A large number of pigmentary disorders exist and although not all dermatologic entities are covered in this article, some of the pigmentary disorders that an internist or general practitioner is likely to encounter are reviewed. The clinician must be able to recognize pigmentary changes that suggest a serious underlying disease. In a situation where a malignancy or other serious illness may exist, an extensive evaluation to detect the disorder is required. This approach can allow for early treatment and referral to an appropriate specialist if necessary. The clinician must also recognize that

The pigmented purpuric dermatoses (PPD) are a group of disorders that most often occur on the lower extremities of middle-aged adults as asymptomatic, yellow-orange patches with petechiae. The PPD represent a benign, often chronic, capillaritis of unknown cause. We present a case of PPD on the abdomen of a young male and emphasize careful observation in such a patient to reveal possible progression to purpuric mycosis fungoides.

Three additional cases of hypomelanosis of Ito (HI) are reported. HI is a syndrome manifested by irregular macular hypopigmented lesions, resembling the negative image of incontinentia pigmenti. Their cutaneous manifestations, associated abnormalities and the peculiar characteristics of the reported cases are discussed.

Incontinentia pigmenti (IP) is an unusual genodermatosis occurring almost exclusively in female patients. IP is characterized by swirling hyperpigmented skin lesions and associated with a high incidence of systemic defects. Nearly one third of patients present with neurologic complications. Consequently, neurologists and other clinicians involved in the care of these patients must be able to identify the cutaneous markers and systemic findings of IP. This article discusses the clinical presentations, laboratory findings, and pathogenesis of IP. A systematic approach to the evaluation and manag

An unusual pattern of zebra-like hyperpigmentation was observed in a black male infant with congenital defects that included an atrial septal defect, dextrocardia, auricular atresia, deafness, and growth retardation. The hyperpigmentation was strikingly linear and symmetrical and followed a dermatomal distribution, with involvement of the trunk and extremities. Results of histological, histochemical, and electron microscopic studies were normal except for an increase in the number of melanocytes in the bands of hyperpigmentation. This pigmentary anomaly appears to be transient and has faded to

Five cases of progressive cribriform and zosteriform hyperpigmentation are described. The following criteria were fulfilled: (1) uniformly tan cribriform macular pigmentation in a zosteriform distribution; (2) a histologic pattern that consisted of a mild increase in melanin pigment in the basal cell layer and complete absence of nevus cells; (3) no history of rash, injury, or inflammation to suggest postinflammatory hyperpigmentation; (4) onset well after birth with gradual extension; and (5) lack of other associated cutaneous or internal abnormalities. This appears to be a newly described en

A diagnosis of primary adrenocortical insufficiency was made in a shapely, suntanned girl whose sole complaint was increasing pigmentation. Plasma cortisol was low in spite of markedly elevated levels of ACTH. Plasma cortisol, urinary 17-oxogenic steroids and urinary aldosterone did not respond to three days of ACTH stimulation. Addison's disease can be diagnosed and treated before development of anorexia, weight loss, weakness and other classical symptoms.
