Management of crush syndrome.

Our experience in treating seven patients with severe crush injury of the lower limbs is described. They were brought to hospital 12 h after rescue and had no treatment until then. All seven developed acute renal failure due to myoglobinuria and dehydration. Five were anuric and three non-oliguric. All developed severe sepsis and two had also acute respiratory failure. No bleeding tendency was observed. They were treated along the following lines: early extensive fasciotomy and removal of dead tissues; early fluid challenge; early peritoneal dialysis and/or hemodialysis; high caloric, high pro…

Hemodialysis treatment of methanol intoxication.

Hemodialysis was undertaken to treat methanol intoxication in a patient with intravascular hemolysis, myoglobinuria, systemic acidosis, renal insufficiency, retinal abnormalities, and a blood methanol level of 400 mg/dl, 29 h after methanol ingestion. Sixteen hours of treatment were required to achieve safe blood levels. Calculated methanol removal was 218 g, or 73% of the ingested dose, and extraordinarily large volumes of ethanol were required to maintain blood levels greater than 100 mg/dl. All the manifestations of methanol poisoning were reversed and there were no permanent sequelae. It i…

Isolation of influenza virus from muscle in myoglobinuric polymyositis.

We report the first isolation of influenza virus from muscle in a man with myoglobinuria and acute polymyositis. Influenza virus was isolated from cultures of Madin Darby bovine kidney and primary rhesus monkey kidney cells inoculated with muscle homogenates in the presence of trypsin; the virus was identified by neutralization and hemagglutination inhibition studies using influenza B/Lee antiserum. Viral plaque assay was performed with Madin Darby canine cells. Viral antigen was also detected by specific immunofluorescence in muscle, and myxovirus-like particles were seen in subsarcolemmal va…

Acute renal failure in Rocky Mountain spotted fever.

Review of clinical and pathologic data from ten fatal cases of Rocky Mountain spotted fever (RMSF) revealed the importance of acute renal failure in the clinical course and of multifocal perivascular interstitial nephritis as the principal pathologic lesion. In nine cases, Rickettsia rickettsii were demonstrated by immunofluorescence in the areas of vasculitis. Evidence was lacking for the role of disseminated intravascular coagulation, glomerulonephritis, or myoglobinuria in the pathogenesis of acute renal failure in these cases. Rickettsia-induced vascular injury led to acute renal failure b…

Influenza A viral infection associated with acute renal failure.

The clinical and laboratory findings in four cases of acute renal failure following the onset of influenza A viral infection (Port Chalmers/1/73) are presented. Although the pathophysiologic mechanisms affecting the kidney in these cases varied, the ensuing renal failure in each patient was severe. Findings suggestive of acute myoglobinuria developed in one patient, and disseminated intravascular coagulation (DIC) occurred in another. The role of viruses in the pathogenesis of renal disease is reviewed. Despite inconclusive evidence that the influenza virus can cause human renal disease, the s…

The detection of myoglobin in urine and its distinction from normal and variant haemoglobins.

Victims of severe injuries frequently pass haemoglobin, myoglobin, or both proteins in their urine. If the kidneys of such persons are to be considered as donor material for transplantation, a pathology department may be requested to identify which of these pigments is present. If freshly passed urine is available, haemoglobinuria in the absence of myoglobinuria may be rapidly identified by spectroscopy. However, the rapid degradation of myoglobin to the met-myoglobin form will make spectroscopic recognition of this pigment in the urine unreliable. In the absence of variant haemoglobins, myogl…

Colchicine induced rhabdomyolysis.

A 59-year-old man with chronic obstructive pulmonary disease (COPD), atrial fibrillation, and gout developed acute dyspnea, cough, and diffuse muscle aches and pains. He had commenced colchicine (0.6 mg b.i.d. p.o.), for the first time, one month earlier for recurrent gout attacks. Clinical examination revealed atrial fibrillation, an exacerbation of his pulmonary disease, tender muscles, especially calves, and diffuse muscle weakness. Laboratory results included creatinine phosphokinase 6961 U/l (1% MB), microscopic hematuria, myoglobinuria, elevated creatinine 1.6 mg/dl, and blood urea nitro…

Amoxapine overdose in a young man: a transient mitochondrial abnormality?

Amoxapine is a second-generation tricyclic antidepressant structurally related to the neuroleptic loxapine. It was previously marketed as an alternative to traditional tricyclic antidepressants because of alleged shorter onset of action and fewer cardiotoxic effects. However, various adverse reactions, including cardiac dysrhythmias, renal failure, coma, seizures, and neuroleptic malignant syndrome, were reported during therapy or after acute overdose. A 14-year-old boy ingested 1900 mg of amoxapine and developed seizures, hypertension, hyperpyrexia, altered mental status, myoglobinuria, renal…

[Exertion-related abnormalities in the urine].

Strenuous exercise can lead to the occurrence of various abnormalities in the urine. Hematuria and proteinuria are those most frequently described. Other less common possibilities are pigmenturia like hemoglobinuria and myoglobinuria. These urine abnormalities are found not only in sportsmen/women or soldiers, but also in percussionists. Other possible causes are hereditary metabolic myopathy and the use of drugs or medication. Exertion-related urine abnormalities occur frequently and are usually benign. Any abnormalities should disappear after a period of 24-72 h of rest. Exertional rhabdomyo…

Early recognition of unsuspected malignant hyperthermia and successful management of serve myoglobinuric renal failure in subsequent rhabdomyolysis: a case report.

Malignant hyperthermia (MH) is a rare inherited disorder of skeletal muscle, its inheritance being autosomal dominant and its mutant gene located on chromosome 19. MH is occasionally observed during general anesthesia when using some special triggering agents. In the susceptible patient, it presents with a fulminant skeletal muscle hypermetabolic crisis and proceeds to serve rhabdomyolysis. Once rhabdomyolysis is established, acute renal failure is not an inevitable consequence, yet it is the fatal complication if the condition is not appropriately managed. We describe a case of acute renal fa

Severe rhabdomyolysis in well conditioned athletes.

Two well conditioned high school athletes developed rhabdomyolysis after rather modest exercise in activities for which they had not specifically trained-weight lifting and playing bongo drums. Neither patient had a history of exercise intolerance or rhabdomyolysis before. The extent of the rhabdomyolysis in the first patient was extreme (CPK 181,690), and he did not develop renal impairment. The second patient's course was complicated by the use of a contrast study, and he developed marked renal impairment.

Myoglobinuric renal failure after generalised tonic-clonic seizures. A case report.

A 47-year-old man developed progressive renal impairment after a series of seven generalised tonic-clonic seizures. The patient did not become oliguric and because recovery of renal function was rapid, dialysis was not required. The diagnosis of myoglobin-induced renal failure was made on the basis of markedly elevated muscle enzyme values, and myoglobin in the urine.

An unusual complication of rubber band ligation of hemorrhoids.

Rubber band ligation of hemorrhoids has had a low incidence of complications until recently, when five deaths resulted from bacterial septicemia or toxemia. The case presented describes a severe soft-tissue infection following banding successfully treated with antibiotics, surgical debridement, and hyperbaric oxygen.
