Thrombophilic risk factors for placental stillbirth.

OBJECTIVES: To define the characteristics of placental stillbirth and the possible contribution of thrombophilic risk factors. STUDY DESIGN: A prospective cohort study was performed. Women diagnosed with antenatal stillbirth (>20 weeks) of singleton pregnancies between 2006 and 2008 were referred postpartum for evaluation. Maternal risk factors, fetal, placental and cord abnormalities, and a detailed thrombophilia screening, including inherited and acquired thrombophilia, were evaluated. Fetal autopsy and placental pathology were encouraged. Placental stillbirth was defined as death of a norm

Placental findings in malnourished term neonates.

BACKGROUND: The aim of the present study was to investigate placental pathologies in malnourished term neonates. METHODS: A group of term newborns was evaluated at birth for fetal malnutrition (FM) using the Clinical Assessment of Nutritional Status (CANS) score. The study group consisted of 37 malnourished neonates and 13 well-nourished newborns, including their placentas. Infants with FM were subdivided into two groups: appropriate for gestational age (AGA) and small for gestational age (SGA). RESULTS: The proportion of subjects having antepartum complications was 18.9% in the FM group, wh

Disorders of placental circulation and the fetal brain.

Disorders of the placental circulation, including the release of deleterious mediators to the fetus, are important risk factors for central nervous system complications. These disorders result in discrete patterns of placental injury detectable by a thorough placental pathologic examination. Consideration of the location, severity, multiplicity, and timing of these lesions is critical to a full understanding of their significance. Less than 10% of placentas from term infants that later develop cerebral palsy lack any evidence of placental abnormalities potentially related to adverse outcome.

How to study placental vascular development?

Both exogenous and endogenous factors during pregnancy may impact placental vascular development and cause different malformations of placental vessels. In humans, consequences of abnormal vascular development have been associated with different pregnancy-related pathologies ranging from miscarriage to intrauterine growth restriction or preeclampsia. Pregnancy-associated exposure to bacterial or viral infections or pharmacologic or toxic agents may also influence vascular development of the placenta and lead to preterm labor and delivery. Several steps of vascular adaptation on both the fetal

[Placental chorangiosis associated with abruption and hypoxia].

Chorangiosis is a rare abnormality of placental capillary proliferation, associated with hypoxia in utero. A 23-year-old primigravida developed placental abruption at 33 weeks' gestation, and an emergency cesarean section was performed. Placental histology revealed chorangiosis. The infant was in critical state and died on the next day. Our case confirms that perinatal mortality can be associated with chorangiosis. This diagnosis should be considered especially when no other significant obstetric incident is identified. Placentas should be monitored for placental abruption, a known consequence

Extreme obesity and risk of placental abruption.

BACKGROUND: Data on extreme obesity and placental abruption are scarce. This study aimed to determine the association between pre-pregnancy weight and placental abruption and whether pregnancy weight gain impacts this risk. METHODS: We used the Missouri maternally linked cohort files (years 1989-1997). Analyses were restricted to singleton live births (n = 461 729). Maternal body mass index (BMI) was classified as normal (18.5-24.9) (referent group), obese [Class 1 (30.0-34.9), Class 2 (35.0-39.9) and extreme or Class 3 (> or =40)]. Pregnancy weight gain categories included: < or =0.22 kg/wee

HELLP syndrome and placental inflammatory pathology.

HELLP syndrome, acronym for hemolysis (H), elevated liver enzymes (EL), and low platelet count (LP), is a multisystemic disease that complicates pregnancy and is considered a severe variant of hypertensive disorders in pregnancy, that causes maternal and perinatal mortality and morbidity. The pathogenesis of HELLP syndrome is not completely understood and the obstetric approach with the induction of delivery is still the only specific therapy in HELLP syndrome. It is well known that the placenta and the incomplete trophoblast invasion of spiral arteries have a central role, but especially in s

Placental gene expression and pre-eclampsia.

