What if it is the eyes? The effect of eye movement disorders on cognitive testing in MS.

BACKGROUND: Visual cognitive tests that rely on saccades may be affected by internuclear ophthalmoplegia (INO), irrespective of cognitive ability. INO and cognitive decline are common in people with multiple sclerosis (PwMS). We assessed the impact of INO on visual versus non-visual cognitive tests in PwMS. METHODS: PwMS (n=197) completed auditory and visual neuropsychological tests, and demographically corrected Z-scores were obtained. INO was quantified using the DEMoNS infrared oculography protocol, applying the versional dysconjugacy index (VDI) of area under the saccadic trajectory (AUC)

Acute Lacunar Infarct in the Right Dorsal Pons Presenting as Internuclear Ophthalmoplegia Following Percutaneous Cardiac Intervention.

A 58-year-old male with a history of type 1 diabetes, hypertension, smoking, and obesity, was seen by the inpatient neurology team for symptoms of dizziness and diplopia after cardiac catheterization. His symptoms caused him to feel nauseated and vomit and were improved with closing his right eye. It was noted that he was unable to adduct his right eye and had nystagmus of his left eye on exam. He had no ptosis, and convergence was preserved. The remaining physical exam was normal. Initial limited magnetic resonance imaging (MRI) was within normal limits; however, repeat limited brain MRI with

Bilateral Congenital Trochlear-Oculomotor Synkinesis in a 5-Month-Old Infant.

Oculomotor synkinesis is an unusual phenomenon that encompasses various presentations with few associations reported in the literature. It can occur with or without a previous setting of oculomotor nerve injury. The authors present the first reported case of bilateral congenital trochlear-oculomotor synkinesis in a 5-month-old infant. [J Pediatr Ophthalmol Strabismus. 2024;61(5):e50-e53.].

Acute bilateral hypotropia and esotropia complex as first manifestation of multiple sclerosis: a case report.

A 21-year-old Japanese woman presented with sudden eye movement disorders. An ophthalmic examination revealed bilateral hypotropia and esotropia complex. Brain magnetic resonance imaging revealed abnormal signals in the posterior and medial part of the lower pontine tegmentum (including periventricular and subcortical white matter) that were suggestive of demyelination. A cerebrospinal fluid test was positive for oligoclonal bands. She was subsequently diagnosed with multiple sclerosis and was administered intravenous methylprednisolone and oral dimethyl fumarate, with complete recovery from h

[A case of cardioembloic stroke with wall-eyed bilateral internuclear ophthalmoplegia (WEBINO) syndrome].

Here, we report a case of an 85-year-old man who presented sudden onset of diplopia, dysarthria, and gait disturbance. On admission, he exhibited bilateral adduction palsy, convergence palsy, and binocular exotropia in the forward gaze showing wall-eyed bilateral internuclear ophthalmoplegia (WEBINO) syndrome. He had a history of chronic nonvalvular atrial fibrillation. DWI-MRI revealed acute ischemic lesions in the paramedian pontine tegmentum, lower midbrain, both cerebellar hemispheres, and left frontal cortex. He was thus diagnosed with an acute phase of cardioembolic stroke. Subsequently,

Acute cerebellar ataxia and myoclonus with or without opsoclonus: a para-infectious syndrome associated with COVID-19.

BACKGROUND AND PURPOSE: Patients with COVID-19 can have central or peripheral neurological manifestations. METHODS: The cases of two patients with acute cerebellar ataxia and myoclonus associated with COVID-19 are reported (with Video S1) and five previously reported patients are discussed. RESULTS: Acute cerebellar ataxia and myoclonus started between 10 days and 6 weeks after the first manifestations of COVID-19. Opsoclonus or ocular flutter was present in four patients. Patients were treated with intravenous immunoglobulins and/or steroids except for one patient, resulting in a striking i

Internuclear Ophthalmoplegia as the First Manifestation of Pediatric-Onset Multiple Sclerosis and Concurrent Lyme Disease.

