Guillan-Barré syndrome is an acute, symmetrical polyneuropathy with distinctive features. The early clinical course involves painful paresthesia that is usually followed by proximal motor weakness. Albuminocytologic dissociation in the cerebrospinal fluid is considered diagnostically important. Therapy ranges from supportive measures including physical therapy to surgical intervention for residual deformities. A case with pedal manifestations is presented.

Oral poliovirus vaccine (OPV) developed by A. Sabin has been effectively used to control poliomyelitis in Brazil, and the last case with the isolation of a wild poliovirus strain occurred in March 1989. Although the vaccine controlled the circulation of wild strains and poliomyelitis cases associated with these strains were not detected during the last eight years, rare cases classified as vaccine-associated paralytic poliomyelitis (VAPP) have been detected. Molecular characterization studies of poliovirus strains isolated from VAPP cases and from healthy contacts have confirmed that the isola

A rapidly progressive, generally symmetric, ascending flaccid paraparesis or quadriparesis that develops in an infant or child constitutes an uncommon but important pediatric neurologic emergency that requires immediate evaluation and treatment. The differential diagnosis primarily includes acute neuropathies, most commonly the childhood Guillain-Barré syndrome and, rarely, acute transverse myelitis or infantile poliomyelitis. A clinical distinction may be difficult in the younger child in whom detailed sensory examination is not possible. Although most children with Guillain-Barré syndrome us

Demyelinating conduction block is an important hallmark to distinguish chronic inflammatory demyelinating polyneuropathy (CIDP) from hereditary neuropathies, motor neuron disease and other axonal type neuropathies. The electrodiagnostic criteria of partial conduction block proposed by Asbury and Cornblath, more than 20% drop in CMAP amplitude between proximal and distal sites (e.g. elbow and wrist), may be inadequate, because 20% to 30% drop sometimes occurs in cases with hereditary demyelinating neuropathies. Rhee et al showed that pure temporal dispersion can produce amplitude reduction up t

This study reported four cases of variant of Guillain-Barre Syndrome (GBS). Clinical features showed muscle fasciculation, Fisher Syndrome, GBS with family Leber's disease and idiopathic dysautonomia. There were oligoclonal protein in 3 patients and elevation of protein of CSF in 4 cases. The neurogenic lesion of limb muscle was confirmed by EMG and NCV in all patients. The sural nerve biopsy was characterized by inflammatory demyelination. The diagnosis and pathogenesis were discussed.

Two patients presented with an acute syndrome of internal ophthalmoplegia in the absence of external ophthalmoplegia, bilateral involvement of other cranial nerves, and minimal evidence of peripheral neuropathy. Cerebrospinal fluid protein was slightly raised, and moderate slowing of nerve conduction velocity was observed peripherally. It is suggested that these cases may represent a variant of acute post-infectious polyneuritis.

Four of 106 consecutive patients with acute Guillain-Barré syndrome had preceding bacterial enteritis caused by Campylobacter jejuni. One had a severe illness with axonal damage and poor outcome; three had typical courses with good recovery. Preceding illnesses in the other 102 patients were the following: diarrhea in nine (with negative stool cultures in five), upper respiratory tract infections in 46, hepatitis in six, other infectious illnesses in eight, and none in 33. Eight patients previously reported with Campylobacter associated with Guillain-Barré syndrome, and the serologic and clini

A case of the Guillain-Barré-Strohl syndrome ( GBSS ) in a 7 year-old-patient convalescing from infectious hepatitis possibly by virus A is presented. The GBSS represent a rare complication from this disease and in more than 50% of all cases reported in the literature it appeared after the icteric phase. Other neurologic complications are described as follows: cranial nerve involvement, encephalitis, myelitis, meningitis, peripheral neuropathy, tremor syndrome, behaviour dysfunction and focal neurological signs.

Primarily a disease of the Peripheral Nervous System, GBS occurs most frequently following a viral infection. The exact cause is unknown. The diagnosis of GBS depends mainly on clinical, laboratory, and electrodiagnostic findings. Generally, the patient will present with a progressive motor weakness of more than one extremity and areflexia. There will also be a predictable progression including symmetry of motor involvement, mild sensory symptoms, and recovery. Cranial nerves as well as the ANS have also been involved in GBS. Recovery from the disease is generally good although it may take man

A CSF electrophoresis on a particular gelatinized cellulose acetate (Cellogel RS) has been tested. The method allows the routine separation of many protein fractions in concentrated CSF in a very brief time and affords the recognition and quantification of oligoclonal banding, that is of extreme importance in the diagnosis of some neuropathies as multiple sclerosis.

Chronic inflammatory demyelinating polyneuropathy (CIDP) is defined as a dysimmune disorder of the peripheral nervous system (PNS) resulting in sensorimotor deficits. However, an increasing body of data suggests that CIDP also goes along with features, such as autonomic, circadian, fatigue, mood, and subtle cognitive dysfunctions. Some of these non-sensorimotor symptoms (NSMS) challenge the concept of an exclusive PNS disease. Pragmatically, the high prevalence of NSMS calls for a more holistic disease assessment and surveillance to achieve optimal therapeutic results.

BACKGROUND: Immune-related adverse events (irAEs) induced by immune checkpoint inhibitors vary widely, raising diagnostic challenges. The management of suspected irAEs requires close collaboration between oncologists and organ specialists. Rarely, irAEs can occur as overlap syndromes and may require intensive care.
CASE SUMMARY: A 69-year-old man required intensive care for severe overlap syndrome induced by a programmed death-ligand 1 inhibitor. The presentation as recurrent hypercapnic coma complicated the diagnosis of polyradiculoneuritis combined with myocarditis. The finding of anti-titi

Chronic inflammatory demyelinating polyneuropathy (CIDP) is a rare neurological disorder that causes progressive weakness, sensory dysfunction, and often debilitating muscle spasms and pain. A man in his early 30s with a history of Hodgkin's lymphoma treated with chemotherapy developed neurologic symptoms initially misdiagnosed as chemotherapy-induced neuropathy. A diagnosis of CIDP was subsequently confirmed by nerve conduction studies. Despite treatment with muscle relaxants, benzodiazepines, botulinum toxin injections, and opioids, the patient's muscle spasms remained refractory. Dantrolene

BACKGROUND: Autoimmune nodopathy (AN) has emerged as a novel diagnostic category that is pathologically different from classic chronic inflammatory demyelinating polyneuropathy. Clinical manifestations of AN include sensory or motor neuropathies, sensory ataxia, tremor, and cranial nerve involvement. AN with a serum-positive contactin-1 (CNTN1) antibody usually results in peripheral nerve demyelination. In this study, we reported a rare case of AN with CNTN1 antibodies characterized by the presence of CNTN1 antibodies in both serum and cerebrospinal fluid, which is associated with cerebellar d

Background and Objectives: Studies have suggested that, by applying certain nerve ultrasound scores, demyelinating and axonal polyneuropathies can be differentiated. In the current study, we investigated the utility of ultrasound pattern sub-score A (UPSA) and intra- and internerve cross-sectional area (CSA) variability in the diagnostic evaluation of demyelinating neuropathies. Materials and Methods: Nerve ultrasound was performed in patients with chronic inflammatory demyelinating polyneuropathy (CIDP) and acute inflammatory demyelinating polyneuropathy (AIDP) and compared to patients with a
