Flap Reconstruction for Esophageal Perforation Following Anterior Cervical Plate Fixation.

Anterior cervical plate fixation is a common surgical treatment for cervical spine trauma, disc herniation, or cervical spondylosis. Esophageal perforation following anterior cervical plate fixation is a rare but serious complication. Management of esophageal perforation is controversial; however, we suggest treating most cases surgically because this condition is slow to heal and often fatal. We managed 2 cases of esophageal perforation following anterior cervical plate fixation by flap reconstruction with the pectoralis major muscle in one case and a jejunal free flap in the other. Here, we

Consistent changes in muscle phenotype and mitochondrial abundance underlie dive performance across multiple lineages of diving ducks.

Diving animals must sustain high muscle activity with finite oxygen (O2) to forage underwater. Studies have shown that some diving mammals exhibit changes in the metabolic phenotype of locomotory muscles compared with non-divers, but the pervasiveness of such changes across diving animals is unclear, particularly among diving birds. Here, we examined whether changes in muscle phenotype and mitochondrial abundance are associated with dive capacity across 17 species of ducks from three distinct evolutionary clades (tribes) in the subfamily Anatinae: the longest diving sea ducks, the mid-tier div

Structural abnormalities in the human diaphragm in drowning and hanging deaths: preliminary results.

PURPOSE: Previous experiments in rats have indicated that there are histological changes in skeletal muscle in drowning deaths; these changes include muscle fibers that contain ragged red fibers (RRF). The purpose of this study was to examine whether these changes also occur in humans. METHODS: Histologic and histochemical examinations of three muscles (diaphragm, pectoralis, and psoas) were performed on 24 cadavers with three different causes of death: 8 drowning, 8 hanging, and 8 sudden cardiac disease. Muscle samples were stained with hematoxylin-eosin, MGT, nicotinamide adenine dinucleoti

Contralateral C7 to C7 nerve root transfer in reconstruction for treatment of total brachial plexus palsy: anatomical basis and preliminary clinical results.

OBJECTIVEContralateral C7 (CC7) nerve root has been used as a donor nerve for targeted neurotization in the treatment of total brachial plexus palsy (TBPP). The authors aimed to study the contribution of C7 to the innervation of specific upper-limb muscles and to explore the utility of C7 nerve root as a recipient nerve in the management of TBPP.METHODSThis was a 2-part investigation. 1) Anatomical study: the C7 nerve root was dissected and its individual branches were traced to the muscles in 5 embalmed adult cadavers bilaterally. 2) Clinical series: 6 patients with TBPP underwent CC7 nerve t

Anesthetic experience during reconstruction surgery of a chest wall defect in a patient with Poland syndrome: A case report.

Poland syndrome, which is characterized by an absence of pectoralis muscles and their innervated nerves, is marked by variable chest wall defects due to deficits of 2nd, 3rd, 4th, and 5th ribs and costal cartilage. Additionally, there are other reported combined anomalies of the heart, lung, kidney, ipsilateral hand and foot associated with the syndrome. The lung on the side with the thoracic deformity is more hypoplastic or smaller than the lung on the unaffected side, which can cause herniation of the lung or paradoxical ventilatory movement in severe cases, thereby increasing the risk of re…

Case reports: unusual cause of shoulder pain in a collegiate baseball player.

The objective of reporting this case was to introduce a unique cause of shoulder pain in a high-level Division I NCAA collegiate baseball player. Various neurovascular causes of shoulder pain have been described in the overhead athlete, including quadrilateral space syndrome, thoracic outlet syndrome, effort thrombosis, and suprascapular nerve entrapment. All of these syndromes are uncommon and frequently are missed as a result of their rarity and the need for specialized tests to confirm the diagnosis. This pitcher presented with nonspecific posterior shoulder pain that was so severe he could

Lymphatics of the breast and the rationale for different injection techniques.

Recent studies suggest that the sentinel lymph node in the axilla can be identified no matter where the tracer is administered in the breast. Tracers injected in the skin or subareolar plexus may be the quickest to travel to the axilla but rarely identify sentinel nodes outside the axilla. Tracers administered in or around the tumor identify sentinel nodes in the axilla but also in the internal mammary chain, in the breast parenchyma, in between the pectoralis muscles, and in the supraclavicular fossa. Extra-axillary sentinel nodes can be seen in approximately 25% of the patients. Administrati…

Microfocused Ultrasound for Facial Photorejuvenation: A Review.

Microfocused ultrasound is a unique technology to treat skin laxity of the brow, lower face, and the rhytides of the décolletage. Over the past several years, the efficacy and safety of this device has been well documented and its adoption widespread. By delivering focused acoustic energy, which is converted to heat, this device creates predictable and reproducible microcoagulative zones that initiate a concentrated inflammatory wound response. By targeting the deep reticular dermis and superficial muscle and fascial planes, such as the superficial musculoaponeurotic system, platysma, and pect

A case of thoracic degloving injury with flail chest.

