[Diffusion weighted imaging of SCA3/MJD and SPG4].

OBJECTIVE: To determine the value of diffusion weighted imaging (DWI) in the diagnosis of hereditary spinocerebellar ataxia 3 and the Machado Joseph disease (SCA3/MJD) and hereditary spastic paraplegia 4 (SPG4). METHODS: We scanned 13 patients with SPG4, 30 patients with SCA3/MJD (21 onset patients and 9 with only genetic abnormalities), and 27 healthy volunteers with DWI. The processing data were apparent diffusion coefficient (ADC). The above data were grouped for comparative study. RESULTS: In the precentral gyrus, posterior limb of the internal capsule, cerebral peduncle, pons, cerebella…

Midbrain atrophy in vascular Parkinsonism.

BACKGROUND: Midbrain atrophy is a well-known feature of progressive supranuclear palsy (PSP). Some clinical features of vascular parkinsonism (VP) such as pseudobulbar phenomena, lower body predominance and early postural instability suggest that the brainstem could be associated with VP. The aim of this study was to determine whether midbrain atrophy was present in patients with VP. METHODS: We measured the midbrain (Amd) and pons area (Apn) of 20 patients with VP, 15 patients with probable PSP and 30 patients with idiopathic Parkinson's disease (IPD). The Amd and Apn were measured on mid-sa…

Region-specific changes in gene expression in rat brain after chronic treatment with levetiracetam or phenytoin.

PURPOSE: It is commonly assumed that antiepileptic drugs (AEDs) act similarly in the various parts of the brain as long as their molecular t … microarrays. RESULTS: Levetiracetam (150 mg/kg for 90 days) changed the expression of 65 genes in pons/medulla oblongata, two in hippocampus, and one in frontal cortex. Phenytoin (75 mg/kg), in contrast, changed the expression of only three genes in pons/medulla oblongata, but 64 genes in hippocampus, and 327 genes in frontal cortex. Very little over …

Transpontine connection fibers between corticospinal tracts in hemiparetic patients with intracerebral hemorrhage.

OBJECTIVES: A connection of fibers between corticospinal tracts (CSTs) at the pons, originating from the CST of the affected hemisphere, has been observed in hemiparetic patients with stroke. The authors investigated the incidence and the clinical significance of transpontine connection of fibers (TCFs) in hemiparetic patients with intracerebral hemorrhage (ICH), using diffusion tensor tractography (DTT). SUBJECTS AND METHODS: Forty-two patients with ICH with weakness of the affected extremities at the time of DTI scanning and 41 age-matched control subjects were recruited. TCFs were classifi…

Central pontine and extrapontine myelinolysis associated with type 2 diabetic patient with hypokalemia.

Central pontine myelinolysis (CPM) is a demyelinating disease of the pons often associated with the demyelination of extrapontine areas of the central nervous system. Although the etiology and pathogenesis are unclear, CPM is usually associated with hyponatremia or its rapid correction, and chronic alcoholism is also a common underlying condition. We observed a 43-year-old man with diabetes mellitus who developed central pontine and extrapontine myelinolysis with no apparent evidence of hyponatremia, serum hyperosmolality or associated rapid correction, or history of alcohol abuse. On admissio…

[A case of infarction in the pontine tegmentum involving the bilateral MLF and unilateral paramedian pontine reticular formation].

A 61-year-old man with lesions of the bilateral medial longitudinal fasciculus (MLF) and left paramedian pontine reticular formation (PPRF) … aramedian portion of the midpontine tegmentum. It suggests that WEBINO is caused by a lesion in the pons, and that paralytic pontine exotropia, one and a half syndrome and WEBINO are considered to be analogous to one another. The associated abnormality of vergence eye movements suggests that these are also generated by the PPRF in the pons.

Elimination of vasopressin analgesia following lesions placed in the rat hypothalamic paraventricular nucleus.

Pain thresholds are increased following central administration of arginine vasopressin (AVP), an effect which appears not to be mediated thr … ed in animals with lesions placed in the PVN at either 7 or 35 days after surgery. VP levels in the pons-medulla and the lumbo-sacral spinal cord were measured by radioimmunoassay, as well as VP-like immunoreactivity in the PVN and spinal cord with immunocytochemistry. Beeswax Alcohol Prevents Low-Density Lipoprotein Oxidation and Demonstrates Antioxidant Activities in Zebrafish Embryos and Human Subjects: A Clinical Study.

Oxidative stress is one of the primary instigators of the onset of various human ailments, including cancers, cardiovascular diseases, and dementia. Particularly, oxidative stress severely affects low-density lipid & protein (LDL) oxidation, leading to several detrimental health effects. Therefore, in this study, the effect of beeswax alcohol (BWA) was evaluated in the prevention of LDL oxidation, enhancement of paraoxonase 1 (PON-1) activity of high-density lipid & protein (HDL), and zebrafish embryo survivability. Furthermore, the implication of BWA consumption on the oxidative plasma variab

Continued in situ DNA fragmentation of microglia/macrophages in white matter weeks and months after traumatic brain injury.

