STUDY DESIGN: A retrospective radiologic analysis of Paget's disease of the spine.
OBJECTIVES: The prevalence, anatomic distribution, mechanisms of formation of Pagetic vertebral ankylosis (PVA) and the possibility of a relationship to diffuse idiopathic skeletal hyperostosis (DISH) were assessed in a large population of persons with Paget's disease.
SUMMARY OF BACKGROUND DATA: Acquired vertebral ankylosis is not a common feature of the Paget's disease of the spine and its mechanisms of formation remain unknown. In some reports, PVA was associated with radiographic signs of DISH.

Metabolic bone diseases often present in old age and some are more easily treatable than others. Osteoporosis is best managed by prevention, with maximisation of peak bone density and reduction of subsequent bone loss. Although hormone replacement therapy is most useful in prevention, it also has a role in established osteoporosis. Other treating agents include calcium, calcitriol, calcitonin and bisphosphonates. Skeletal metastases from prostate cancer is common and usually do not pose a diagnostic dilemma. This study reviews radiographic appearances of prostatic metastases to the appendicular skeleton in four patients where the appearances simulated osteosarcoma, Paget's disease and Paget's sarcoma. Prostatic metastases to long bones can produce appearances considered characteristic of other lesions and suggest misleading alternative diagnoses.

Some epidemiological aspects of Paget's disease were examined using questionnaires completed by 864 patients with physician-diagnosed Paget's disease and 500 controls of similar ages. Specifically studied were issues of familial aggregation of the disorder, history of exposure to certain infectious diseases, other medical disorders, and calcium intake (as assessed by milk consumption) in childhood. Three cases of subtrochanteric fractures in patients with Paget's disease are presented which were treated by closed intramedullary nailing using the Russel-Taylor reconstruction nail. In one case with severe deformity of the femur, diaphyseal osteotomy was required to enable implant insertion. All fractures (including the osteotomy) united. There were no operative or long-term complications and no implant failures. The reconstruction nail is ideal for such cases and has technical and biomechanical advantages over other implants.

We report the features of a group of 41 patients with Paget's disease (PD) who were identified in a cross sectional study in the "Sierra de la Cabrera" (Madrid). As the screening test, we used serum alkaline phosphatase quantification, and the diagnosis was confirmed by radiological study. This biochemical parameter shows an excellent diagnostic performance, with a sensitivity of 98% and a specificity of 85%. We have compared the value of MRI with conventional radiography and CT in 40 predominantly sclerotic skeletal lesions. We have found that sclerosis and other changes in the bone structure can be demonstrated by MRI in much the same way as with conventional radiography and CT. Thirty-four patients with known Paget's disease were examined by CT in order to characterise the appearances of the bones on transverse sections. The typical findings have been related to their anatomical localisation and frequency. The differential diagnosis from conditions with similar CT appearances, such as osteoplastic metastases, fibrous dysplasia or osteomyelosclerosis, is discussed. The CT appearances of Paget's disease are typical in the same way as conventional radiographs.

This report presents clinical pictures of 11 patients with Paget's disease of bone treated at our clinic since 1977 and discusses the disease progress in seven patients who were given Elcatonin and EHDP and followed-up for a mean period of four years and eight months. All the patients responded to the drugs and the main clinical effect was disappearance of pain. They did not develop side effects which were especially a problem. Serum levels of osteocalcin, the major noncollagenous bone protein, are elevated in patients with certain metabolic bone diseases, but the effects of other illnesses on serum osteocalcin levels are not known. We measured serum osteocalcin concentrations in 250 patients in a rehabilitation hospital who suffered from various illnesses. Five cases are presented in which lesions in the ribs were first detected on bone scanning. In all cases routine radiology was normal and the bone scans were of value in obtaining either the correct diagnosis or defining the extent of disease.

An excavation conducted in 2002 by the Lombardy Archaeological Heritage in the St. Giulio Church (Cassano Magnago-Northern Italy) revealed an adult skeleton with an important alteration of the left femur.