A 75-year-old male patient suffered a chest degloving injury when he fell on his back and was run over by a small farm tractor he was pulling. At the time of patient admission, the paradoxical motion of the right chest wall was remarkable; and he had an open fracture of the right humerus, a dislocation of the right ankle, and a laceration of the right forearm. Chest computed tomography revealed fractures of the fifth to seventh ribs and detachment of both the right pectoralis major muscle and serratus anterior muscle from the chest wall, with a disconnected right thoracic cavity. Because the r

Scapular instability associated with brachial plexus irritation: a proposed causative relationship with treatment implications.

Brachial plexus irritation and other compression neuropathies can be diverse in their presentations and can cause a myriad of signs and symptoms. The purpose of this paper is to review the pertinent anatomy, kinesiology, and neurophysiology and to outline one possible cascade of events that may contribute to more diffuse upper extremity symptoms. Scapular instability and local myofascial trigger points resulting in possible secondary muscle imbalances are described. Their possible relationship to brachial plexus irritation in addition to the implications of the irritation are also discussed. T

Muscle mass loss in breast cancer patients of reproductive age (≤ 45 years) undergoing neoadjuvant chemotherapy.

PURPOSE: Loss of muscle mass is associated with negative clinical outcome in breast cancer (BC) patients. Therefore, the aim of the study is to evaluate if there is pectoralis muscle area (PMA) depletion, reflecting loss of muscle mass, in breast cancer patients of reproductive age (≤ 45 years) undergoing neoadjuvant chemotherapy (NAC) and to correlate PMA with clinical and histopathological data. MATERIAL AND METHODS: This monocentric study, approved by our institutional review board, enrolled a total of 52 consecutive patients (mean age 37 ± 4.96 years) with histologically proven primary br

Impact of Anabolic Steroids on Tendons: A Narrative Review.

Anabolic-androgenic steroids (AAS), synthetic testosterone derivatives, are used to increase muscle mass and performance. However, high doses negatively affect tendons and ligaments, disrupting collagen balance and increasing injury risk due to a mismatch between muscle strength and tendon resilience. AAS use increases tendon rupture risk by weakening collagen structure and elasticity, despite muscle growth. Injuries often occur at major tendon sites and require imaging-based diagnosis, with surgery preferred in active individuals. Education and prevention are key due to AAS-related systemic r

Muscle Activations of the Upper Extremity and Core during Elevation and Rotational Movements in Overhead Throwing Athletes.

BACKGROUND: A strong body of literature has been published outlining muscle activity differences during sports performance in groups of overhead athletes. However, there are limited studies that have directly compared the muscle activity in overhead athletes with and without history of shoulder injury during functional everyday tasks. PURPOSE: This study aimed to identify muscle activities across fourteen upper extremity and core muscles during three functional everyday movements in athletes with and without history of shoulder injury. STUDY DESIGN: Cross-Sectional Study. METHODS: Thirty-tw

Upper limb electromyographic analysis of manual wheelchair transfer techniques in individuals with spinal cord injury: A systematic review.

BACKGROUND AND PURPOSE: The objective of this study is to determine the upper limb muscle electromyographic (EMG) activity required during various manual wheelchair transfers in the population of spinal cord injury (SCI). METHODS: This review included observational studies reporting the (EMG) activity of upper limb muscles during wheelchair transfers in people with SCI. We searched electronic databases and reference lists of relevant literature between 1995 and March 2022 with English language limits, yielding 3870 total articles. Two independent researchers performed data extraction and cond

THE ONTOGENY OF SHIVERING THERMOGENESIS IN THE RED-WINGED BLACKBIRD (AGELAIUS PHOENICEUS).

The ontogeny of shivering thermogenesis was investigated in the altricial red-winged blackbird (Agelaius phoeniceus). Two indices of heat production the rate of oxygen consumption (V(dot)O2) of the bird and the electromyographic (EMG) activity of the pectoralis (PECT) and gastrocnemius (GAST) muscles were measured simultaneously in adult and nestling red-winged blackbirds as they were subjected first to thermoneutral temperatures and subsequently to progressively colder ambient temperatures (Ta). The ontogenetic changes in both indices indicated that the capability for thermogenesi

Gaping conditions of the Pectoralis minor (tenders) in commercial broilers: Prevalence, histology, and gene expression.

Gaping is a recently described condition that affects the Pectoralis minor (tender) muscle of broiler chickens, characterized by post-mortem separation of myofiber that leads to meat depreciation and economic losses. In this study, we aimed at understanding prevalence, morphological features, and transcriptomics signatures of this poorly understood myopathy. Between July 2022 and January 2023, a total of 5,180 chicken tenders were collected from 32 flocks across two plants in the USA, handling light (2.7 kg) and heavy (4.1 kg) birds. The prevalence of moderate and severe gaping was 24.8 % and

Novel Diagnostic and Treatment Techniques for Neurogenic Thoracic Outlet Syndrome.

Neurogenic thoracic outlet syndrome is a challenging condition to diagnose and treat, often precipitated by the triad of repetitive overhead activity, pectoralis minor contracture, and scapular dyskinesia. The resultant protracted scapular posture creates gradual repetitive traction injury of the suprascapular nerve via tethering at the suprascapular notch and decreases the volume of the brachial plexus cords and axillary vessels in the retropectoralis minor space. A stepwise and exhaustive diagnostic protocol is essential to exclude alternate pathologies and confirm the diagnosis of this dyna
