Postural muscle dyscoordination in children with cerebral palsy.

The present paper gives an overview of the knowledge currently available on muscular dyscoordination underlying postural problems in children with cerebral palsy (CP). Such information is a prerequisite for developing successful therapeutic interventions in children with CP. Until now, three children with CP functioning at GMFCS (Gross Motor Function Classification System) level V have been documented. The children totally or partially lacked direction specificity in their postural adjustments and could not sit independently for more than 3 seconds. Some children functioning at GMFCS level IV

Accuracy of clinical estimation of abnormal head postures.

PURPOSE: Correct diagnosis and analysis of an abnormal head posture (AHP) is critical in the clinical assessment of patients with strabismus and nystagmus. This study was performed to evaluate the accuracy and precision of clinicians estimating the degree of face turns and head tilts. DESIGN: Experimental simulation study. PARTICIPANTS: Two normal volunteers and 4 pediatric and 1 general ophthalmologists. METHODS: Using a cervical range of motion (CROM) device, single axis head position (chin up, chin down, right and left face turn, and head tilts) were modeled, and clinical estimates of th…

Paradoxical postural headaches in cerebrospinal fluid leaks.

Two patients with cerebrospinal fluid (CSF) leak, one at the level of fourth thoracic spine and another with undetermined level of leak, presented with paradoxical postural headaches in that the headaches were present when in a horizontal position and resolved if the patients were upright. One patient improved spontaneously and the other responded to a targeted epidural blood patch. Paradoxical postural headache is yet another headache type that can be associated with CSF leak and CSF volume depletion. Its mechanism is uncertain, but it could be related to congestion and dilatation of cerebral

Primitive reflexes and postural reactions in the neurodevelopmental examination.

The primitive reflexes and the postural reactions comprise one of the earliest, simplest, and most frequently used tools among child neurologists to assess the central nervous system integrity of infants and young children. Infants with cerebral palsy have been known to manifest persistence or delay in the disappearance of primitive reflexes and pathologic or absent postural reactions. The clinical significance of asymmetric tonic neck reflex, Moro, palmar grasp, plantar grasp, Galant, Babinski, Rossolimo, crossed extensor, suprapubic extensor, and heel reflex, alone or in combination, as well

Quality of life in patients with postural tachycardia syndrome.

OBJECTIVES: To quantify quality of life and identify demographic and clinical correlates of functioning in a well-characterized sample of patients with postural tachycardia syndrome (POTS). PATIENTS AND METHODS: Prospective patients were those seen at the Mayo Clinic Autonomic Disorders Laboratory from September 2000 to June 2001. Neurologists made diagnoses of POTS according to established criteria. Patients completed a questionnaire packet that included measures of quality of life (36-Item Short-Form Health Survey [SF-36]) and symptom severity (Autonomic Symptom Profile). Additional clinica

Postural control and glycerol test in Ménière's disease.

Ménière's disease is a clinical disorder, characterized by fluctuating hearing loss, recurrent spontaneous episodic vertigo, tinnitus and aural fullness, which may be defined as the idiopathic syndrome of endolymphatic hydrops. The most important test for diagnosis of Ménière's disease is the glycerol test. This is a simple and rapid method and several authors have confirmed its efficiency for identifying endolymphatic hydrops. This test provides information on the cochlear response to the osmotic changes produced by glycerol in the inner ear, whereas modifications in the vestibular labyrinth

Idiopathic orthostatic intolerance and postural tachycardia syndromes.

Upright posture imposes a substantial gravitational stress on the body, for which we are able to compensate, in large part because of the autonomic nervous system. Alteration in autonomic function, therefore, may lead to orthostatic intolerance. On one extreme, patients with autonomic failure caused by degenerative loss of autonomic function are severely disabled by orthostatic hypotension and may faint whenever they stand up. Fortunately, such patients are relatively rare. On the other hand, disabling orthostatic intolerance can develop in otherwise normal young people. These patients can be …

[Postural hypotension with non-neurogenic triggers in the elderly].

INTRODUCTION: Orthostatic hypotension (OH) symptoms are the most frequent reason for patients to undergo an autonomic examination. The incidence of these symptoms is greater in two groups: young people with neurally mediated syncope, and adults older than 60 years, all who have normal results in the EKG and EEG test. The baroreflex afferents induce a continuous blood pressure adjustment through the heart rate variation. There are functional changes with aging which reduce this baroreflex sensitivity and therefore the accommodation to the haemodynamic requirements is frequently impaired. A numb

Disorientation and postural ataxia following flight simulation.

BACKGROUND: Motion sickness-like symptoms can afflict pilots training in military simulators. This simulator sickness involves symptoms of gastrointestinal distress, eyestrain and disorientation. A simulator sickness Questionnaire (SSQ) with subscales available for each of these dimensions has been developed to assess the problem. HYPOTHESIS: This study examined the hypothesis that there is a strong correlation between the SSQ subscale which summarized self report of disorientation symptoms and an objective measure of post-simulation postural instability. METHODS: Data from two Navy simulato

Postural tachycardia syndrome: time frequency mapping.

