Restoration of motor control and proprioceptive and cutaneous sensation in humans with prior upper-limb amputation via multiple Utah Slanted Electrode Arrays (USEAs) implanted in residual peripheral arm nerves.

BACKGROUND: Despite advances in sophisticated robotic hands, intuitive control of and sensory feedback from these prostheses has been limited to only 3-degrees-of-freedom (DOF) with 2 sensory percepts in closed-loop control. A Utah Slanted Electrode Array (USEA) has been used in the past to provide up to 81 sensory percepts for human amputees. Here, we report on the advanced capabilities of multiple USEAs implanted in the residual peripheral arm nerves of human amputees for restoring control of 5 DOF and sensation of up to 131 proprioceptive and cutaneous hand sensory percepts. We also demonst

An evaluation of sensorimotor integration during locomotion toward a target in Parkinson's disease.

Recent research suggests that basal ganglia dysfunction may result in problems integrating concurrent vision and proprioception during movement. We evaluated dopaminergic system involvement in this sensorimotor process during locomotion within a large sample of Parkinson's disease (PD) patients while "On" and "Off" their dopaminergic medications (n=25), in conditions that selectively manipulated the availability of proprioception, vision or both. The present experiment focused on two main objectives: i) to examine the relative influence of visual and proprioceptive inputs on locomotion and tar…

Limb Preference Changes after Focused-Ultrasound Thalamotomy for Tremor.

BACKGROUND: Magnetic resonance-guided focused-ultrasound (MRgFUS) thalamotomy is an effective treatment for essential and other tremors. It targets the ventrointermedius (Vim) nucleus, which is the thalamic relay in a proprioceptive pathway, and contains kinesthetic cells. Although MRgFUS thalamotomy reduces some risks associated with more invasive surgeries, it still has side effects, such as balance and gait disturbances; these may be caused by the lesion impacting proprioception. OBJECTIVES: Our aim was to quantitatively measure the effects of MRgFUS on proprioception and limb use in essen…

Tremor in chronic inflammatory demyelinating polyneuropathy: Proof of unifying network model for dystonia.

Traditional hypotheses for the pathogenesis of dystonia, the third most common movement disorder, have focused primarily on the basal ganglia. Contemporary theories have emphasized the role of the cerebellum. The modulation of peripheral proprioception also affects dystonia. We proposed a unifying network model for dystonia where the cerebellum, basal ganglia, and peripheral proprioception are connected in a circuit that forms the neural integrator network, ensuring steady position. We suggested that impairment anywhere along this circuit leads to common phenomenology-slow drifts followed by c…

Anterior cruciate ligament function after tibial eminence fracture in skeletally mature patients.

We compared anterior cruciate ligament function in skeletally mature patients after treatment of tibial eminence fractures with that of patients in two other groups: patients who had anterior cruciate ligament deficiency and patients who had undergone anterior cruciate ligament reconstruction using bone-patellar tendon-bone autografts. The Lysholm questionnaire was used to evaluate symptoms and KT-1000 arthrometry was used to determine objective knee laxity at an average follow-up of 5.2 years. Knee joint proprioception was assessed with a new method designed to test joint position sense. The …

A Protocol for Evaluation and Rehabilitation of Distal Radius Fractures Using Sensorimotor Input: A Case Series.

BACKGROUND: Proprioception and sensorimotor input are used to treat neurological and joint injuries. Following distal radius fractures (DRF) there is a temporary loss of proprioception that should be addressed. We created a protocol for evaluation, and a treatment plan following wrist surgery that is based on proprioceptive and sensorimotor input. We describe a series of patients undergoing surgery for DRF that were evaluated and treated with these protocols. METHODS: Both evaluation and treatment protocols included comprehensive sensorimotor procedures performed with eyes open and closed. Th…

Temporal discrimination at the neck region is associated with severity of cervical dystonia.

Cervical dystonia (CD) is a rare movement disorder marked by involuntary neck muscle contractions. Although somatosensory dysfunction has been proposed, the underlying pathophysiology remains unclear. Temporal discrimination (TD) deficits are discussed as potential markers of a dystonic endophenotype, possibly linked to proprioceptive impairments. This study aimed to assess TD in the neck region and explore its relationship with proprioception in head movements. We evaluated somatosensory temporal discrimination threshold (STDT) and temporal discrimination movement threshold (TDMT) in 20 CD pa…

Chemogenetic stimulation of proprioceptors remodels lumbar interneuron excitability and promotes motor recovery after SCI.

Motor recovery after severe spinal cord injury (SCI) is limited due to the disruption of direct descending commands. Despite the absence of brain-derived descending inputs, sensory afferents below injury sites remain intact. Among them, proprioception acts as an important sensory source to modulate local spinal circuits and determine motor outputs. Yet, it remains unclear whether enhancing proprioceptive inputs promotes motor recovery after severe SCI. Here, we first established a viral system to selectively target lumbar proprioceptive neurons and then introduced the excitatory Gq-coupled Des…

Use of a Textured Insole to Improve the Association Between Postural Balance and Ankle Discrimination in Young Male and Female Dancers.

UNLABELLED: Ballet dancers require a high level of postural balance (PB) and proprioception ability during performance. As textured insoles inserted into ballet shoes were found to improve proprioception ability, and better proprioceptive acuity was associated with better PB, the aim of the present study was to investigate whether the association between ankle inversion movement discrimination (AIMD) and PB changed following wearing textured insoles in young male and female dancers. METHODS: Forty-four dancers from the Australian Ballet School, ages 14-19 yrs, were tested for static and dynam…
