An attempt was made to identify the less severe cases of precocious puberty and to describe their natural course. A group of 17 girls with precocious puberty and a bone age advance over chronological age of less than two years (group 1) was compared with a group of 19 patients with severe precocious puberty and bone age advance of two years or more (group 2). Male rats were grafted on Day 21 of age with 'young' (21 days old) or 'adult' (90 days old) pituitary glands and then treated daily with 4 mg bromocriptine/kg or vehicle. Plasma samples were obtained on Days 21, 25 and 35 and when balano-preputial separation occurred. Both types of grafts advanced the age at which balano-preputial separation occurred and increased prolactin concentrations. Forty-four patients (42 f, 2 m) with precocious puberty (31 idiopathic, 1 familial, 7 cerebral, 5 McCune-Albright) were treated with cyproterone acetate for periods of 1-8.75 years in different (P less than 0.05) daily dosages of 117 +/- 6.1 mg/m2 per day (mean +/- SEM, group A, N = 20) and 60.8 +/- 2.42 mg/m2 per day (group B, N = 24). Circhoral administration (250 ng/h, i.v.) of GnRH induced a preovulatory-like surge of LH and subsequent luteal function in 4 of 4 ewe lambs 1 month before expected date of puberty. One boy and 13 girls with central precocious puberty were treated for 1 year using Buserelin, a GnRH analogue, given intranasally (0.3 mg, four times a day). After 1, 3 and 12 months of therapy, the gonadotropin responses to GnRH were abolished in all the patients whereas mean basal serum concentrations of luteinizing hormone (LH) remained similar to those of pubertal controls. Fifteen girls with true precocious puberty were examined by computerized tomography. In seven hypothalamic hamartomas were suspected. A pneumoencephalogram was performed in six cases (one patient refused), with the following results. In five, the radiological features were highly suggestive of tuber cinereum hamartoma. Seven children with precocious puberty were treated with cyproterone acetate and their adrenal function studied. In each child the drug caused significant adrenal hypofunction which was secondary to adrenocorticotrophic hormone suppression. None of these children had clinical evidence of adrenal hypofunction and it is assumed that the drug has glucocorticoid-like properties. Children receiving cyproterone acetate should carry information to warn doctors of this effect since they may require steroid cover during illness.

We measured the production rates and metabolic clearance, disappearance and excretion rates of FSH and LH as well as plasma testosterone and delta 4-androstenedione in males with precocious and delayed puberty. In precocious puberty, we found modestly elevated FSH production early in puberty, reaching adult levels by midpuberty and remaining constant thereafter. A syndrome of growth resistance, obesity, and intellectual impairment with precocious puberty is described. Two girls presenting with many of the manifestations of the Prader-Willi syndrome, including growth resistance, obesity, and intellectual impairment, also exhibited precocious sexual maturation. It is suggested that this syndrome represents a hypothalamic disturbance closely related to the Prader-Willi syndrome and should be considered one of its variants.
