[Fever, tetraparesis and confusion - neurological phenomena in influenza].

In this case study, we present an unusual course of a 40-year-old patient with clinical signs of a viral infection with leading gastrointestinal symptoms. In the short term, he developed a progressive ascending sensorimotor tetraparesis with autonomic symptoms as well as qualitative disturbance of consciousness and the need for intensive medical care. Imaging revealed transverse myelitis and a diffusion disorder of the splenium (so-called CLOCC lesion). A diagnosis of "mild encephalopathy with reversible splenial lesion (MERS)" and "longitudinally extensive transverse myelitis (LETM)" was made

The economic and societal impact of periodontal and peri-implant diseases.

Periodontal and peri-implant diseases result from a chronic inflammatory response to dysbiotic microbial communities and are characterized by inflammation in the soft tissue and the ensuing progressive destruction of supporting bone, resulting in tooth or implant loss. These diseases' high prevalence, multifactorial etiology, extensive treatment costs, and significant detriment to patients' quality-of-life underscore their status as a critical public health burden. This review delineates the economic and sociocultural ramifications of periodontal and peri-implant diseases on patient welfare an

Medullary Renal Cell Carcinoma: A Case Study.

Medullary renal cell carcinomas are exceedingly rare and essentially uniformly and rapidly fatal. Expeditious diagnosis is crucial. Immediate treatment with a clinical trial or platinum-based chemotherapy is needed for metastatic disease given the aggressive nature of medullary renal cell carcinomas. In this article, we discuss a 24-year-old man with no known significant past medical history who presented with a progressive cough and shortness of breath. After evaluation at an urgent care and four evaluations in the emergency department, the patient was admitted and ultimately diagnosed with m

The importance of body image and aesthetic medicine in systemic sclerosis.

Systemic sclerosis is a progressive fibrotic autoimmune disease which mainly affects young women causing fibrosis of the skin and internal organs. In systemic sclerosis patients, facial and hand alterations are significant as these may have an impact on body image. In fact, systemic sclerosis could cause disfigurement of the most visible parts of the body, hindering self-confidence and quality of life. Therefore, the assessment of body image in systemic sclerosis is crucial in order to optimize a tailored care of the patient. In the near future, the creation of a pathway involving dedicated sp

Advances in the Treatment of Congenital Infiltrating Lipomatosis of the Face: The Role of PIK3CA Mutations and Microvascular Reconstruction.

Congenital infiltrating lipomatosis of the face is a rare aggressive-benign disorder characterized by progressive hemifacial overgrowth and complex, often asymmetrical, facial differences. Recently linked with the PIK3CA-Related Overgrowth Spectrum, it arises from mosaic mutations in the PIK3CA gene. Treatment, largely supportive and tailored to individual clinical presentations, requires a multidisciplinary approach. This article reviews the longitudinal care of a young adult patient from 8 years of age, detailing his journey through combined orthognathic and microvascular reconstructive surg

Systemic lupus erythematosus and melioidosis.

We reported a case of a young female patient presented with sepsis and diagnosed with melioidosis and systemic lupus erythematosus (SLE) within the same admission. She presented with 1-week history of productive cough, progressive dyspnoea together with prolonged fever, arthralgia, rashes and oral ulcers. She had septicemic shock, respiratory failure requiring intubation and ventilation in intensive care unit and subsequently developed acute renal failure requiring haemodialysis. Antibiotics and immunosuppressive treatment including low-dose intravenous cyclophosphamide were commenced. She had

Massive Macroglossia After Posterior Cranial Fossa Surgery: A Case Report.

A 16-year-old boy with Chiari 1 malformation presented for an elective suboccipital craniectomy and C1 laminectomy. His intraoperative course was uneventful. At the conclusion of the procedure, he met extubation criteria and followed commands. After extubation, he developed progressive upper airway obstruction and became obtunded. He was reintubated via videolaryngoscopy, which showed edema not only to the tongue, but also to the posterior pharynx and blisters over the vocal folds and epiglottis. The patient was transferred to the pediatric intensive care unit intubated and sedated. This repor

Necrotizing panniculitis: a skin condition associated with acinar cell carcinoma of the pancreas.

Pancreatic panniculitis (PP) is a rare cutaneous eruption that is associated with severe pancreatic disease. A patient presented with a fever, joint pains, and an erythematous rash with draining pustules that had spread from his legs to his arms over 4 months. Thorough investigation revealed stage IV acinar cell carcinoma of the pancreas. The rash was a result of necrotizing PP. The variable cutaneous manifestations of internal malignancies may challenge primary care physicians and dermatologists when patients present without findings associated with malignancy. Panniculitis should be kept in

Late-life dementia. Review of the APA guidelines for patient management.

Management of dementia in older patients requires an individualized and multimodal approach that involves use of psychiatric, psychotherapeutic, psychosocial, and somatic tools and treatments, in addition to patient and family education. The progressive nature of dementia and the invariable presence of comorbidity complicates the management task, although symptoms characteristic of dementia's phases can provide helpful clinical clues to guide evolving care. In 1997, the American Psychiatric Association published the "Practice guideline for the treatment of patients with Alzheimer's disease and

Guillain-Barré syndrome: disease process.

