T.O.HO.-PCNL Score for Predicting Success of Percutaneous Nephrolithotripsy: A New Score Development Study.

Background/Objectives: The aim of this study was to develop a modified version of the T.O.HO. score for predicting percutaneous nephrolithotomy (PCNL) success. Materials and Methods: Patient demographics, stone-related parameters, perioperative findings, and postoperative outcomes were recorded. We reviewed the data of 155 patients who underwent percutaneous nephrolithotomy (PCNL) between October 2020 and December 2024. Patients were divided into two groups: success and failure. While preserving the validated components of the existing T.O.HO. score, the stone location parameter was restructur

Less Is More: Going Green in Urological Procedures.

INTRODUCTION: Operating rooms produce significant waste that is disproportionate compared with other departments within health care systems. In this pilot study, we aim to quantify the recyclable and nonrecyclable waste generated by urologic procedures at our institution as well as survey other urologists' recycling practices. METHODS: Data were collected from January 2022 to October 2024 for 5 urologic procedures: ureteral stent exchange, ureteroscopy and laser lithotripsy for stone removal, percutaneous nephrolithotomy, robotic-assisted radical prostatectomy, and radical cystectomy. Weight

Predicting postoperative fever in culture-negative patients undergoing mini-PCNL using MAP score-augmented machine learning: a retrospective cohort study.

PURPOSE: Postoperative fever is a common complication following percutaneous nephrolithotomy (PCNL) that occurs even in patients with sterile urine cultures. Traditional risk-assessment tools are insufficient in this subset of patients. This study aims to develop a risk prediction model for detecting postoperative fever in patients with sterile preoperative urine cultures by integrating the Mayo Adhesive Probability (MAP) score with machine learning (ML) techniques. METHODS: This retrospective cohort study included 730 patients with sterile urine cultures who underwent mini-percutaneous nephr

Surgeon, and Institution Characteristics Associated Surgical Preferences in the Pediatric KIDney Stone Care Improvement Network.

OBJECTIVE: To reveal barriers and opportunities to implement evidence for the management of pediatric kidney stone disease, we determined surgeon and institutional factors associated with preferences for the type of surgical intervention for kidney and ureteral stones. METHODS: We conducted a cross-sectional study of urologists participating in the Pediatric KIDney Stone Care Improvement Network (PKIDS) trial. Questionnaires ascertained strengths of urologists' preferences for types of surgery as well as characteristics of participating urologists and institutions. The outcome was the strengt

Retroperitoneal laparoscopic pyelolithotomy as salvage therapy for refractory infectious renal calculi: A case report.

Refractory infectious renal calculi with severe pyonephrosis pose a clinical dilemma due to ineffective drainage and contraindicated percutaneous nephrolithotomy (PCNL). We report a 56-year-old male with diabetes and paraplegia who developed left renal pelvic calculi (2.5 cm) with refractory infection after failed stenting/nephrostomy. Retroperitoneal laparoscopic pyelolithotomy was performed, achieving complete stone removal and pus drainage. The patient recovered uneventfully with no infection recurrence at 3-month follow-up. This procedure is a safe, effective minimally invasive salvage the

Squamous papilloma of the renal pelvis mimicking transitional cell carcinoma.

Squamous papilloma of the renal pelvis is an extremely rare entity. To the best of our knowledge, no case has been reported till date. A 45-year-old female presented to us with flank pain for two months. She had undergone left-sided percutaneous nephrolithotomy three years ago. Preoperative evaluation suggested the presence of a transitional cell tumor arising from the left renal pelvis. Diethylene-triamine-pentaacetic acid renogram showed a non-functional left kidney. She underwent left nephroureterectomy. Histopathology showed squamous papilloma of the renal pelvis. Subsequent follow-ups hav

[Left renal pelvic tumor 32 years after calyco-ileo-vesiconeostomy].

This is a case report of left renal pelvic tumor found 32 years after left calico-ileo-vesiconeostomy. The patient has undergone right nephrectomy for absence of renal function at 26 years-old and left urinary reconstruction was performed using the intestine for pyelo-ureteral junction obstruction at 28 years old because of bilateral congenital hydronephrosis. Later he was treated for recurrent left renal stone with percutaneous nephrolithotripsy, nephrolithotomy and extracorporeal shock wave lithotripsy. When he was 56 years old, macrohematuria appeared. He received left nephrectomy under the

[Traumatic rupture of giant hydronephrosis due to ureteral stone: a case report].

A case of rupture of giant hydronephrosis due to blunt trauma is reported. A 49-year-old male was admitted to our hospital complaining of right flank pain and gross hematuria after falling from a roof. Radiological evaluation lead to the diagnosis of marked right hydronephrosis with severe parenchymal thinning secondary to ureteral stone, left staghorn calculus and the rupture of right kidney. Right nephrectomy was performed and the postoperative course was uneventful. Three weeks later, percutaneous nephrolithotomy (PNL) was performed for left staghorn calculus. Nephrolithiasis in a 17-Year-Old Male With Seckel Syndrome and Horseshoe Kidneys: Case Report and Review of the Literature.

