General practitioner requests for upper abdominal ultrasound: their effect on clinical outcome.

Many radiology departments offer direct access upper abdominal ultrasound (US) scanning to general practitioners (GPs). We aimed to examine the influence of the scan results on the subsequent management and clinical outcome of these patients. A retrospective review was performed on all the patients referred for primary upper abdominal US (n = 82) from a single Birmingham GP practice, of 10,000 patients, between 1991 and 1996. The follow-up period from US was mean 27.9 months, SD 18.2 months. 79 referrals complied with published guidelines. 77 referrals were for suspected gallstone disease. 23

Nutritional therapy in human immunodeficiency virus (HIV) infection.

Malnutrition remains highly prevalent among individuals living with human immunodeficiency virus (HIV), and it continues to represent a critical determinant of health outcomes. The double burden of malnutrition-comprising both undernutrition and overweight/obesity-has significant clinical implications, as it not only affects disease progression but also complicates the management of HIV and its associated conditions. Undernutrition can exacerbate immune suppression and increase susceptibility to opportunistic infections, while overweight and obesity contribute to the rising burden of metabolic

Practice-Level Variation in Outpatient Cardiac Care and Association With Outcomes.

BACKGROUND: Utilization of cardiac services varies across regions and hospitals, yet little is known regarding variation in the intensity of outpatient cardiac care across cardiology physician practices or the association with clinical endpoints, an area of potential importance to promote efficient care. METHODS AND RESULTS: We included 7 160 732 Medicare beneficiaries who received services from 5635 cardiology practices in 2012. Beneficiaries were assigned to practices providing the plurality of office visits, and practices were ranked and assigned to quartiles using the ratio of observed to

"Testing-only" visits: an assessment of missed diagnoses in clients attending sexually transmitted disease clinics.

BACKGROUND: At sexually transmitted disease (STD) clinics, advances in testing technology coupled with increasing demands and diminishing resources have promoted the use of testing-only visits (clinic visits with testing for STDs but no full examination) to meet increasing demands for STD services. OBJECTIVES: The aims of the present study were to estimate the prevalence of STD diagnoses that could become "missed diagnoses" if patients would use testing-only visits and to examine patient characteristics associated with these potential missed diagnoses. METHODS: We conducted a self-administer

[New survey indicators for long-term care facilities and professional practice necessary to help take the guesswork out of pain assessment].

Taiwan's Department of Health included pain management as part of the long-term care facility accreditation process in 2012. Although this makes such facilities success in managing and reducing the pain perceptions of residents a consideration in facility evaluations and certification approval, pain remains a commonly neglected issue at Taiwan s long-term care facilities. Despite the high prevalence and consequences of pain among this group, healthcare professionals still have difficulty accurately assessing and treating pain. In order to enhance institutional pain assessment and management, t

Intraoperative air leak site detection in spontaneous pneumothorax through carbon dioxide insufflation during thoracoscopic surgery.

BACKGROUND: Although thoracoscopic surgery is commonly performed in patients with a pneumothorax and persistent air leak, it is still difficult to identify a definite air leak site during thoracoscopic surgery. The purpose of this study was to determine the safety, efficacy, and feasibility of intraoperative air leak site detection in spontaneous pneumothorax through low-pressure carbon dioxide (CO2) insufflation during thoracoscopic surgery. METHODS: Of 54 patients who underwent thoracoscopic pneumothorax operations between March 2017 and March 2018, 22 pneumothorax patients underwent surger

Impact of consultant-led care in early pregnancy unit.

Early pregnancy complication remains a significant cause of maternal morbidity and mortality. Despite the paucity of evidence to support consultant-led early pregnancy unit over nurse- or sonographer-led services, hospitals have devoted scarce resources to appoint consultants to lead their early pregnancy units. We compared the management and outcomes of confirmed and suspected ectopic pregnancy 1 year before and one year after the transition from a nurse-led to a consultant-led early pregnancy unit in a London hospital. Our study showed improvements in the rates of negative laparoscopy, ruptu

Factors influencing blood loss and postoperative morbidity in children undergoing craniosynostosis surgery: a retrospective study.

BACKGROUND: Craniosynostosis is a condition resulting from the premature fusion of cranial sutures. Corrective surgery is often associated with a large amount of blood loss, with transfusion of red blood cells (RBC) and fresh frozen plasma (FFP). The aims of this study were to determine the variables associated with increased blood loss and postoperative complications. METHODS: A retrospective analysis was performed of 42 pediatric patients who underwent craniosynostosis surgery. We analyzed the following: demographic parameters, duration of surgery, intraoperative blood loss, RBC, FFP and fl

The significance of preoperative radiological examinations in patients treated with Cloward's operation.

During a fifteen-year period 1,106 patients with symptoms and signs of cervical disc disease underwent Cloward's operation. Plain x-rays were performed in 94%, but the severity and extension of degenerative changes had no prognostic value. In 91% of the patients myelography was performed and in recent years mainly metrizamide was used as it was found to be more accurate. The findings on the myelograms were correlated according to age, severity and number of affected disc levels, but were independent of the duration of symptoms. Patients with medullary symptoms were found more often than not to

Treatment of cervical disc disease using Cloward's technique. The prognostic value of clinical preoperative data in 1,106 patients.

During a fifteen-year period 1,106 patients underwent Cloward's operation due to cervical disc disease. An analysis of the clinical features showed that the diseases had a very complex pattern of symptoms and signs. In young patients a syndrome of herniated disc predominated whereas a clinical picture of "cervical spondylosis" was found more often in the elderly. It was demonstrated that best results were obtained in young patients, patients with slight symptoms and patients with monoradicular symptoms. A short duration of symptoms is essential for a good outcome, and, on the basis of the data
