Case Report: Aumolertinib response as a diagnostic clue for intrapulmonary metastases.

Lung adenocarcinoma, which is most common in non-smokers and women, rarely presents with diffuse pulmonary nodules. Distinguishing monoclonal intrapulmonary metastases (IPM) from polyclonal multiple primary lung cancers (MPLC) is essential for treatment planning. Although next-generation sequencing (NGS) accurately determines clonal origin, it can be costly and time-consuming. We report a lung adenocarcinoma case with diffuse solid nodules in a 52-year-old non-smoking female. Empirical treatment with aumolertinib resulted in rapid, homogeneous regression of lesions within four weeks, supportin

Cavitary Hodgkin Lymphoma of the Lung.

It has been shown that some cases of Hodgkin lymphoma (HL) may present with pulmonary parenchymal involvement usually in the form of multiple irregularly marginated pulmonary nodules. Other radiographic patterns such as consolidation, interstitial infiltrates, and cavitary lesions are less common. We present a case of HL, nodular sclerosis type, with pulmonary involvement presenting as a large cavitary consolidation and axillary and mediastinal lymphadenopathy. Initial diagnostic work-up including sputum culture, bronchoscopy, and a fine needle aspiration of lymph node was not conclusive favor

A case of atypical skull base osteomyelitis with septic pulmonary embolism.

Skull base osteomyelitis (SBO) is difficult to diagnose when a patient presents with multiple cranial nerve palsies but no obvious infectious focus. There is no report about SBO with septic pulmonary embolism. A 51-yr-old man presented to our hospital with headache, hoarseness, dysphagia, frequent choking, fever, cough, and sputum production. He was diagnosed of having masked mastoiditis complicated by SBO with multiple cranial nerve palsies, sigmoid sinus thrombosis, and septic pulmonary embolism. We successfully treated him with antibiotics and anticoagulants alone, with no surgical interven

Nodular regenerative hyperplasia of the liver associated with primary pulmonary hypertension.

Nodular regenerative hyperplasia (NRH), a rare hyperplastic condition of the liver, is reported in two patients with primary pulmonary hypertension (PPH). The first patient was a 26-year-old man who died of PPH and showed multiple NRH without cirrhosis of the liver. The second patient was a 25-year-old man who had a PPH with pulmonary arterial thrombi and NRH of the liver. NRH has been described in association with immune disease, hematopoietic disorder, and diabetes mellitus, so that NRH with PPH is considered to be very rare. Histologic findings of the lungs show typically plexogenic pulmona

Pulmonary Langerhans' cell histiocytosis: radiologic resolution following smoking cessation.

We describe two patients with histologically proven pulmonary Langerhans' cell histiocytosis in whom radiologic improvement occurred following smoking cessation. The patients had 23- and 25-pack-year smoking histories, respectively. High-resolution CT revealed multiple small nodules, located predominantly in the upper and middle lung fields. There was a close temporal relationship between smoking cessation and radiologic improvement.

Analysis of the risk factors associated with complications following CT-guided percutaneous core needle biopsy of small pulmonary nodules (≤1.5 cm): A single-center retrospective study.

The aim of this study was to identify the risk factors associated with complications following computed tomography (CT)-guided percutaneous core needle biopsy (PCNB) of small pulmonary nodules (≤1.5 cm). From August 2019 to May 2023, the baseline and procedural technique characteristics of 107 consecutive patients who underwent PCNB for small pulmonary nodules (≤1.5 cm) in our hospital were analyzed retrospectively. The predictors of procedure-related pneumothorax and pulmonary hemorrhage were determined using univariate and multivariate logistic regression analyses. The incidences of pneumoth

Acute respiratory failure mimicking acute respiratory distress syndrome due to parenchymal infiltration by metastatic melanoma.

Malignant melanoma is the most aggressive form of skin cancer and carries a predisposition for metastasis to many different organs. Pulmonary dissemination is common, most often presenting as multiple discrete pulmonary nodules. While a variety of other intrathoracic patterns can occur, diffuse parenchymal infiltration causing acute respiratory failure is an extremely rare manifestation of metastatic disease. We present a case of an otherwise healthy man who developed rapidly progressive respiratory failure mimicking acute respiratory distress syndrome due to melanomatous infiltration of the l

Conventional and diffusion-weighted MR imaging of intracranial tuberculomas. A case report.

Intracranial tuberculoma is a rare form of central nervous system tuberculosis. We here report on conventional and diffusion-weighted cranial MR images of a non-immunocompromised patient with multiple intracranial tuberculomas, tuberculous lymphadenitis and pulmonary tuberculosis. Conventional MR imaging revealed multiple ring-enhancing mass lesions. At follow-up MR, appearances of both edema and number and size of nodules were decreased. Diffusion-weighted MR was normal and normal ADC values were found in this case of tuberculomas.

Giant bullous emphysema successfully treated with percutaneous drainage followed by resection: A case complicated by lung cancer diagnosed by intraoperative biopsy.