Primarily a disease of the Peripheral Nervous System, GBS occurs most frequently following a viral infection. The exact cause is unknown. The diagnosis of GBS depends mainly on clinical, laboratory, and electrodiagnostic findings. Generally, the patient will present with a progressive motor weakness of more than one extremity and areflexia. There will also be a predictable progression including symmetry of motor involvement, mild sensory symptoms, and recovery. Cranial nerves as well as the ANS have also been involved in GBS. Recovery from the disease is generally good although it may take man

The adult respiratory distress syndrome.

Acute lung injury from a variety of causes may result in capillary leakage and the nonhydrostatic edema of the adult respiratory distress syndrome. The mechanism of development of this syndrome is not yet fully understood. There is increasing hypoxemia from disturbance of normal ventilation-perfusion relationships in the lung and progressive loss of compliance. Treatment is empiric and requires the supportive use of mechanical ventilation and high inspired oxygen fractions, usually with the addition of positive end-expiratory pressure in the breathing circuit. Careful monitoring of respiratory

Fractures of the dorsal and lumbar spine.

A study of 60 children with thoracic and lumbar spine injuries indicates that the child's response to trauma is unique. It is concluded that the potential for growth and development and the presence of healthy tissues separate these injuries from comparable injuries in adults. The epiphyseal growth partly or completely restores vertebral body height, leads to a mild spinal deformity in stable injuries, and can contribute to rapidly progressive spinal deformity in unstable injuries with or without paralysis. Healthy intervertebral discs account for the multiple compression fractures and lack of

Influenza A Presenting as Life-Threatening Rhabdomyolysis and Acute Kidney Injury Requiring Renal Replacement Therapy.

Influenza A is primarily a respiratory pathogen but may rarely cause severe extrapulmonary manifestations, including myositis, rhabdomyolysis, and acute kidney injury (AKI). Recognition of this complication is often delayed because respiratory symptoms may be absent or overshadowed by systemic manifestations. We present a case of a previously healthy 31-year-old man who presented with fever, generalized myalgia, oliguria, and dark urine. Laboratory evaluation demonstrated extreme rhabdomyolysis with a peak creatine kinase (CK) concentration of 468,130 U/L, severe transaminitis consistent with

Anesthesia management for pediatric intestinal transplantation-a single-centre case series of eleven patients.

PURPOSE: Pediatric intestinal transplantation is the main treatment modality for children with intestinal failure who develop severe complications including intestinal failure-associated liver disease, progressive loss of central venous access, and repeated admissions requiring critical care management. We aimed to describe the perioperative management of patients undergoing intestinal transplantation at a tertiary children's hospital between 2012 and 2023, identifying challenges and potential solutions. METHODS: We retrospectively reviewed the anesthetic management of 11 children who underwe

Palliative care in kidney transplant recipients with graft failure.

Kidney transplantation is the best renal replacement therapy for patients with end stage kidney disease. It provides longer patient survival and better quality of life than dialysis. The clinical course after kidney transplantation could be complex and variable. Patients may develop various complications or even kidney graft failure. Symptom burden related to uraemia in patients with graft failure, and the side-effects of immunosuppression, cause psychological distress and adversely affect the quality of life of patients. Treatment decisions in patients with graft failure can be challenging to

Progressive muscle relaxation versus manual lymphatic drainage on restless leg syndrome and sleep quality in hemodialysis patients: Randomized Controlled Trial.

BACKGROUND: Restless leg syndrome (RLS) is a prevalent sensory disturbance experienced by those undergoing hemodialysis (HD), leading to mental illnesses and sleep disruption. OBJECTIVES: This study compared the effect of Progressive muscle relaxation (PMR) versus Manual lymphatic drainage (MLD) on Restless leg syndrome (RLS) and sleep quality in HD patients. METHODS: double-blinded, Randomized controlled trial (RCT), patients with HD (N = 117) were randomly assigned to three equal groups: group A (PMR with medical care), group B (MLD with medical care), and group C (control, received medica

Leveraging ICT Tools to Improve Kidney Health: A Comprehensive Review of Innovations in Nephrology.

Background: Chronic kidney disease (CKD) and end-stage renal disease (ESRD) represent a growing global health burden, affecting nearly one in ten adults worldwide. CKD is associated with high morbidity, premature mortality, reduced quality of life and enormous healthcare costs, and is primarily driven by dialysis and kidney transplantation. The silent and progressive nature of CKD means that most patients are diagnosed late, when irreversible damage has already occurred and costly kidney replacement therapies (KRT) become necessary. Dialysis services are resource-intensive, requiring significa

Feeling Through the Patient's Eyes: Delivering the Diagnosis of Glaucoma as a Therapeutic Intervention.

Glaucoma is a chronic, progressive optic neuropathy; however, receiving the diagnosis represents more than a clinical event. For many patients, it is a psychological turning point associated with fear of blindness, loss of independence, and becoming a burden, regardless of disease stage. Psychological distress, health beliefs, communication, and treatment adherence are closely interconnected in glaucoma care, although the specific effect of the diagnostic conversation has not yet been prospectively established. In this Perspective article, we propose that delivering a glaucoma diagnosis should
