Interpreting central hemodynamics in durable biventricular circulatory support patients.

INTRODUCTION: Progressive developments in mechanical circulatory support devices have seen the increasing use of devices which directly augment pulmonary blood flow, including the deployment of durable pumps in the biventricular configuration (BiVAD). This paradigm introduces complex haemodynamic interactions which have not been fully characterized, compared to LVAD alone. We sought to investigate the effect of BiVAD support on central hemodynamics with particular regard to assessing pulmonary flow and vascular properties. METHODS: This retrospective cohort study included LVAD and BiVAD patie

Société de Biomécanique Young Investigator Award: In silico study of the effects of Left Ventricular Assist Device inflow cannula design and pump unsteady flow on left ventricular hemodynamics.

Advances in left ventricular assist devices (LVADs) have improved hemocompatibility and durability, making this technology a viable lifesaving long-term treatment option for individuals with advanced heart failure (HF). Despite these improvements, the rate of stroke in LVAD patients remains high. The inflow cannula design impacts Left Ventricle (LV) hemodynamics, potentially creating thrombogenic flow patterns. This research aims to analyze the LV hemodynamic differences of two LVAD inflow cannula designs: one protruding 16 mm into the LV (HeartMate III, HM3) and the other flush with the myoca

Utility of True Fast Imaging with Steady-State Precession in Detecting Arachnoid Veils of the Posterior Fossa.

INTRODUCTION: Arachnoid membranes are well recognized as a cause of cerebrospinal fluid (CSF) flow impairment in disorders such as obstructive hydrocephalus and syringohydromyelia, but can be difficult to detect with standard noninvasive imaging techniques. True fast imaging with steady-state precession (TrueFISP) can exhibit brain pulsations and CSF dynamics with high spatiotemporal resolution. Here, we demonstrate the utility of this technique in the diagnosis and management of arachnoid membranes in the posterior fossa. CASE PRESENTATIONS: Three symptomatic children underwent cine TrueFISP

Intervillous blood flow in patients with missed abortion.

Источник пометил публикацию как отозванную

AIM: Analysis of the color Doppler features of intervillous flow in cases of missed abortion, and their comparison with the findings in normal pregnancy. METHODS: Transvaginal color and pulsed Doppler was used in the analysis of 38 patients with missed abortion and 40 patients with normal pregnancy. The gestational age ranged from 6 to 11 weeks. Repeated analysis of variance was used to compare the groups. RESULTS: Two types of Doppler signals were detected in the intervillous space: pulsatile and continuous. Impedance in the intervillous space of both groups did not change significantly wit

Evidence for the Existence of Two Opposing Pulse Waves in Retinal Vein Segments Within the Optic Disc.

PURPOSE: Pulse wave velocity (PWV) is used to assess vascular disease. We recently introduced a non-invasive technique for measuring retinal PWVs and presented evidence of venous pulse wave propagation opposite blood flow in the central optic disc close to maximum pulsation. The purpose of this study was to characterize venous pulse wave propagation more peripherally in the optic disc, where little is currently known about its dynamics. METHODS: We measured PWVs in optic disc veins in six healthy eyes at varying intraocular pressures (57 measurements in total). Our technique involves acquirin

Doppler assessment of fetal aortic isthmus blood flow in two different sonographic planes during the second half of gestation.

OBJECTIVE: To compare the reliability of Doppler blood flow measurements of the fetal aortic isthmus (AoI) according to whether the sampling plane is obtained from the traditional longitudinal aortic arch (LAA) view or the more recently described three vessels and trachea (3VT) view of the fetal upper mediastinum. METHODS: Doppler blood flow measurements of pulsatility index (PI), resistance index (RI), peak systolic (PSV), end-diastolic (EDV) and time-averaged maximum (TAMXV) velocities were performed in the AoI of 40 fetuses between 24 and 36 weeks of gestation. All measurements were sample

Estimation of the minimum pump speed to prevent regurgitation in the continuous flow left ventricular assist device: left ventricular drainage versus left atrial drainage.

Due to the fact that centrifugal and axial pumps do not require valves, there is a possibility of back flow when the pump speed is low. To estimate the minimum required pump speed to prevent this regurgitation, an in vitro simulation test was conducted. A pulsatile pump simulated the natural heart while a centrifugal pump simulated the continuous flow left ventricular assist device (LVAD). The LVAD flow was attained from the left atrial (LA) drainage or left ventricular (LV) drainage. The minimum or regurgitate flow was observed in the systolic phase with LA drainage and in the diastolic phase

In-silico analysis of outflow graft implantation orientation and cerebral thromboembolism incidence for full LVAD support.

We investigate tailoring cannula implantation angles of left ventricle assist devices (LVAD) to reduce cerebral embolism risk for full LVAD support. We resolve pulsatile hemodynamics with a multi-scale computational fluid dynamics model coupled to a Lagrangian scheme tracking 2-5 mm particles for three cannula implantations. Blood is modeled as non-Newtonian. Cerebral flow distribution is altered depending on anastomosis angle and comparison of means embolization rates between steady and unsteady flow models show that unsteady modeling is more accurate even in the full LVAD support case. Post-traumatic high-flow priapism: management by superselective transcatheter autologous clot embolization and duplex sonography-guided compression.

