitis) and to decrease uric acid excretion (hypothyroidism, metabolic syndrome, and nephroangiosclerosis) may operate in a single patient, illustrating the dramatic clinical course of untreated g

Is it a tic or Tourette's? Clues for differentiating simple from more complex tic disorders.

Tics are characterized by sterotyped, purposeless, and irregularly repetitive movements and usually can be classified as chronic motor or vocal tic disorders, transient tic disorders, or Tourette's syndrome. The latter is a complex disorder associated with multiple tics and often accompanied by other conditions, such as ADHD and obsessive-compulsive disorder. Treatment can be difficult, and drug therapy should begin with agents least likely to cause problems for the patient. Education of the patient and family and support from the physician and other care providers are essential elements of ef

Educational and behavioral strategies for improving medication compliance in juvenile rheumatoid arthritis.

Compliance with regimens for chronic conditions such as juvenile rheumatoid arthritis (JRA) is often poor, and the potential benefits of therapy are thus compromised. In this study a compliance intervention involving educational and behavioral strategies was shown effective in improving medication compliance for two of three patients with JRA. The intervention was introduced in a time-lagged fashion (multiple baseline design) with repeated measures of compliance. The strategies were less complex than other compliance interventions, such as token reinforcement strategies, and therefore would be