We present a case of bilateral giant bullous emphysema (GBE) with rapidly progressive dyspnea. The dyspnea was thought to be due to tension bullae caused by the check valve mechanism in COVID-19 bronchitis. Multiple nodules were also detected on both sides of the lung. As the patient had poor pulmonary reserve for surgical bullectomy, we first performed percutaneous intracavitary drainage. Prior to this procedure, we placed a chest tube in the thoracic cavity to avoid tension pneumothorax. As a result, the patient's remaining lung expanded and respiratory status improved, allowing him to under

Pulmonary lymphomatoid granulomatosis in a 4-year-old girl: A case report.

BACKGROUND: Pulmonary lymphomatoid granulomatosis (PLG) is a lymphoproliferative disease associated with Epstein-Barr viral infection occurring mainly in adults and rarely in children. It is characterized by multiple pulmonary nodules. Its diagnosis depends on lung biopsy findings. Most patients are immunodeficient, and it commonly presents in children undergoing chemotherapy for leukemia. We report the case of a child with PLG caused by a mutation in the macrophage-expressed gene 1 (MPEG1), suggesting possible PLG occurrence in children undergoing treatment for pulmonary nodular lesions. CAS…

The chest X-ray features of chronic respiratory disease in HIV-infected children--a review.

Several features of human immunodeficiency virus (HIV) infection contribute to the development of chronic respiratory disease in children. These include the frequency and severity of acute chest infections, as well as the increased risk of pulmonary tuberculosis, aspiration, cardiovascular disease, lymphocytic interstitial pneumonitis or pulmonary neoplasia. The chest radiograph (CXR) remains the most accessible investigation for respiratory disease and plays an important role in the baseline assessment and follow-up. This review focuses on the CXR abnormalities of HIV-related chronic respirat

Thoracic Computed Tomographic Characteristics of Canine Blastomycosis.

While thoracic radiographic studies have described the broad thoracic findings of canine blastomycosis, thoracic computed tomographic (CT) findings have been minimally reported. This single-center, retrospective, descriptive case series study aimed to characterize thoracic CT features of canine blastomycosis. CT studies of dogs diagnosed with blastomycosis with confirmed or presumed pulmonary involvement were included and reviewed by board-certified radiologists. A total of 23 dogs met the inclusion criteria. Soft-tissue-attenuating pulmonary parenchymal lesions (nodules, masses, or consolidat

Cigarette smoking: CT and pathologic findings of associated pulmonary diseases.

The health risks associated with cigarette smoking are well known. Cigarette smoking is the most important causative factor in the development of bronchogenic carcinoma. Pulmonary diseases such as chronic bronchitis, centrilobular and panacinar emphysema, respiratory bronchiolitis-associated interstitial lung disease (RB-ILD), and pulmonary Langerhans cell histiocytosis are also related to cigarette smoking. In adenocarcinoma and squamous cell carcinoma, the most common manifestation at computed tomography (CT) is a solitary pulmonary nodule; in small cell carcinoma, hilar and mediastinal aden

Epidermal growth factor receptor mutation-positive advanced lung adenocarcinoma presenting with acute respiratory failure diagnosed by thin bronchoscope through transnasal route under high-concentration oxygen mask.

A 59-year-old woman complained of continuous dyspnea. Computed tomography revealed multiple pulmonary nodules, mildly small enlarged mediastinal lymph nodes and a nodule in the liver segment 8. Her dyspnea worsened with respiratory failure 4 days after presentation. Liver biopsy was not possible as she could not hold her breath; thus, we performed bronchoscopy. For biopsy, the pulmonary nodules with a positive bronchus sign were preferred over the mildly small enlarged mediastinal lymph nodes. Bronchoscopy under non-invasive positive pressure ventilation (NPPV) or high-flow nasal cannula (HFNC…

[Radiographic and pathological findings in 4 patients with pulmonary cryptococcosis].

We reviewed the records of 4 patients with pathologically diagnosed pulmonary cryptococcosis to determine whether there was any relationship between radiographic and pathological findings. The underlying disease were diabetes mellitus (patient 1), rheumatoid arthritis treated with glucocorticoids (patient 2), and adultonset T cell leukemia (patients 3 and 4). All radiographs showed multiple nodules, and patchy and localized infiltrates, which progressed to diffuse interstitial infiltration. According to Mark's classification, the pathological findings in patients 1 and 2 showed granulomatous p

Epithelioid haemangioendothelioma of the lung presenting with pulmonary nocardiosis.

Epithelioid haemangioendothelioma of the lung is a rare tumour, originally described as intravascular bronchioalveolar tumour. The typical clinical findings are those of bilateral multiple pulmonary nodules in young or middle aged Caucasian women. Pulmonary nocardiosis is an unusual disease affecting patients with immunodeficiency or chronic obstructive lung disease. The first case of pulmonary epithelioid haemangioendothelioma (PEH) associated with pulmonary nocardiosis, in a 36-year-old woman presenting with progressive dyspnoea and fever, is described. The neoplasm was diagnosed by thoracos…
