Responsive management of early resistance in cognitive-behavioral therapy for generalized anxiety disorder.

OBJECTIVE: Client resistance limits the efficacy of cognitive-behavioral therapy (CBT), rendering it an important process marker for clinicians. Yet, little is known about how CBT therapists' responsively navigate resistance. This study examined whether differences in therapists' behavior during moments of early resistance in CBT predict distal (posttreatment worry) and proximal (subsequent resistance) therapy outcomes. We hypothesized that clinicians' use of supportive, motivational interviewing (MI)-consistent strategies versus directive CBT methods during resistance episodes would relate to

Communication during radiation therapy education sessions: The role of medical jargon and emotional support in clarifying patient confusion.

OBJECTIVE: Radiation oncology consultations involve explanation of complex technical concepts using medical jargon. This study aimed to: analyse types and frequency of medical jargon that radiation therapists (RTs) use during education sessions; identify how patients seek clarification from RTs; and, explore RTs communication strategies. METHODS: Education sessions were audio-recorded and transcribed. Medical jargon was analysed using MaxDictio (a vocabulary analysis programme). A distinction was made between specialised (specialised terms used in RT or cancer) and contextual jargon (common e

Therapist-youth agreement on alliance change predicts long-term outcome in CBT for anxiety disorders.

BACKGROUND: In individual cognitive behavioral therapy (ICBT) for youth anxiety disorders, it is unclear whether, and from whose perspective, the alliance predicts outcome. We examined whether youth- and therapist-rated alliance, including level of youth-therapist alliance agreement, predicted outcome in a randomized controlled trial. METHODS: Youth (N = 91, M age = 11.4 years (SD = 2.1), 49.5% boys, 86.8% Caucasian) diagnosed with separation anxiety disorder, social phobia, or generalized anxiety disorder drawn from the ICBT condition of an effectiveness trial were treated with an ICBT progr

[Tragic, funny, instructive--mistakes in psychotherapy with children and adolescents].

This article refers to the felt sense of therapeutic mistakes, it does not focus on clear legal mistakes. Theoretical and general aspects of the scientific field of failure research are described. Furthermore, the authors present a model for the classification of different types of mistakes and illustrate how therapeutic mistakes/failure can be used creatively in supervision. Finally, various possibilities of finding a good way to deal with mistakes in therapeutic work with children and adolescents are discussed.

Assessment of communication skills and self-appraisal in the simulated environment: feasibility of multirater feedback with gap analysis.

INTRODUCTION: Multirater assessment is a powerful means of measuring communication skills. The use of gap analysis to assess self-appraisal is a strength of this technique. On the basis of Kalamazoo Consensus Statement framework and 360-degree assessment models, we developed a multirater instrument with gap analysis, with the goals of examining both communication skills and situational self-appraisal, and assessing the feasibility of the combined approach. METHODS: The multirater communication skills instrument was used to assess Pediatric and Neonatal Intensive Care fellows after participati

Association between skin testing in the pediatric emergency department and adherence to follow-up in children with asthma.

BACKGROUND: Efforts to enroll inner-city asthmatic children into continuity care after a pediatric emergency department (PED) visit are frequently unsuccessful. Providing parents with documentation of their child's allergic status and how this can be used to tailor an asthma management plan may improve adherence to scheduled continuity appointments. OBJECTIVE: To determine whether skin testing children during PED visits for wheezing and providing parents with skin test results improves adherence to follow-up visits. METHODS: A convenience sample of children aged 2 to 12 years with asthma who

"We are people too": consumer participation and the potential transformation of therapeutic relations within drug treatment.

BACKGROUND: While there is growing recognition of the benefits of user involvement within drug treatment there is scant literature documenting the actual implementation of such initiatives. Nonetheless, the extant research is remarkably consistent in identifying poor relationships between service users and staff as a principal barrier to the successful implementation of consumer participation. Focussing on participants' accounts of change within the 'therapeutic alliance', this paper investigates a consumer participation initiative introduced within three Australian drug treatment services. Patient self-management and pharmacist-led patient self-management in Hong Kong: a focus group study from different healthcare professionals' perspectives.

BACKGROUND: Patient self-management is a key approach to manage non-communicable diseases. A pharmacist-led approach in patient self-management means collaborative care between pharmacists and patients. However, the development of both patient self-management and role of pharmacists is limited in Hong Kong. The objectives of this study are to understand the perspectives of physicians, pharmacists, traditional Chinese medicine (TCM) practitioners, and dispensers on self-management of patients with chronic conditions, in addition to exploring the possibilities of developing pharmacist-led patien

"Nothing Comes to Mind…": Challenges With Identifying One's Own Role in Preventable Adverse Outcomes in Interprofessional Birthing Unit Teams, and the Implications for Quality Improvement Initiatives.

INTRODUCTION: Preventable adverse perinatal outcomes have a devastating impact on patients and providers and form the basis of many quality improvement (QI) and patient safety initiatives in birthing unit teams, including fetal health surveillance (FHS) training programs. Birthing unit staff attitudes regarding the role of interprofessional relationships on FHS decisions remain largely unexplored with respect to preventable adverse outcomes. METHODS: In this intervention-primed, constructivist grounded theory study, members across all five professions providing intrapartum care at one academi

Therapist empathy, homework compliance, and outcome in cognitive behavioral therapy for generalized anxiety disorder: partitioning within- and between-therapist effects.

Although client-perceived therapist empathy relates to positive therapy outcomes, including in cognitive behavioral therapy (CBT), little is known about how empathy exerts its ameliorative effect. One possible way is by promoting clients' subsequent homework compliance, a variable that also predicts positive outcomes in CBT. The present study sought to investigate simultaneously, in the context of 43 therapist-client dyads receiving 15 sessions of CBT for generalized anxiety disorder, (1) the association of early client-perceived therapist empathy (averaged over sessions 1, 3, 5) with mid-trea
