Metabolic and endocrine disease and arthritis.

Many metabolic and endocrine disorders have effects on the musculoskeletal system, either due to primary changes in bone and collagen or resulting in secondary arthritic and bone changes. Disorders of ossification include heterotopic ossification, diffuse idiopathic skeletal hyperostosis (DISH), and the spondyloarthropathies. The early stages of heterotopic ossification have been analyzed using immunohistochemical techniques to trace the distribution of noncollagenous proteins. Several radiographic features of DISH and ankylosing spondylitis were discussed, including rib hyperostosis and synde

Thoracic Deformity in Fibrodysplasia Ossificans Progressiva.

» Fibrodysplasia ossificans progressiva (FOP), the most severe form of heterotopic ossification (HO) in humans, may lead to severe thoracic deformity (TD) and thoracic insufficiency syndrome (TIS), the most common cause of mortality in individuals with FOP.» In this study, we examine the multiple causes of TD in FOP and propose a dynamic model for the development and evolution of TD that can be used to guide clinical care. This model posits that multiple factors, all originating from the causative gain-of-function mutation in activin receptor A, type 1, lead to TD in FOP.» Factors that lead to

The first approved drug for abnormal bone formation: palovarotene challenges and way forward.

The current communication was planned to assess the validity of a potential cure for two disorders of bone formation that have traditionally been limited to symptomatic treatment in orthopaedic medicine. Heterotopic ossification and fibrodysplasia ossificans progressive, also known as Stoneman Syndrome, are rare conditions that result in abnormal bone formation within soft tissues, causing immense suffering and high mortality rate due to limited treatment options. Several studies have supported the promising outcomes associated with palovarotene, a novel drug, in terms of reducing lesion volum

Clinical staging of Fibrodysplasia Ossificans Progressiva (FOP).

Fibrodyplasia ossificans progressiva (FOP) is an ultra-rare genetic condition of heterotopic ossification (HO) that results in progressive loss of joint function, ultimately rendering movement impossible. Death is most commonly the result of thoracic insufficiency syndrome, or complications related to recurrent respiratory infections. There are no current treatments for FOP, but early and emerging clinical trials offer hope for this devastating disease. With the recent reporting of a comprehensive global natural history study, scales to assess joint dysfunction, and a more accurate prediction

Fibrodysplasia Ossificans Progressiva: A Man Turned to Stone.

Fibrodysplasia ossificans progressiva (FOP) is an exceptionally rare genetic disorder, representing humans' most debilitating form of extraskeletal ossification. It is characterized by progressive postnatal heterotopic ossification of connective tissue and malformations of the big toes. In FOP, ectopic ossification usually begins in the upper paraspinal muscles and then spreads from axial to appendicular regions, cranial to caudal directions, and proximal to distal sites. The mean life expectancy for these patients is typically 40-50 years. Most patients need partial or complete assistance wit

Fibrodysplasia ossificans progressiva with two emergency laparotomies: A case report.

INTRODUCTION: Fibrodysplasia ossificans progressiva (FOP) is a disorder causing progressive heterotopic ossification of muscles, tendons, and ligaments. Invasive procedures such as surgery should be avoided, because physical stimulation causes heterotopic ossification. PRESENTATION OF CASE: A 40-year-old Japanese man with FOP was transported to our hospital with sudden abdominal pain. Emergency surgery was performed because a computed tomography scan showed the presence of intraabdominal free air. We diagnosed peritonitis due to perforation of Meckel's diverticulum and performed a small intes

Ossification of Bilateral Sacrotuberous Ligaments: Two Cases Report and Literature Review.

Ossification of the sacrotuberous ligament is a rare occurrence in soft tissue, with only 15 cases reported in the past few decades. We reported two cases of bilateral ossification in sacrotuberous ligaments and provided a concise review of the literature on this pathology. Clinical data, radiographic outcomes, and diagnostic and treatment details were obtained. This study aimed to summarize this disease's characteristics and investigate its pathogenesis through a review of literature from the last thirty years. This condition is often incidentally confirmed in elderly males via imagiological

Indomethacin: an adjunct to surgical excision of immature heterotopic bone formation in a patient with a severe head injury. A case report.

This case reports heterotopic ossification which produced ankylosis around the hip joint in a young adult treated with steroids for moderate cognitive defects secondary to brain trauma. It is associated with a high recurrence rate, especially if the bone is immature. The patient's heterotopic bone was surgically excised nine months post-injury, augmented with six weeks of indomethacin, and followed for three years.

The application of a hollow trephine in femoral retrograde intramedullary nailing technique.

PURPOSE: The purpose of this study was to describe and evaluate the use of a specially designed hollow trephine to create the entry point through the femoral condyle during retrograde interlocking intramedullary nailing for femoral fractures. METHODS: From June 2019 to December 2021, we treated 11 patients (5 men, 6 women; mean age, 64 years; age range 40-77 years) with mid-distal femoral fractures by retrograde intramedullary femoral nailing using a self-designed hollow trephine for femoral condyle reaming and cancellous bone harvesting. The mode of all the nails is static. Patients were fol

Heterotopic bone formation within a missile track.

A case is presented which is thought to be the first described example of heterotopic ossification occurring within the path of a bullet. Although the information was not available from prior medical records, the bullet presumably passed though bone or periosteum, thereby seeding the permanent cavity and facilitating ossification within the surrounding muscle and soft tissue.

Morpholino-Mediated Exon Skipping Targeting Human ACVR1/ALK2 for Fibrodysplasia Ossificans Progressiva.

Fibrodysplasia ossificans progressiva (FOP) is a rare autosomal-dominant disorder characterized by progressive heterotopic ossification. More than 95% of cases are caused by a recurrent mutation (617G > A; R206H) of ACVR1/ALK2, a bone morphogenetic protein (BMP) type I receptor. Recent studies revealed that ACVR1R206H induces heterotopic ossification by aberrant activation in response to Activin A. Because ACVR1R206H is a hyperactive receptor, a promising therapeutic strategy is to decrease the activity of ACVR1 in patients. Here, we describe a method to reduce ACVR1 expression in FOP patient

Factors affecting outcome of partial radial head fractures: A Retrospective Cohort Study.

BACKGROUND: The purpose of this study is to evaluate which factors will affect range of motion (ROM) and function in partial radial head fractures. The hypothesis is that conservative treatment yields better outcomes. MATERIALS AND METHODS: This retrospective comparative cohort study included 43 adult volunteers with partial radial head fracture, a minimum 1-year follow up, separated into a surgical and non-surgical group. Risk factors were: associated injury, heterotopic ossification, worker's compensation, and proximal radio ulnar joint (PRUJ) implication. Outcomes included radiographic ROM