Pre-eclampsia and related syndromes, such as the HELLP (haemolysis, elevated liver enzymes and low platelets) syndrome, have a strong genetic component, although the nature, origin and recurrence rate of the genetic risks involved are not necessarily the same for each form of pregnancy-induced hypertension or population. This review addresses these items in the context of the recent understanding that (a) common diseases can be caused by common genetic variations, (b) the placental genotype can control the maternal phenotype, (c) epistasis and epigenetics are involved, and (d) a conserved path

[Placental insufficiency and intrauterine growth retardation].

AIM: Placental insufficiency is a pathological condition consisting of a placental functional deficit with multifactorial etiology; it can cause maternal complications such as edema, proteinuria, hypertension, etc. Our study aims to establish if placenta analysis after birth can lead to the identification of basic morphological alterations which can be easily documented and useful for the diagnosis of feto-neonatal pathologies. METHODS: We examined 60 pregnant women (45 primipara, 15 multipara). They were hospitalized in the period from March 1998 to March 2004 in different pregnancy periods

Placental lesions in a case of DiGeorge sequence.

This work describes some placental alterations found in a partial form of DiGeorge sequence, namely, hypoplasia of a cord artery with internal calcification of an extensive endoluminal thrombosis, and widespread calcification of microthrombi in the arteries of the second and third order villous branches. Hypoplasia of a cord artery is a relatively rare event, and is also associated with malformations of the gastroenteric and cardiovascular system, as sometimes described in the DiGeorge sequence. Interesting placental alterations are reported and their likely physiopathologic basis and pathogen

Amniotic fluid and placental stem cells.

Human amniotic fluid has been used in prenatal diagnosis for more than 70 years. It has proven to be a safe, reliable, and simple screening tool for a wide variety of developmental and genetic diseases. However, there is now evidence that amniotic fluid may have more use than only as a diagnostic tool and may be the source of a powerful therapy for a multitude of congenital and adult disorders. A subset of cells found in amniotic fluid and placenta has been isolated and found to be capable of maintaining prolonged undifferentiated proliferation as well as able to differentiate into multiple ti

Prenatal laser treatment of a placental chorioangioma.

Large placental chorioangiomas are rare complications in pregnancy. We present a case of a placental chorioangioma, complicated by polyhydramnios, in which obliteration of its blood supply using diode laser coagulation at 25 weeks of gestation was followed by an uneventful pregnancy and delivery of a healthy baby with a normal follow-up at the age of 9 months. Laser coagulation of the feeding vessels seems to be an effective treatment for chorioangiomas when the blood supply is superficial and the feeding vessels are small in diameter.

Metabolic syndrome and the risk of placental dysfunction.

BACKGROUND: Placental dysfunction (PD), which may manifest partly as the hypertensive disorders of pregnancy and abruption or infarction of the placenta, occurs more commonly in women with obesity, chronic hypertension, diabetes mellitus, and dyslipidemia-each a major feature of the metabolic syndrome. However, the relationship between the metabolic syndrome and the future risk of PD or fetal demise is unknown. METHODS: We completed a retrospective cohort study of 1.03 million women who had a first documented delivery in the province of Ontario between 1990 and 2002. Using linked administrati

Risk and risk estimation of placental abruption.

OBJECTIVE: Several variables related to increased risk of placental abruption are also risk factors for venous thromboembolism. Prior second trimester-, third trimester, and repeated fetal loss are reported to be associated to thrombophilias. However, it is yet not known if they are also related to placental abruption. STUDY DESIGN: A retrospective case-control study of 161 women with placental abruption and 2371 unselected gravidae without placental abruption. The medical files were scrutinized and the selected variables were investigated in relation to the development of placental abruption

Placental thickness in the first half of pregnancy.

PURPOSE: This study was conducted to establish normal values of placental thickness during the first half of pregnancy. METHODS: Normal pregnant women with singleton pregnancies between 8 and 20 weeks of gestation were recruited into the study. All the newborns were normal at birth. Placental thickness was measured perpendicularly through the thickest part of the placenta on transabdominal scans. The placental thickness data were analyzed for mean, standard deviation, 95% confidence interval, and 2.5(th), 5(th), 50(th), 95(th), and 97.5(th) percentile for each week of gestational age.
