Nonspecific (Heterologous) Protection of Neonatal BCG Vaccination Against Hospitalization Due to Respiratory Infection and Sepsis.

BACKGROUND: Bacille Calmette-Guerin (BCG) vaccination has been suggested to have nonspecific beneficial effects in children from developing countries, reducing morbidity and mortality caused by unrelated pathogens. OBJECTIVE: We aimed to assess the heterologous protective effects of BCG vaccination against respiratory infection (RI) and sepsis not attributable to tuberculosis in children born in Spain. METHODS: We conducted a retrospective epidemiological study using data from the Official Spanish Registry of Hospitalizations (CMBD-HA) to identify differences in hospitalization rates (HR) in

TLR2-induced IL-10 production impairs neutrophil recruitment to infected tissues during neonatal bacterial sepsis.

Sepsis is the third most common cause of neonatal death, with Group B Streptococcus (GBS) being the leading bacterial agent. The pathogenesis of neonatal septicemia is still unsolved. We described previously that host susceptibility to GBS infection is due to early IL-10 production. In this study, we investigated whether triggering TLR2 to produce IL-10 is a risk factor for neonatal bacterial sepsis. We observed that, in contrast to wild-type (WT) pups, neonatal TLR2-deficient mice were resistant to GBS-induced sepsis. Moreover, if IL-10 signaling were blocked in WT mice, they also were resist

Pattern and etiology of culture-proven early-onset neonatal sepsis: a five-year prospective study.

OBJECTIVES: To investigate the incidence of early-onset neonatal sepsis and identify the main pathogens over a 5-year period in Kuwait. METHODS: Blood samples were collected from all infants with any clinical or laboratory feature suggestive of sepsis, at the main maternity hospital in Kuwait. Cases of early-onset neonatal infection were defined as culture of a single potentially pathogenic organism from blood or cerebrospinal fluid from infants younger than 7 days of age, in association with clinical or laboratory findings consistent with infection. RESULTS: The overall incidence of early-o…

Early onset neonatal sepsis: the burden of group B Streptococcal and E. coli disease continues.

BACKGROUND: Guidelines for prevention of group B streptococcal (GBS) infection have successfully reduced early onset (EO) GBS disease. Study results suggest that Escherichia coli is an important EO pathogen. OBJECTIVE: To determine EO infection rates, pathogens, morbidity, and mortality in a national network of neonatal centers. METHODS: Infants with EO infection were identified by prospective surveillance at Eunice Kennedy Shriver National Institute of Child Health and Human Development Neonatal Network centers. Infection was defined by positive culture results for blood and cerebrospinal f

Efficacy of tumor necrosis factor alpha and eicosanoid inhibitors in experimental models of neonatal sepsis.

The potential role of tumor necrosis factor alpha (TNF alpha) and eicosanoids in the pathogenesis of experimental neonatal sepsis models was investigated. Lethality was induced in neonatal rats by administration of heat killed group B streptococci (GBS, 7 mg kg-1 intracardially) or Salmonella enteritidis endotoxin (0.35 mg kg-1 intracardially). The relative efficacy of six compounds with putative TNF alpha and eicosanoid inhibitory actions were tested. These were: ibuprofen (3 and 20 mg kg-1), a cyclo-oxygenase inhibitor; CGS85515 (30 mg kg-1), a lipoxygenase inhibitor; LY203647 (30 mg kg-1), …

[Early onset of neonatal sepsis with non-encapsulated Haemophilus influenzae--a new problem in Denmark].

During the past decade an increasing incidence of neonatal septicaemia caused by non-encapsulated Haemophilus influenzae has been reported in both the USA and Europe. From the Danish county of Northern Jutland four cases are reported in premature infants. Respiratory distress and septicaemia were apparent within the first 12 hours after birth, and clinical findings were similar to early onset neonatal septicaemia caused by group B haemolytic streptococci or pneumococci. One neonate born at the 28th week of gestation died, and three other neonates of gestational age 30 to 34 weeks recovered. Th

A randomised prospective comparison of cefotaxime versus netilmicin/penicillin for treatment of suspected neonatal sepsis.

In an open prospective study performed in 2 neonatal units, infants with suspected neonatal sepsis (SNS) of unknown microbial cause were randomly allocated to receive treatment with either cefotaxime (CTX) or netilmicin plus penicillin (N + P). 236 patients were entered into the trial, of whom 222 were evaluable. The number of 'definitely' and 'probably' infected babies was similar in both groups. There was no difference in clinical outcome between patients in the 2 treatment groups and no side effects were recorded for either of the antibiotic regimens. Antibiotic sensitivity testing of bacte…

Renal resistive index and neonatal sequential organ failure assessment score as acute kidney injury risk predictors in preterm neonates with late-onset neonatal sepsis.

AIM: Neonatal acute kidney injury (AKI) is an independent predictor of mortality with serum creatinine (sCr) remaining the gold standard. However, by the time sCr increases, the kidney may already have lost part of its function. Late-onset neonatal sepsis (LONS) is a common trigger of AKI through inflammation and renal hypoperfusion. The renal resistive index (RRI) is a Doppler-derived marker of renal perfusion, and the neonatal sequential organ failure assessment (nSOFA) score reflects systemic organ dysfunction that may accompany sepsis-associated AKI. This study aimed to compare the diagnos…

Neonatal Dengue Infection Mimicking Late-Onset Neonatal Sepsis in Endemic Regions: A Case Report Series and a Proposed Diagnostic Framework for Antimicrobial Stewardship.

Neonatal dengue infection may mimic late-onset neonatal sepsis (LOS) in endemic regions, leading to misdiagnosis and unnecessary antibiotic exposure. We describe a case series of neonates admitted to a tertiary referral hospital in a dengue-endemic area with suspected LOS who underwent standard sepsis evaluation, including blood cultures, inflammatory markers, and clinical assessment, and were subsequently tested for dengue infection by anti-dengue virus IgM using ELFA methodology on the VIDAS® platform. All patients presented with fever and hematologic abnormalities, particularly thrombocytop…

Determinants of neonatal mortality among hospitalised neonates with sepsis at Queen Elizabeth Central Hospital, Blantyre, Malawi: A mixed-methods study.

Neonatal sepsis-related deaths remain a significant health problem contributing to higher morbidity and mortality rates, particularly in low resource settings, such as Malawi. However, there is limited information to associate risk factors and health system factors with mortality. This study investigated the risk factors associated with mortality and explored health system factors contributing to deaths among neonates with sepsis at Queen Elizabeth Central Hospital (QECH), Blantyre, Malawi. This mixed-method study utilised a convergent parallel approach to describe the determinants of neonatal…
