Diamond-Blackfan anemia: clinical features and treatment results in 4 cases.

Diamond-Blackfan anemia is a rare hematological disease characterized by selective marrow erythroid hypoplasia. We present the clinical and hematological profile and results of treatment in four children. The median age at presentation was 14 months with a median age of onset of pallor at 7.5 months. Mean Hb at presentation was 2.9 g/dl. All children were started on prednisolone with response in one child. One child was switched to dexamethasone and responded transiently relapsing in 4 months but responded to further dexamethasone. Two children required cyclosporin for 3 and 8 weeks for respon…

Hairy cell leukemia-variant.

Hairy cell leukaemia variant is a very rare chronic lymphoproliferative disorder and is closely related to hairy cell leukemia. We hereby describe a case of hairy cell leukaemia variant for the first time in Saudi Arabia. An elderly Saudi man presented with pallor, massive splenomegaly, and moderate hepatomegaly. Hemoglobin was 7.7 g/dl, Platelets were 134 x109/l and white blood count was 140x10 9/l with 97% being abnormal lymphoid cells with cytoplasmic projections. The morphology, cytochemistry, and immunophenotype of the lymphoid cells were classical of hairy cell leukaemia variant. The bon…

Immunoglobulin A and immunoglobulin G biclonal gammopathy in a dog with multiple myeloma.

A nine-year-old golden retriever was referred for evaluation of chronic anorexia, vomiting, and diarrhea. Low body weight, mucous membrane pallor, and palpably enlarged liver and spleen were detected by physical examination. Anemia, hyperglobulinemia, and concurrent trichuriasis and coccidiosis were identified upon initial diagnostic evaluation. Punctate vertebral lysis was apparent radiographically. Atypical plasma cell proliferation was found in the bone marrow, liver, and spleen. An immunoglobulin A and immunoglobulin G biclonal gammopathy was demonstrated by serum protein electrophoresis a…

Surgical treatment of pheochromocytomas.

Pheochromocytomas are tumors that develop from chromaffin tissue of the embryonic sympathoadrenal system. These tumors may occur anywhere chromaffin tissue exists but most often develop in the adrenal medulla. Less than 50% of patients are diagnosed with pheochromocytomas while alive, and most of these tumors are found on autopsy. The classic signs and symptoms of pheochromocytomas are headache, perspiration, palpitations, pallor, and paroxysmal hypertension. Elevated levels of vanillylmandelic acid and metanephrines in patients' 24-hour urine collections are the most reliable diagnostic indic…

[Biofeedback treatment of Raynaud's disease].

Raynaud's disease is characterized by intermittent peripheral vasoconstriction leading to pallor, cyanosis and reactive vasodilation of the arterioles of fingers and toes. These phenomena are accompanied by sensations of cold or warmth, pain and difficulty in manipulating the palms. Ulcerations of the fingertips can occur in severe cases. Since conservative medical treatment, consisting of preventive measures and changing various habits, results in alleviation in only half the patients, sympathectomy is often required. Psychological intervention, including biofeedback, also has a significant r…

[QT prolongation and circulatory arrest after an injection of erythromycin in a newborn infant].

A 8 day-old full-term newborn showed severe cardiac disturbances after intravenous injection of erythromycin. The neonate, suspected of having Chlamydia pneumonitis because of tachypnea and rhinitis, had been given 5 injections of erythromycin without clinical effect. Pallor, vomiting and bradycardia developed a few minutes after the 6th injection, and ECG showed ventricular arrhythmia, prolonged QT interval and an atrioventricular block. The infant died in intensive care unit. This case and the analysis of other published cases of cardiac disturbances following the parenteral use of erythromy…

The background adaptation of the flatfish, Paralichthys olivaceus.

The background adaptation of the flatfish, Paralichthys olivaceus, was studied by sliding beneath the fish a long strip of plastic sheet with various patterns to serve as a background. The body shade and pattern differed depending upon the pattern used. In general, the fish showed maximal pallor on a white background and darkened when lying on a black background. With a pattern of large checks, several black patches became conspicuous. The fish showed the same tendency in terms of response even when the left eye was covered. However, the fish was prevented from manifesting these changes when t…

[Papilledema. Follow-up using laser scanning tomography].

