Multimicrobial infection of the dental pulp triggers inflammatory responses and ultimately causes bone destruction in the periradicular tissues. Besides bacteria, noxious substances such as degraded protein components and cholesterol could also act as antigens and elicit a host response, which can be harmful to periradicular tissues. Histologically, a dense infiltration of immunocompetent cells is seen in periradicular lesions and their host reactions may induce bone resorption. Polymorphonuclear leucocytes (PMN) and macrophages migrate to the periapical lesions and phagocytose pathogens as a

INTRODUCTION: The purpose of this study was to evaluate the expression of Dickkopf-1 (DKK-1), a secreted antagonist of the Wnt (wingless)/beta-catenin signaling pathway, during the development of periapical lesions in rats.
METHODS: Periapical lesions were induced in Wistar rats by occlusal exposure of the pulp of their mandibular first molars. The animals were sacrificed randomly at 0, 7, 14, 21, and 28 day after pulpal exposure. Jaws containing the first molar were obtained and routinely prepared for histologic, immunohistochemical, and enzyme histochemical double immunofluorescence analyse

UNLABELLED: Radicular cysts are the most common odontogenic cystic lesions of inflammatory origin and are usually asymptomatic and are left unnoticed, until detected by routine radiography. It is managed either by surgical enucleation or marsupialization. Conventional nonsurgical endodontic treatment has a high degree of clinical success, but in certain cases surgical intervention becomes necessary. This is the case report of a patient presenting with unilateral radicular cyst. The lesion is managed by marsupialization using a novel and safe method of separating the soft tissue mass of the per

This study was undertaken to re-examine early and recent morphologic descriptions of gingival and periodontal inflammation based on a study … Similarly, deep penetrations of inflammatory cells into the alveolar bone, periodontal ligament and periapical tissues, along with fibrosis and enlargement of the marrow spaces, were common findings with advancing disease. More widespread distributions of inflammatory cells than previously described were found in clinically normal gingiva, while in more inflamed gi …

A total of 93 cases comprising 132 teeth with signs and symptoms or radiographic evidence of periapical pathoses were treated endodontically and were followed up for a period of 2 years. Nonsurgical management was successful in 84.4% of the cases. Histopathologic examination of tissue specimens from cases in which treatment failed revealed that only 35.7% of the cases involved apical cysts, while the remaining 64.3% involved chronic inflammatory tissue. Interestingly, 50% of the failures were observed at or 1 year after completion of treatment, which stresses the need for long-term follow-up o

BACKGROUND: Equine cheek teeth disorders, especially pulpar/apical infections, can have very serious consequences due to the frequent extension of infection to the supporting bones and/or adjacent paranasal sinuses. Limited studies have assessed the accuracy of computed tomographic (CT) imaging in the diagnosis of these disorders, and no study has directly compared imaging and pathological findings of the alveoli of diseased equine cheek teeth.
OBJECTIVE: To validate the accuracy of CT and radiographic imaging of cheek teeth disorders by comparing CT and radiographic imaging, gross and histol

BACKGROUND: In clinical practice, the buccal approach is typically the primary choice for endodontic microsurgery. Owing to the thickness of the buccal bone plate, the distance between the buccal bone plate and palatal lesion location, and soft tissue traction, the palatal approach may be more suitable for microsurgery for apical periodontitis of the palatal roots of the maxillary molars. However, the length of the palatal root, location of the greater palatine artery (GPA) and foramen (GPF), and surgical field of observation make palatal surgery challenging.

If an abscess is not able to establish drainage through the skin surface or into the oral cavity, it may spread diffusely through fascial planes of the neck's soft tissue. Once the infection descends into the submandibular space, it may extend to the lateral pharyngeal space, and then to the retro-pharyngeal space. From here, it may reach the thyroid gland. The authors here describe a case of submandibular phlegmon derived from a periapical abscess of inferior premolar, which has reached the thyroid gland. The damage caused to the gland resulted in the release of a conspicuous quantity of thyr

Antiresorptive medications are essential in treating diseases of pathologic osteoclastic bone resorption, including bone cancer and osteoporosis. Bisphosphonates (BPs) are the most commonly used antiresorptives in clinical practice. Although inhibition of bone resorption is important in regulating unwanted malignant and metabolic osteolysis, BP treatment is associated with potential side effects, including osteonecrosis of the jaws (ONJ). Recently, non-BP antiresorptive medications targeting osteoclastic function and differentiation, such as denosumab, have entered the clinical arena. Denosuma

The dental pulp is essential for tooth health and sensory function. However, conventional treatments for infected or necrotic pulp, such as root canal therapy, frequently result in tooth loss and increased fracture risk. Regenerative endodontic procedures (REPs) aim to regenerate pulp tissue while preserving tooth vitality. Exosomes from human umbilical cord mesenchymal stromal/stem cells (hUCMSCs) demonstrated potential for tissue regeneration. In this case report, the potential of hUCMSC-derived exosomes for regenerating the dental pulp of a pulpectomized tooth was investigated. The patient,

BACKGROUND: Early implant placement is one of the treatment options in postextraction sites in the anterior maxilla. Implant placement is performed after a soft tissue healing period of 4 to 8 weeks. Implant placement is combined with a simultaneous guided bone regeneration (GBR) procedure to rebuild esthetic facial hard and soft tissue contours.
METHODS: In this prospective case-series study, 20 consecutive patients treated with an implant-borne single crown were prospectively followed for 12 months. Clinical, radiologic, and esthetic parameters were recorded to assess treatment outcomes.

Necrotic immature permanent teeth with open apices pose a difficult management problem in clinical endodontics. Conventional apexification techniques resolve infection but do not support continued root development, leaving the tooth structurally vulnerable. Regenerative endodontic procedures (REPs) have emerged as a biologically oriented alternative that harnesses stem cells, scaffolds, and growth factors to form new tissue within the root canal space and encourage root maturation. This narrative review discusses the biological principles underlying REPs, the clinical protocols currently in us

Enamel pearls are developmental anomalies caused by enamel formation in atypical locations during tooth development. While the exact etiology of these developmental anomalies remains unclear, they occur in approximately 2-6 % of the population. In this case report, a 15-year-old female presented with pain in the region of the right maxillary first molar. Radiographic imaging showed a periapical radiolucency and an opaque mass in the furcation area. Cone-beam computed tomography (CBCT) identified a round radiodensity consistent with an enamel-dentin pearl, which probably promoted the developmen