We report the case of a 17-year-old male with Seckel syndrome and horseshoe kidneys which had a 7 mm kidney stone in the lower pole calyx of the right moiety. The patient had a history of rotoscoliosis with 60° dextroconvex curvature and hepatic steatosis. Attempted ureteroscopy was unsuccessful due to stone location and anatomy. Percutaneous nephrolithotomy was ultimately required. This case highlights the difficulties of endoscopic treatments of renal calculi in patients with abnormal renal anatomy and dysmorphia. To our knowledge, this is the first reported case of nephrolithiasis in a pati

A Rare Case of Vertebral Osteomyelitis Caused by Mycobacterium farcinogenes in an Immunocompetent Host.

We report a rare case of vertebral osteomyelitis due to Mycobacterium farcinogenes in a patient who had undergone left-sided percutaneous nephrolithotomy for renal stones. Following the procedure, he developed a wound site infection that progressed to vertebral osteomyelitis and a paravertebral abscess. Initial treatment with empirical anti-tubercular drugs was ineffective. A computed tomography-guided aspiration and biopsy identified acid-fast bacilli as M. farcinogenes using Matrix-assisted laser desorption ionization-time-of-flight. Treatment was then tailored based on the susceptibility re

Staghorn Calculus with Adenomatoid Tumor: A Case Report.

Testicular adenomatoid tumor is very rare. More understanding of the tumor and treatment is required for better outcomes. A 63-year-old man visited a urologist for staghorn calculus. During follow-up, he had recurrent left flank pain and intermittent hematuria. Computed tomography demonstrated left staghorn calculus, and then further assessment revealed left testicular swelling. Ultrasound showed epididymal mass. Percutaneous nephrolithotomy was deferred and we performed left radical orchiectomy. A pathologic examination revealed testicular adenomatoid tumor. This case highlights the importanc

Microperc for the management of renal calculi in pelvic ectopic kidneys.

Management of stone disease in an ectopic kidney is challenging. Laparoscopy or ultrasound guided percutaneous nephrolithotomy and retrograde intra-renal surgery are the preferred techniques for these stones. We performed ultrasound guided microperc using a 16 G needle for the management of renal calculi in pelvic ectopic kidneys in two patients. There was no intraoperative or post-operative complication. Both patients had complete stone clearance and were discharged on the first post-operative day. Ultrasound guided microperc is a safe and effective option for the management of small renal ca

Pelvi-ureteric junction obstruction in autosomal-dominant polycystic kidney disease: an association yet to be reported.

Autosomal-dominant polycystic kidney disease (ADPKD) is the most common inherited renal cystic disease. It is characterised by the development of renal parenchymal cysts and a variety of other extrarenal manifestations. Pelvi-ureteric junction (PUJ) obstruction has not been described in association with ADPKD in the literature. We present a case of a 23-year-old man presenting with bilateral flank pain. On evaluation he was diagnosed to have ADPKD with bilateral renal calculi and left-sided PUJ obstruction. He underwent successful right percutaneous nephrolithotomy and left laparoscopic dismem

Retroperitoneoscopic management of caliceal diverticular calculi.

INTRODUCTION: The management of caliceal diverticular calculi has changed from an open surgical approach to endoscopic management. TECHNICAL CONSIDERATIONS: Different minimally invasive treatment modalities, such as extracorporeal shock wave lithotripsy, ureteroscopy, percutaneous nephrolithotomy, laparoscopy, and retroperitoneoscopy, can be offered to the patient. We report on a retroperitoneoscopic operative technique using endosonography for location and performing nephrotomy with complete excision of the caliceal diverticulum and ligation of the diverticular neck with an Endo-loop. Ureteromat. Method to facilitate ureterorenoscopy and avoid dilatation.

The history of rigid ureterorenoscopy and the development of the Ureteromat are briefly described. The technical principles of the Ureteromat, the technique for its use, and the basic principles for performing ureterorenoscopy aided by this irrigation system are described. In our experience, the average operating time diminished from 66 to 22 minutes with the Ureteromat, severe complications dropped from 4 to 0.9 per cent, and the success rate for calculi in the ureter increased from 72 to 98 per cent. Furthermore, the Ureteromat facilitates other procedures (flexible ureteroscopy and flexible

The destruction of renal calculi by external shock-waves: practical operation and initial results with the Dornier lithotripter.

ESWL is now the treatment of choice for most renal calculi. Large stones (20 mm) and staghorn stones may require preliminary debulking before lithotripsy and stones in the lower part of the ureter require pushing back into the renal pelvis before treatment. Compared with open surgery and percutaneous nephrolithotomy the procedure is less traumatic for the patient and may result in a shorter hospital stay and a faster return to normal life for the patient. It has been estimated that in 10 to 20 years' time around 70% of patients with urinary stone disease will be treated by ESWL.

Energy sources in percutaneous surgery. Are lasers ready to become the gold standard? A narrative review.

INTRODUCTION: Nephrolithiasis is a prevalent and recurrent condition affecting approximately 10-11% of the population. Percutaneous nephrolithotomy (PCNL) and mini-PCNL are established treatments for large renal stones. Outcomes are influenced by the choice of lithotripsy energy source. Available technologies include pneumatic, ultrasonic, Holmium:YAG (Ho:YAG), and thulium fiber laser (TFL), each with specific advantages and limitations. This review summarizes current evidence on these energy sources in PCNL and mini-PCNL. MATERIALS AND METHODS: A narrative review was performed using PubMed a