Orthostatic tachycardia is common but its specificity remains uncertain. Our preliminary work suggested that using autonomic function testing in conjunction with time-frequency mapping (TFM), it might be possible to characterize a subset of the postural tachycardia syndrome (POTS), that is due to a restricted autonomic neuropathy. We describe 20 patients (17 women and 3 men, aged 14-43 years) with florid POTS and 20 controls (14 women and 6 men, aged 20-41 years). Autonomic failure was quantified by its distribution (cardiovagal, adrenergic and sudomotor) and severity, a symptom profile was ge

Postural hypotension in diabetic autonomic neuropathy: a review.

Postural hypotension is uncommon in diabetes but can occur secondary to autonomic neuropathy. Symptoms are rare and include dizziness, weakness, blurred vision, tiredness, and loss of consciousness. The pathophysiology of postural hypotension is not clear, but changes in intravascular volume, heart rate, cardiac output, and splanchnic vascular resistance are similar in patients and controls. The main factors producing hypotension are a blunted catecholamine response to standing, and failure of lower limb vascular resistance to increase adequately. Treatment for symptomatic postural hypotension

Phobic postural vertigo: a first follow-up.

Seventy-eight patients with phobic postural vertigo (PPV) and 17 patients with psychogenic disorder of stance and gait (PSG) were asked to evaluate their condition 6 months to 5.5 years after their original referral and short-term psychotherapy. Two results seem most important: (1) PPV had a favorable course with a 72% improvement rate (22% of patients becoming symptom free), whereas the majority of patients with PSG (52%) remained unchanged; (2) the majority of patients with PPV experienced complete remission or considerable improvement even if their condition had lasted between 1 and 20 year

[Evaluation of postural sway in normals and patients with vertigoes].

A measurable parameter in the study of static equilibrium was described. The locus of postural sway was measured with a set of computer during 1 minute period in 68 normal subjects and 82 patients suffering from central and peripheral vertigoes with eyes closed. The results demonstrated: 1) There are five patterns of postural sway: centripetal, forward-backward, right-left, diffuse and multicentric patterns; centripetal pattern was the most prominent figure pattern in the normal subjects, diffuse pattern was the most prominent figure pattern in the central vertigoes. 2) Length of locus and vel

Characteristics of postural stability in patients with aminoglycoside toxicity.

Postural stability in patients with bilateral vestibular deficits from aminoglycoside toxicity was characterized by examining their ability to use different sensory cues to maintain balance and by recording their automatic postural responses to sudden translational and rotational (pitch) perturbations of the support surface. We found our patients had increased sway on sensory tests in which either visual or somatosensory cues were altered and were unable to maintain their balance when both visual and somatosensory cues were altered compared to age-matched normal subjects. The amount of vestibu

Postural control of fish related to gravity input.

The disappearance of gravity input during space flight causes many functional disorders in animal and human postural control system. On the other hand, it has been shown that these functional deficits reduce or disappear with time and adapt to weightlessness. Unusual fish behavior was reported during the first few days of orbit flight and also during postflight recovery in Skylab 3 experiment. Similar fish behavior was observed in microgravity obtained during the parabolic flight of airplane. These results suggest that fish might provide a good model for investigating adaptation of sensory-mot

A postural model of balance-correcting movement strategies.

The patterns of joint torques and movement strategies underlying human balance corrections were examined using a postural model. Two types of support-surface perturbation, dorsiflexion rotation (ROT) and rearward translation (TRANS), were employed. These two perturbations were adjusted to produce similar profiles of ankle dorsiflexion in order to obtain information on the role of lower leg proprioceptive inputs on triggering balance corrections. In addition, the dependence of balance control on head angular and linear accelerations was investigated by comparing the responses of normal and vest

Postural sway following inversion sprain of the ankle.

The single-limb sway of 20 individuals with a history of unilateral inversion ankle sprain was compared to that of a control group of 30 individuals without a history of ankle sprain. Using a force platform to obtain center-of-pressure data, the linear distance traveled (mm) and the mean power frequency, (Hz) of postural sway were calculated for each subject. The results of this study showed that postural sway amplitude was significantly greater in the injured group than in the control group. Contrary to previous investigations, this study indicates that individuals with a history of inversion

Cardiovascular response during postural change in the elderly.

In order to evaluate the heart rate and blood pressure response during sitting and standing, as well as during the change from sitting to standing, we studied 40 healthy men and women, aged 60-76 years, by means of a noninvasive beat-to-beat blood pressure tracking cuff. Heart rate and blood pressure curves immediately after standing exhibited pronounced flattening relative to a younger population, with no differences between men and women. Only four subjects showed systolic blood pressure drops of at least 15 mmHg in response to standing. Both handgrip and mental arithmetic significantly modi

Postural hypotension: its meaning and management in the elderly.

Aging is associated with a considerable number of alterations in function of the autonomic nervous system and with systems involved in the control of cardiovascular response to postural changes. However, these alterations themselves do not generally lead to symptomatic orthostatic hypotension. In combination with other factors, older patients can develop marked problems with orthostatic hypotension--notably, certain drug regimens, some degree of underlying heart failure, or such common geriatric illnesses as parkinsonism and diabetes. Treatment regimens must be designed to minimize side effect