BACKGROUND Internuclear ophthalmoplegia (INO) presents as a disruption of horizontal conjugate ocular movement and is an uncommon finding in the pediatric population. Its presence warrants a thorough evaluation to search for demyelinating, mass effect, inflammatory, or infectious etiologies. CASE REPORT A 15-year-old African American girl presented to the Emergency Department with acute horizontal binocular diplopia in left gaze. An ophthalmic examination revealed a right INO. She denied any fever, chills, or neck stiffness. Complete blood counts and a metabolic panel were unremarkable. Magnet

Superior Oblique Myokymia: A Case Report of Surgical Treatment, Review of the Literature, and Consideration of Surgical Approach.

BACKGROUND: Superior oblique myokymia (SOM) is a rare disorder characterized by episodic microtremor of the eyeball. in patients with SOM, intermittent contraction of the superior oblique muscle causes irregular and rotatory eye movement, causing oscillopsia and diplopia. Microvascular decompression (MVD) of the trochlear nerve is potentially a definitive treatment method for SOM; however, owing to its rarity, this disorder is not well-known to neurosurgeons, and thus the optimal surgical approach has not yet been determined. CASE DESCRIPTION: A 77-year-old woman with left SOM had experienced

Diagnosing and quantifying a common deficit in multiple sclerosis: Internuclear ophthalmoplegia.

OBJECTIVE: We present an objective and quantitative approach for diagnosing internuclear ophthalmoplegia (INO) in multiple sclerosis (MS). METHODS: A validated standardized infrared oculography protocol (DEMoNS [Demonstrate Eye Movement Networks with Saccades]) was used for quantifying prosaccades in patients with MS and healthy controls (HCs). The versional dysconjugacy index (VDI) was calculated, which describes the ratio between the abducting and adducting eye. The VDI was determined for peak velocity, peak acceleration, peak velocity divided by amplitude, and area under the curve (AUC) of

Bilateral abducens nerve palsy in an infant case of fulminant acute disseminated encephalomyelitis: a case report.

BACKGROUND: Sixth (abducens) nerve palsy (ANP) is far less frequent in children and has not been reported as a sign of acute disseminated encephalomyelitis (ADEM). We present an infant case of ADEM with bilateral abducens nerve palsy (BANP). CASE PRESENTATION: We report one case of BANP in a 15-month-old boy of fulminant ADEM. The patient underwent physical examinations and brain MRI scan three times during about six months follow-up. The patient had BANP and developmental regression when he regained consciousness from a coma, and the signs had persisted for 6-months. CONCLUSIONS: BANP can b

Horizontal Tonic Conjugate Gaze Deviation in a 4-Week-Old Infant: What Can the Eyes (of the Parents) Tell?

A 4-week-old healthy infant failed to make eye contact when approached from the left. When evaluated by us at the age of 11 weeks, left tonic conjugate gaze deviation was noted. Brain MRI showed a left frontotemporal large tension arachnoid cyst causing right falcial herniation. During the insertion of an Ommaya reservoir, hemosiderin was found to be coating the inner wall of the cyst, which may indicate that the asymptomatic cyst evolved to a tension cyst due to spontaneous hemorrhage. Surgical decompression resulted in complete restoration of the horizontal gaze and considerable reduction in

Impairment of saccade adaptation in a patient with a focal thalamic lesion.

The frequent jumps of the eyeballs-called saccades-imply the need for a constant correction of motor errors. If systematic errors are detected in saccade landing, the saccade amplitude adapts to compensate for the error. In the laboratory, saccade adaptation can be studied by displacing the saccade target. Functional selectivity of adaptation for different saccade types suggests that adaptation occurs at multiple sites in the oculomotor system. Saccade motor learning might be the result of a comparison between a prediction of the saccade landing position and its actual postsaccadic location. T
