Development and evaluation of a predictive algorithm for unsatisfactory response among patients with pulmonary arterial hypertension using health insurance claims data.

OBJECTIVE: This study aimed to develop and validate a predictive algorithm for unsatisfactory response to initial pulmonary arterial hypertension (PAH) therapy using health insurance claims. METHODS: Adult patients with PAH initiated on a first PAH therapy (index date) were identified from Optum's de-identified Clinformatics Data Mart Database (1/1/2010-12/31/2019). A random survival forest algorithm was developed using patient-month data and predicted the "survival function" (i.e. risk of not having unsatisfactory response) over time. For each patient-month observation, risk factors were ass

How Does the Skeletal Oncology Research Group Algorithm's Prediction of 5-year Survival in Patients with Chondrosarcoma Perform on International Validation?

BACKGROUND: The Skeletal Oncology Research Group (SORG) machine learning algorithm for predicting survival in patients with chondrosarcoma was developed using data from the Surveillance, Epidemiology, and End Results (SEER) registry. This algorithm was externally validated on a dataset of patients from the United States in an earlier study, where it demonstrated generally good performance but overestimated 5-year survival. In addition, this algorithm has not yet been validated in patients outside the United States; doing so would be important because external validation is necessary as algorit

Development of a macrophages-related 4-gene signature and nomogram for the overall survival prediction of hepatocellular carcinoma based on WGCNA and LASSO algorithm.

BACKGROUND: Immune system instability and poor prognosis are the two major clinical performance of hepatocellular carcinoma (HCC). Abnormal expression of MiR-424-5p has been reported to accelerate the progression of liver cancer, but it mediated immune cell infiltration imbalance is still unknown. We aim to mine the immune-related genes (IRGs) targeted by miR-424-5p and construct a multi-gene signature to improve the prognostic prediction of HCC. METHODS: The HCC-related data of the cancer genome atlas (TCGA) database and the GSE14520 dataset of the gene expression omnibus (GEO) database were

Predictive prosthetic socket design: part 2-generating person-specific candidate designs using multi-objective genetic algorithms.

In post-amputation rehabilitation, a common goal is to return to ambulation using a prosthetic limb, suspended by a customised socket. Prosthetic socket design aims to optimise load transfer between the residual limb and mechanical limb, by customisation to the user. This is a time-consuming process, and with the increase in people requiring these prosthetics, it is vital that these personalised devices can be produced rapidly while maintaining excellent fit, to maximise function and comfort. Prosthetic sockets are designed by capturing the residual limb's shape and applying a series of geomet

Implementing the Risk of Endometrial Malignancy Algorithm (REM) adding obesity as a predictive factor: Results of REM-B in a single-center survey.

OBJECTIVE: In 2013, our group assessed a risk stratification tool of endometrial cancer (EC), called REM (Risk of Endometrial Malignancy). A well known risk factor for EC is body mass index (BMI). In fact, (BMI > 30 and <35 kg/m2) and severe obesity (BMI > 35 kg/m2) were associated with a 2.6-fold and a 4.7-fold increase in EC risk, respectively. Therefore, in the present study we aim to improve the performance of REM, including BMI and developing a new scoring system, called REM-B (Risk of Endometrial Malignancy score associated to BMI), to classify patients into high risk or low risk groups

The addition of anti-Müllerian hormone in an algorithm for individualized hormone dosage did not improve the prediction of ovarian response-a randomized, controlled trial.

STUDY QUESTION: Does the addition of anti-Müllerian hormone (AMH) to a conventional dosage regimen, including age, antral follicle count (AFC) and BMI, improve the rate of targeted ovarian response, defined as 5-12 oocytes after IVF? SUMMARY ANSWER: The addition of AMH did not alter the rate of targeted ovarian response, 5-12 oocytes, or decreased the rate of ovarian hyperstimulation syndrome (OHSS) or cancelled cycles due to poor ovarian response. WHAT IS KNOWN ALREADY: Controlled ovarian hyperstimulation (COH) in connection with IVF is sometimes associated with poor ovarian response result

A novel algorithm for prediction and detection of hypoglycemia based on continuous glucose monitoring and heart rate variability in patients with type 1 diabetes.

Hypoglycemia is a common and serious side effect of insulin therapy in patients with diabetes. Early detection and prediction of hypoglycemia may improve treatment and avoidance of serious complications. Continuous glucose monitoring (CGM) has previously been used for detection of hypoglycemia, but with a modest accuracy. Therefore, our aim was to investigate whether a novel algorithm that adds information of the complex dynamic/pattern of heart rate variability (HRV) could improve the accuracy of hypoglycemia as detected by a CGM device. Data from 10 patients with type 1 diabetes studied duri

Beneficial application of quantitative coronary angiography (edge detection algorithm) in analysis of dissected coronary arteries to predict long-term patency.

The present study evaluated the application of quantitative coronary angiography (edge detection algorithm) for the analysis of coronary dissection lesions after balloon angioplasty. Acute and late results were obtained by the edge detection algorithm in 60 patients with 66 dissected lesions (NHLBI types B-C). The edge detection algorithm delineated the border of the true lumen in 32 lesions (group with automated analysis alone, 48.5%) and included the dissection cap in the analysis in 34 lesions in which manual editing was adjuncted (group with manual editing, 51.5%). In both groups, the mini

A Hybrid Closed-Loop Blood Glucose Control Algorithm with a Safety Limiter Based on Deep Reinforcement Learning and Model Predictive Control.

Due to the complexity of blood glucose dynamics and the high variability of the physiological structure of diabetic patients, implementing a safe and effective insulin dosage control algorithm to keep the blood glucose of diabetic patients within the normal range (70-180 mg/dL) is currently a challenging task in the field of diabetes treatment. Deep reinforcement learning (DRL) has proven its potential in diabetes treatment in previous work, thanks to its strong advantages in solving complex dynamic and uncertain problems. It can address the challenges faced by traditional control algorithms,

A DNA Methylation-based algorithm Improves Lung Cancer risk prediction in the Prostate, Lung, Colorectal and Ovarian Cancer Screening Trial.

INTRODUCTION: Measuring DNA methylation levels at cg05575921 can improve prediction of lung cancer (LC) risk in a screening eligible population. However, these findings were based on a limited number of largely White study participants, with a history of heavy smoking (>30 pack years [PY]) and the cg05575921 based-metric was not directly compared to existing standards for LC prediction.] METHOD: We determined cg05575921 methylation levels in a nested case and control cohort featuring 1156 LC cases and 3039 controls, matched for age, sex, race and self-reported smoking status (current and forme

Local Promoter Methylation Disorder algorithm reveals bidirectional epigenetic disruption in DNMT3A-mutated AML and predicts azacitidine treatment response.

BACKGROUND: DNMT3A mutations occur in 20-25% of acute myeloid leukemia (AML) cases and are associated with poor prognosis, yet the epigenetic mechanisms underlying treatment response remain poorly understood. Traditional methylation analyses focus on mean levels, overlooking the heterogeneity that may be central to therapeutic vulnerability. We developed a Local Promoter Methylation Disorder (LPMD) algorithm to quantify methylation instability and evaluate its clinical utility in predicting azacitidine response. METHODS: The LPMD algorithm was developed using the GSE62298 dataset (68 AML pati