Paraffin-embedded material from the pons of head-injured patients whose disability could be attributed to diffuse traumatic axonal injury, and controls, was identified from the department's archive. The cases were divided into three groups based on survival, viz Group 1 (n = 5) who survived for between 4 and 8 weeks, Group 2 (n = 5) for between 3 and 9 months, and Group 3 (n = 5) who survived for more that 12 months. Sections were stained by the TUNEL (TdT-mediated UTP nick end labelling) technique, and by H&E, LFB/CV and immunohistochemically for astrocytes (GFAP) and microglia/macrophages (C…

Interactions between rostral pontine and ventral medullary respiratory neurons.

Lesioning studies have demonstrated that the respiratory rhythm is generated within the brain stem and that connections between the pons and the medulla must be intact for the generation of eupneic breathing in the decerebrate or anesthetized vagotomized cat. However, the nature of proposed functional connections between pontine and medullary respiratory neurons is not well understood. The possibility of interactions between respiratory neurons of the rostral pons (n. parabrachialis medialis, Kölliker-Fuse nucleus) and the ipsilateral ventral respiratory group (VRG; n. retroambigualis, n. ambi…

Typical "brainstem chocolate chip sign" in heterozygous HTRA1-related cerebral small vessel disease.

We report a 59-year-old man with heterozygous HTRA1-related cerebral small vessel disease (hHTRA1-CSVD) presenting with vascular dementia and gait disturbances. Susceptibility-weighted imaging revealed multiple round or ovoid hypointensities around the midbrain, pons, and medulla-termed the "brainstem chocolate chip sign." This feature may represent leptomeningeal venous abnormalities and may serve as a specific imaging marker for hHTRA1-CSVD.

Alternating Hemiplegia with Ipsilateral Supranuclear Facial Palsy and Abducens Nerve Palsy Caused by Pontine Infarction.

A 62-year-old right-handed man was diagnosed with a cerebral infarction in the ventromedial region of the left lower pons. He showed left abducens nerve palsy, left-sided supranuclear palsy of the lower part of the face and right hemiparesis. We hypothesized that the mechanism underlying the patient's ipsilateral supranuclear facial palsy involved the corticofacial fibers after they crossed the midline.

Acute disseminated encephalomyelitis--another cause of post malaria cerebellar ataxia.

The aetio-pathogenesis of delayed onset cerebellar ataxia following Plasmodium falciparum malaria is uncertain. An autoimmune demyelinating pathology has been suspected though not yet definitively substantiated. In the present communication we report a case of delayed onset cerebellar ataxia following acute falciparum malaria, where magnetic resonance imaging revealed demyelinating lesions in the pons and cerebellar peduncles which disappeared after resolution of symptoms.

Epidermoid cyst involving the brain stem.

Brain stem epidermoid cysts are extremely rare. We describe an unusual case of prepontine epidermoid cyst with a large part of the tumour insinuating itself into the pons. Microsurgical total excision of tumour was done without producing any new neurological deficits. Cases reported in the literature either had high mortality/morbidity or were only subtotally removed. Relevant literature is reviewed.

Acetylcholinesterase activity in rat brain: effect of acute cyanide intoxication.

Acetylcholinesterase (AChE) activity in the cerebral cortex, cerebellum, hippocampus, hypothalamus, midbrain, pons and medulla oblongata of mature male rats was determined in control and cyanide-treated animals by a spectrophotometric method. Compared with control, cyanide-treated animals showed significant increases in the AChE activity of the cerrebral cortex, hippocampus and midbrain. This finding suggests an involvement of brain AChE in acute cyanide toxicity.

Cavernous Malformation of the Trigeminal Nerve Manifesting as Facial Hypoesthesia: A Case Report and Literature Review.

Cavernous malformations are relatively common vascular lesions of the central nervous system. These lesions can occur along the entire neuroaxis; however, they are very rare in cranial nerves. Trigeminal cavernous malformations can be classified into four types, according to their location: type G (Gasserian ganglion), type C (between the cisternal and intra-axial portions), type P (intra-axial trigeminal nerve root in the pons), and type S (spinal tract of the trigeminal nerve root below the medulla oblongata). In our review, we found a total of 27 cases of cavernomas affecting the trigeminal…

Osmotic Demyelination Syndrome in a Patient With Schizophrenia: A Case Report.

BACKGROUND: Primary polydipsia affects approximately 11%-20% of patients with chronic schizophrenia, and up to one-third develop symptomatic hyponatremia. Rapid correction of hyponatremia can precipitate osmotic demyelination syndrome (ODS), involving demyelination in the central pons and extrapontine regions. ODS presenting with severe extrapyramidal symptoms (EPS) in schizophrenia has not been previously documented. CASE PRESENTATION: A 34-year-old man with chronic schizophrenia, maintained on oral olanzapine and monthly flupentixol decanoate, developed polydipsia-induced hyponatremia. His …
