Controversy still surrounds the management of necrotic and septic complications of acute pancreatitis. A review of the literature of the past decade dealing with the surgical treatment of pancreatic necrosis, pancreatic abscess and infected pancreatic necrosis has been undertaken. We report a case of adult polymyositis with peculiar muscular pathology of innumerable muscle fiber necrosis and regeneration accentuated in the perifascicular area. A 51-year-old woman developed generalized weakness of the extremities, trunk and bulbar muscles subacutely for two months. The development of an osteonecrotic (avascular necrosis) hip joint in a patient with symptomatic CDC Stage 1V C2 disease is reported. The diagnosis was aided by radiological assessment and confirmed by radioisotope bone scan. This case serves as yet another musculoskeletal manifestation in association with HIV infection.

A case of heparin-induced cutaneous necrosis resembling necrotizing fasciitis occurred in a post-cesarean section patient. She presented with massive necrosis of the skin and abdominal pannus on the sixth postoperative day after a cesarean section for severe preeclampsia and failed induction. A biopsy showed findings characteristic of heparin-induced cutaneous necrosis, permitting conservative therapy.

Twenty-eight consecutive patients with infected pancreatic necrosis were managed by extensive unroofing of the superior retroperitoneum, blunt pancreatic sequestrectomy, laparotomy pad packing of the lesser sac over a layer of Adaptic gauze, and scheduled re-explorations at intervals of 2-3 days (open drainage). An 80-year-old male patient with histologically confirmed temporal arteritis presented with two very rare complications: ocular hypotony and bilateral scalp necrosis. The intraocular pressure gradually normalized on high-dose cortisone therapy. Scalp necrosis was surgically removed and covered with a split-skin graft with good incorporation of the transplant. Scalp necrosis as well as ocular hypotony are serious complications of temporal arteritis, indicating a poor prognosis. To date three similar cases have been reported in the literature.

A cytologically distinctive type of acute hyperoxemic injury of retinal neurons occurs in premature neonates. Ganglion cells in the central well-vascularized neonate retina are susceptible to excessive oxygen and this is expressed morphologically by karyorrhexis of their nuclei.
