Endoscopic endonasal orbital transposition to expand the frontal sinus approaches.

BACKGROUND: The management of frontal sinus disease still remains a challenging issue for rhinologists. Although endoscopic frontal sinus surgery has significantly advanced over the past years, there are some conditions in which the external approaches are still necessary. Recently, the increasing experience in endoscopic sinus surgery together with important technologic advancements have allowed expansion of the indications for selected lesions localized more and more lateral into the frontal sinus. OBJECTIVE: The aim of this study was to describe the endoscopic endonasal approach to the far

Endoscopic diagnosis, treatment, and follow-up of tumours of the nose and sinuses.

In the last decade, a great deal of attention has been focused on functional endoscopic nasal surgery for chronic sinus disease. There has been less emphasis in regard to the obvious advantages of the endoscope for diagnosis, treatment, and follow-up of tumours of the nose and paranasal sinuses. This presentation discusses in detail the reasons that the endoscopic technique is useful when applied to diagnosis of benign or malignant lesions in these areas. This new technique can be used to remove benign tumours, avoiding the external excisions that often can be disfiguring to the patient. The e

Imaging of sinonasal tumours.

More than 70 benign and malignant sinonasal tumours and tumour-like conditions have been described. However, sinonasal tumours are rare, and sinonasal cancers comprise only 3% of all head and neck cancers and 1% of all malignancies, with a peak incidence in the 5th to 7th decades and with a male preponderance. The early symptoms and imaging findings of sinonasal tumours are similar to rhinosinusitis with runny and stuffy nose, lacrimation and epistaxis and therefore neglected both by the patients and doctors. When late symptoms such as anosmia, visual disturbances, cranial neuropathy (Cn II, I

Distinctive Characteristics of Rare Sellar Lesions Mimicking Pituitary Adenomas: A Collection of Unusual Neoplasms.

Background/Objectives: Pituitary tumors account for over 90% of all sellar region masses. However, a spectrum of rare neoplastic, inflammatory, infectious, and vascular lesions-benign and malignant-can arise in the intra- and parasellar compartments and clinically and radiologically mimic PitNETs. We report a cohort of 47 such rare and cystic midline intracranial lesions, emphasizing their distinctive morphological, clinical, and imaging features and the personalized treatment strategies applied. Methods: In this retrospective single-center study, we reviewed all patients treated for suspected

Predicting Odontogenic Sinusitis in Unilateral Sinus Disease: A Prospective, Multivariate Analysis.

BACKGROUND: Odontogenic sinusitis (ODS) is a common cause of unilateral sinus disease (USD), but can be challenging to diagnose due to nonspecific clinical presentations, potentially subtle to absent dental pathology on sinus computed tomography (CT), and underrepresentation in the sinusitis literature. OBJECTIVE: Identify sinonasal clinical variables predictive of ODS in patients presenting with unilateral maxillary sinus opacification on sinus CT. METHODS: A prospective cohort study was conducted on 131 consecutive patients with USD and at least partial or complete maxillary sinus opacific

Direct approach to the anterior and lateral part of the maxillary sinus with an endoscope.

OBJECTIVE: The Caldwell-Luc (CL) approach with a sublabial incision is used to manipulate the anterior wall of the maxillary sinus. Paresthesia is one of the major complications associated with the CL approach. We developed a new method, "direct approach to the anterior and lateral part of the maxillary sinus with an endoscope" (DALMA), that negates the need for a sublabial incision and minimizes dental paresthesia by reducing the risk of anterior superior alveolar nerve injury. This study aimed to describe how to perform the DALMA technique, and to review its effectiveness and associated comp

Clinical diagnosis and treatment experience of 21 case report of isolated sphenoid sinus lesions.

RATIONALE: With the rapid development of medical science and technology, imaging has become an important part of the diagnosis of diseases, which can provide important information about the morphology and scope of the lesion and its relationship with the surrounding tissues. However, due to the fact that isolated pterygoid sinus lesions often have atypical clinical symptoms, and the location of the lesion is deep and the anatomical structure is complex, its diagnosis still faces one of the important challenges, and the misdiagnosis rate is high. The aim of this study was to explore the clinica

Open surgery versus endoscopic surgery in benign neoplasm involving the frontal sinus.

The frontal sinus, because of its proper anatomic features, has a particular relation with nasal cavities. Indeed, its anatomic opening (ostium) is strictly related to a complex ethmoidal structure prechamber mainly composed of the frontal recess. This constitutional feature makes the endoscopic approach more complex in comparison with other major sinuses treatment. In the following work, we present a systematization of surgical approach in relation to different pathologies, analyzing differences and results throughout the comparison of 2 groups: one treated with the endoscopic approach, and the

Computer Aided Design/Computer Aided Manufacturing-Guided Scapular Tip Free Flap Reconstruction for Complex Maxillofacial Defects.

BACKGROUND: Reconstruction of maxillary and mandibular defects following oncologic resection remains challenging due to their three-dimensional complexity and critical role in function and aesthetics. The scapular tip free flap (STFF) provides reliable vascularity, substantial bone stock, and chimeric versatility. Computer-Aided Design and Manufacturing (CAD/CAM) has been widely applied in reconstructive surgery to optimize resection accuracy and flap insetting, but its role in STFF supine harvest and inset has never been reported. This report aims to describe the advantages of application of
