An 8-year-old boy who had undergone excision of the left appendix testis for torsion of the left appendix testis about one and a half years previously was brought to our department on February 18, 1991 because of right scrotal pain of 4 days' duration. Palpation revealed induration with tenderness of the superior portion of the right testicle. A scrotal ultrasonographic tomogram revealed a shadow probably representing an enlarged appendix testis. A palpable mass in a woman's breast represents a potentially serious lesion and requires evaluation by history taking, physical examination, and mammography. The initial objective is to distinguish simple cysts from solid lesions, which can be accomplished with needle aspiration (Fig. 2). A solid lesion requires a firm diagnosis, and this usually calls for removing the lesion for histologic examination. Transrectal ultrasonography of the prostate is a relatively new diagnostic modality being used with increasing frequency in the assessment of prostatic disease. Applications include tumour staging, assisted biopsy and assessment of prostate size. The role of this study for the early detection of carcinoma of the prostate remains controversial and its role in this regard remains to be determined.

The use of mammography, to aid the diagnosis of patients with symptoms or as a screening test, presents the problem of diagnosing mammographic abnormalities not associated with a palpable lesion. To deal adequately with this, close co-operation is required amongst the radiologist, clinician/surgeon and pathologist/cytologist. A significant number of unnecessary biopsies have been carried out when each of the opinions or procedures are performed in isolation and when communication between the different specialties involved is via formal reports as compared to assessment carried out jointly.

Although fine needle aspiration is an efficacious method for diagnosing the palpable breast cancer, a negative aspirate does not exclude the presence of a malignancy. Open breast biopsy remains the standard of care in diagnosing a malignant breast lesion. All breast biopsies should be planned as if the scar were to be excised by a definitive surgical procedure (either mastectomy or tylectomy). New lesions in the treated breast require immediate biopsy because of the high likelihood of recurrent disease. Over a period of four years (September 1986 to September 1990), 32 impalpable testes were found in 24 cryptorchid boys at Sainte-Justine Hospital, Montreal. 11 (34%) of the impalpable testes were intra-abdominal. Out of one hundred and sixty-two patients that examined by contact thermography, forty-five cases involving 55 cancers were assessed for malignancy. The author's experience with lethargy in pediatric patients with intussusception was reviewed retrospectively over a 10-year period and proved to be a symptom elicited on admission in half of the patients. The results indicate that lethargy is an important symptom in patients with intussusception when occurring in association with vomiting, melena, or a palpable abdominal mass, or all three.

Neurogenic tumors in the neck region are relatively infrequent with the exception of those involving the cranial nerves. A case of a ganglioneuroma or neurofibroma involving the nerve root of C4, C5 interspace is reported. The tumor appeared as a mass within the neck with a slight amount of pain in the shoulder and upper arm upon palpation. Surgical exploration revealed a mass superficial to the foramen and erosion of the foramen. A case of a carcinoma of the urachus is described with particular reference to special pathological anatomy and clinical treatment. Due to the rareness of this kind of tumour and its unspecific symptoms its diagnosis is difficult. The therapy corresponds to the general rules of carcinoma surgery.