Normally, the elevation of optic disk swelling is measured by ophthalmoscopy. Laser scanning tomography (LST) permits three-dimensional analysis of distinct intraocular structures under in vivo conditions with high accuracy. We present a case of papillitis, in which the elevation could be determined as 550 microns, leading to a volume of 1.48 mm3, increasing to an elevation of 870 microns and a volume of 1.72 mm3 11 days later. Clinically, we found a swelling of about 1 D in the first examination, increasing to 2 D at the maximum. Six weeks later there was only a distinct temporal pallor to be…

Clinical profile of malaria in children--a prospective study from Aligarh (N. India).

In a prospective, hospital-based study in North India, malaria accounted for 1.5% of paediatric outpatient attendances during 1 year. A marked increase in the prevalence of malaria was noted during the post-monsoon months. Plasmodium falciparum was the causative species in 44.4% of cases, contrary to previous reports of low prevalence of this parasite in the area. Pyrexia with or without chills or rigor, vomiting, pallor and hepatosplenomegaly were the common presenting clinical features. Splenic and hepatic enlargement were seen more frequently with P. vivax than P. falciparum infections (P l…

[Motion sickness in the aerospace environment].

After a brief description of the newest and special form of motion sickness known as "space sickness" arising in space flight, and the various hypotheses on its aetiopathogenesis, motion sickness in general and the air or plane sickness deriving from atmospheric flying are discussed. The aetiopathogenesis of air sickness derives from abnormal stimulations that are primarily vestibular but also visual and somesthesic, and generated by irregular movements or variations in attitude of the plane. Reflex action than produces effects that are primarily neurovegetative (nausea, vomiting, pallor, scia…

Optic disc parameters and onset of glaucomatous field loss. I. Methods and progressive changes in disc morphology.

Serial stereoscopic fundus photographs taken in known relationship to the onset of glaucomatous visual field loss on 12 eyes were intermixed with those from 206 age- and race-matched controls and analyzed in randomized masked fashion. Progressive changes in the size, shape, or contour of the disc, and a newly described parameter, thickness of the nerve fiber layer as it crosses the disc rim, were readily apparent by the time of onset of glaucomatous field loss in all but two abnormal eyes (one case). In the latter instance, direct comparison of stereophotos indicated progressive pallor of the …

Neonatal anemia associated with Southeast Asian ovalocytosis.

The purposes of this study were to evaluate the reliability of the previously described diagnostic criteria for Southeast Asian ovalocytosis (SAO) in adults in the diagnosis of SAO in newborns and to describe the role of SAO in newborn infants presenting with pallor and jaundice. The inclusion criteria in this retrospective descriptive study were that the patient be a newborn with pallor or jaundice and with ovalocytes in the peripheral blood smear (PBS). The exclusion criteria were newborn status with other causes of neonatal hemolysis or anemia. Controls were age-matched newborn infants who …

Effects of CD11b/18 monoclonal antibody on rats with permanent middle cerebral artery occlusion.

The progression of a lesion from ischemic injury to infarct, after the permanent occlusion of a middle cerebral artery, may be influenced by … ion of body weight, counts of white blood cells in the peripheral blood, measurement of the area of pallor in the ischemic hemisphere, counts of necrotic neurons, and counts of leukocytes sequestered in the ischemic hemisphere. In experiments terminated 12 hours after the arterial occlusion (n = 4), there were fewer necrotic neurons in the group treated …

Compressive Optic Neuropathy Due to Dolichoectatic Ophthalmic Artery.

A 63-year-old man experienced progressive vision loss in his right eye over five years. Ophthalmologic examination showed right optic neuropathy with optic nerve pallor. Prior MRI findings of gadolinium enhancement near the prechiasmatic optic nerve had suggested acute optic neuritis. A repeat MRI revealed a dilated vascular structure compressing the optic nerve, confirmed by Time-of-Flight MRA as dolichoectatic ophthalmic artery. Compression-related optic neuropathy caused by a dolichoectatic ophthalmic artery can have similar appearance to acute optic neuritis on MRI. When the clinical histo…

Anterior ischemic optic neuropathy in a patient with papilledema from idiopathic intracranial hypertension.

Patients may lose vision in idiopathic intracranial hypertension from worsening papilledema and optic nerve dysfunction. Acute vision loss may also occur in this context from anterior ischemic optic neuropathy. We report a case of a 29-year-old woman with bilateral moderate papilledema from idiopathic intracranial hypertension who experienced sudden loss of vision in the superior part of the visual field of her right eye. She was found to have a new relative afferent pupillary defect and a stable superior altitudinal defect with optic disc pallor. Papilledema often creates crowding of the opti…
