Pineal Parenchymal Tumor of Intermediate Differentiation (PPTID) and Papillary Tumor of Pineal Region (PTPR): A Review.

BACKGROUND: Pineal parenchymal tumors account for less than 0.3% of all CNS tumors and "Pineal parenchymal tumor of intermediate differentiation" (PPTID; World Health Organization (WHO) grades II and III) exhibit intermediary differentiation and prognosis. However "Papillary tumor of the pineal region" (PTPR; WHO grades II and III) is a distinct entity. OBJECTIVES: This combination of rarity and apparent similarity often leads to perplexity regarding the treatment and prognosis among neurosurgeons. In this review, we have tried to elucidate the differences in clinical as well as treatment mod…

Outcomes of tethered cord repair with a layered soft tissue closure.

BACKGROUND: Previous studies have demonstrated the benefit of releasing symptomatic tethered cords; however, complications such as seroma, cerebrospinal fluid leak, and infection continue to plague these patients. We propose that composite tissue closure of tethered cord repairs yields superior outcomes and that a collaborative effort between neurosurgery and plastic surgery may result in enhanced structural and functional results. METHODS: This is a retrospective study comprised of consecutive patients with tethered cord syndrome by 2 neurosurgeons and 2 plastic surgeons between 1994 and 200…

Macrocephaly in the Primary Care Provider's Office.

Macrocephaly is commonly encountered in the primary care provider's office. It is defined as an occipitofrontal circumference that is greater than 2 standard deviations above the mean for the child's given age. Macrocephaly is a nonspecific clinical finding that may be benign or require further evaluation. An algorithmic approach is useful for aiding in the clinical decision-making process to determine if further evaluation with neuroimaging is warranted. Abnormal findings may signify a harmful underlying cause, requiring referral to a genetic specialist or neurosurgeon.

[Surgical management in complex trauma of the lower limb with soft tissue defects].

This work is studying the treatment for the complex trauma of the lower limb, followed by soft tissue defects, with bones exposure, that necessitate immediate coverage with adequate tissues. The reconstructive method should be chosen according to the location and soft tissue requirement of the defect (local/regional flap, or free tissue transfer). Aggressive management of acute trauma of the lower limb provides good results. First condition for the intervention success is working together: plastic surgeon, orthopaedic surgeon, neurosurgeon, physiotherapist.

Management of the neurosurgical emergencies in pregnancy.

This study presents the files of nine pregnant women with severe neurosurgical diseases (traumatic, vascular, and tumoral) who were treated in the Neurosurgical Clinic, from 1982 to 1988. Conclusions were drawn after a survey according to the optimal therapy applied, to the mother or fetus priority, to the age of patients and to the severity of the neurological impairment. The early intervention of a team including the neurosurgeon, obstetrician and the specialist in the intensive care unit saves one life or both. A delay condemns both.

[Spontaneous rhinorrhea of a rare etiology (apropos of empty sella turcica)].

The authors report a case of non traumatic rhinorrhea due to an empty sella turcica. Such an intra sellar arachnoidocele compress the pituitary gland against the posterior wall, erode the floor and lead to CSF rhinorrhea through the sphenoidal sinus. Diagnosis and treatment are best accomplished by a team approach involving otolaryngologist, neurosurgeon and neuroradiologist. CT Scan and IRM explain the diagnosis. Treatment can be accomplished by a transeptal transphenoidal approach with localization and repair of the leak in the absence of increased intracranial pressure.

Craniovertebral Junction Instability in the Setting of Chiari Malformation.

Described in the seminal paper by Hans Chiari in 1891, the Chiari I malformation (CMI) is a radiographic diagnosis commonly encountered by neurosurgeons and is often treated surgically with generally positive clinic outcomes. Studies have documented that 1% to 4% of patients undergoing MRI of the brain or cervical spine will be diagnosed with CMI, characterized by greater than 5 mm tonsillar herniation below the foramen magnum. More recently CMI has been described as a spectrum of disease, which includes Chiari 0, Chiari 1.5, and the complex Chiari. Primarily through multicenter clinical outco…

Fibrinolytic (Thrombolytic) Therapy for Post Intraventricular Hemorrhagic Hydrocephalus in Preterm Infants.

