Plasma cell granuloma (PCG) of the lung is a rare disease that usually presents as a pulmonary nodule or mass on incidental radiographic examination without symptoms. Although the etiology of PCG is still controversial, many findings have lent support to the lesion being a reactive inflammatory process rather than a neoplastic one. We present the case of a patient who presented with small joint polyarthralgia and mild lymphocytosis. The patient was subsequently diagnosed to have plasma cell leukaemia. Skeletal survey showed bony lytic lesions in the hands and feet with no skeletal lesions elsewhere. The most common sites of bony involvement in plasma cell dyscrasia are skull, vertebrae, ribs and long bones. Plasma cell myeloma (PCM) of the skull base is rarely encountered in neurosurgical practice. PCM has a wide spectrum of pathology, including a quite benign, solitary plasmacytoma (SPC), and an extremely malignant, multiple myeloma (MM) at the two ends of the spectrum. We report six cases of plasma cell balanitis and four of plasma cell vulvitis. The patients' ages ranged from 26-88 years in males and 30-60 in females. All the men were uncircumcised and presented with a discharge and erythematous lesions on the glans penis and prepuce. Plasma cellular osteomyelitis was seen in the metaphysis while sclerosing osteomyelitis affected the diaphysis of long bones. In only about 20% of both forms local bacteria were noted. The onset of symptoms was inconspicuous and both clinical and radiological signs were nonspecific. Curettment or resection usually cured the disease. In some cases, however, aggressive surgery was necessary. As to surgery and outcome sclerosing osteomyelitis appeared less riskful than plasma cellular osteomyelitis. Both forms apparently represent the same basic pathological condition.

We present a case of primary plasma cell leukemia with Bence Jones proteinuria. After combination chemotherapy, leukemic cells and the urinary levels Bence Jones protein were decreased. Small lytic bone lesions were detected only in the skull. Typical plasma cells were rarely seen in peripheral blood on the hyperleukocytic phase, however they were increased in the advanced stages. The most important diagnostic sign was persistent expression of CD38 antigen on leukemic cells throughout the entire course of the illness and these leukemic cells expressed very late antigen-4 (VLA-4) but not VLA-5.

Investigation of biopsies of a suprasellar tumour in a man, 33 years of age, and a supratentorial tumour in a woman, 18 years of age, resulted in histological diagnosis of intracranial plasma cell granuloma, a rare condition. Polyclonality of the plasma cells involved in the process could be verified by immunohistochemical investigation. Chordoma was clinically assumed in the first case and meningioma in the second. Biological behaviour and dubious prognosis are discussed, with reference being made to relevant literature.

Plasma cell pododermatitis was diagnosed in 2 cats with enlargement of the metacarpal and metatarsal footpads, ulceration of one of the affected footpads, and a history of chronic hemorrhage from the ulcerations. Intraabdominal pseudotumor of the plasma cell granuloma type in a 19-year-old man is reported. The patient presented with fever and weight loss lasting months, and the laboratory findings revealed high sedimentation rate, hypochromic, microcytic anemia, thrombocytosis, elevated alkaline phosphatase, decreased thromboplastin time and polyclonal hypergammaglobulinemia. When the plasma cell granuloma was removed, all laboratory findings returned to normal within 3 months and the patient remained asymptomatic during the two-year follow-up.

Acid phosphatase activity was studied cytochemically in bone marrow plasma cells of 32 multiple myelomas, 45 non-myelomatous monoclonal gammopathies and 20 normal subjects. We have found significant differences among these three groups (P less than 0.001). The usefulness of this cytochemical reaction for the study of monoclonal gammopathies is discussed.