While the survival rate of preterm infants has increased dramatically over the last few decades, intraventricular hemorrhage and subsequent hydrocephalus remain major unsolved problems in neonatal intensive care. Once intraventricular hemorrhage occurs, severe neurological sequelae are inevitable. Treatment of this complicated pathology and achievement of favorable neurofunctional outcomes in fragile infants are crucial challenges for pediatric neurosurgeons. Fibrinolytic therapy, which chemically dissolves hematoma, is a promising and useful treatment method. In this paper, the historical bac…

'Having regard to the findings made by the referrer': medicolegal implications of case SC (2020) for hospital clinicians.

A case concerning the management of a child with meningitis in a hospital paediatrics department provided a judgement that has significant potential implications for medical practice. The case establishes that the examination findings of a previous clinician must be taken into account when investigating and treating patients. This case is of medicolegal relevance to clinicians practising in tertiary centres and who receive patients from other hospitals. This article highlights the medicolegal implications of this case, using an example of cauda equina syndrome for neurosurgeons, a condition th…

How I do it-Helsinki style mini-pterional craniotomy for clipping of middle cerebral artery bifurcation aneurysms.

BACKGROUND: Different versions of the mini-pterional (MPT) approach have been described often with the idea the smaller the better. Attempts to reduce incision and craniotomy size for better cosmetic results should not be performed at the expense of safety. METHOD: We present our take on the MPT as a balance between size and safety which can be adopted by vascular neurosurgeons in training. The craniotomy stays within the confines of the superior temporal line and is completely covered by temporal muscle after closure. CONCLUSION: This approach is cosmetically superior while still offering a…

Preoperative Workup for Patients with Pituitary Lesions.

The pituitary gland plays a vital role in hormonal regulation. Pituitary lesions include tumors, cysts, and inflammatory processes that require multidisciplinary care from endocrinologists, neuro-ophthalmologists, neurosurgeons, and otolaryngologists. Treatment is typically aimed at controlling hormonal hypersecretion, decompressing the optic apparatus, and reducing tumor volume, and surgery is a common first-line approach. In this article, we provide a background on the function of the pituitary gland, common pituitary lesions, and their clinical presentations, as well as a summary of the his…

Retroperitoneal dermoid cyst presenting with radiculopathy symptoms: a case report.

Dermoid cysts rarely present in the retroperitoneal space or during adulthood. In this case report, we describe the clinical presentation, operative and post-operative course of a 31-year old with a retroperitoneal dermoid cyst. The patient presented with buttock and leg pain/paresthesia found to have a retroperitoneal mass between the psoas muscle and L5/S1 disk space. We describe the operative approach, including intra-operative images, of the resection by a team of urologists and neurosurgeons. The histology is also presented. Finally, we discuss the benefits of use of intra-operative ultra…

Surgical anatomy of the lateral femoral cutaneous nerve for meralgia paraesthetica: A simple technical guide for surgeons and trainees alike.

Meralgia paraesthetica is an entrapment neuropathy of the lateral femoral cutaneous nerve, usually due to compression at the inguinal ligament as the nerve passes from the pelvis into the thigh. Surgical decompression of the lateral femoral cutaneous nerve is a simple and effective treatment option, but the surgical anatomy of the area is not always familiar to neurosurgeons and neurosurgical trainees alike. This paper is a simple review of the relevant surgical anatomy and the surgical steps of lateral femoral cutaneous nerve decompression, with the aim of providing the busy surgeon and train…

Pituitary Tumors Centers of Excellence.

Pituitary tumors are common and require complex and sophisticated procedures for both diagnosis and therapy. To maintain the highest standards of quality, it is proposed to manage patients in pituitary tumors centers of excellence (PTCOEs) with patient-centric organizations, with expert clinical endocrinologists and neurosurgeons forming the core. That core needs to be supported by experts from different disciplines such as neuroradiology, neuropathology, radiation oncology, neuro-ophthalmology, otorhinolaryngology, and trained nursing. To provide high-level medical care to patients with pitui…