High-flow priapism is caused by uncontrolled arterial inflow into the penis, often from direct arterial trauma. We report a case of arterial or high-flow priapism that was treated with a combination of selective transcatheter autologous clot embolization and duplex sonography-guided compression therapy. Sonography showed a pulsatile, anechoic area at the base of the penis inside the left corpus cavernosum after blunt perineal trauma, and color Doppler examination revealed an arteriosinusoidal fistula. Autologous clot embolization was only partially successful, and 3 sessions of ultrasound-guid

High frequency, high volume ventilation for right ventricular assist.

During ventilation, the lungs may serve as accessory pumps for forward blood flow. We have noted significant hemodynamic improvement in several patients during short periods of vigorous hand ventilation with the Ambu bag and tried to reproduce the hemodynamic improvement in the management of four patients with profound low cardiac output syndrome secondary to right ventricular (RV) failure following open heart surgery. High frequency, high volume (HFHV) ventilation resulted in improved hemodynamics in these patients, as evidenced by pulsatile pulmonary blood flow and increased cardiac output.

Study on the hemodynamic effects of different pulsatile working modes of a rotary blood pump using a microfluidic platform that realizes in vitro cell culture effectively.

Rotary blood pumps (RBPs) operating at a constant speed generate non-physiologic blood pressure and flow rate, which can cause endothelial dysfunction, leading to adverse clinical events in peripheral blood vessels and other organs. Notably, pulsatile working modes of the RBP can increase vascular pulsatility to improve arterial endothelial function. However, the laws and related mechanisms of differentially regulating arterial endothelial function under different pulsatile working modes are still unclear. This knowledge gap hinders the optimal selection of the RBP working modes. To address th

Venous pulsation in the fetal left portal branch: the effect of pulse and flow direction.

OBJECTIVE: To determine whether the waveform in the left portal branch is reciprocal to the waveform found in the ductus venosus and umbilical vein due to difference in pulse direction compared to flow. METHODS: Ten fetuses (gestational age, 18-33 weeks), six with intrauterine growth restriction, three with non-immune hydrops and one with sacrococcygeal teratoma, were examined using ultrasound imaging and pulsed Doppler. Techniques were adjusted to record simultaneously the waveform from neighboring sections of the veins, relate wave components to each other and determine degree of pulsatilit

An Unusual Presentation of Superficial Temporal Artery Pseudoaneurysm Following Penetrating Head Injury.

Superficial temporal artery (STA) pseudoaneurysm is a rare vascular lesion that usually arises after blunt or penetrating trauma to the temporal region. Because the STA is only superficially covered by skin and subcutaneous tissue, it is highly susceptible to injury. We report the case of a 21-year-old male who presented with a painless, pulsatile swelling over the left temporal area three weeks after sustaining a knife injury. Doppler ultrasonography demonstrated a 4×1.2 cm pseudoaneurysm with a characteristic "to-and-fro" flow pattern, and contrast-enhanced CT angiography confirmed an oval p

Improving Mechanical Circulatory Support Outcomes in Failing Bidirectional Glenn Physiology.

There remains high morbidity and mortality with mechanical circulatory support (MCS) in failing bidirectional Glenn (BDG) physiology. We performed a retrospective analysis of children with BDG physiology supported with MCS before and after 2018. Fourteen patients met inclusion criteria (median age 1.5 years, weight 9 kg). Prior to 2018 (n = 7), with variable anticoagulation and strategies including pulsatile VAD, continuous flow VAD, and extracorporeal membrane oxygenation (ECMO), 3 (43%) of patients were transplanted with a total of 536 patient-days of support (median 59 days). Identification of an arteriovenous fistula during free tissue transfer despite negative preoperative angiography: a potentially correctable cause of venous thrombosis.

Although refinements in microvascular technique have resulted in great success with free tissue transfer, long-term anastomotic patency, particularly venous, is less predictable in patients with a history of intravenous drug abuse or other causes of intimal damage and/or venous hypertension. We report an unusual case in which an arteriovenous fistula was identified intraoperatively as the cause of pulsatile venous backflow despite a normal preoperative angiogram. A flow disturbance at the site of venous anastomosis and subsequent development of a venous thrombosis prompted more proximal venous

T2*-weighted placental magnetic resonance imaging: a biomarker of placental dysfunction in small-for-gestational-age pregnancies.

BACKGROUND: The antenatal identification of placental dysfunction in small-for-gestational-age fetuses with normal fetal Doppler flows remains an obstetrical challenge. In a significant fraction of such pregnancies, placental dysfunction is revealed by clinical manifestations such as preeclampsia, preterm delivery, or severe small-for-gestational-age at birth or by abnormal findings in the postnatal placental histologic examination. Therefore, new methods to identify placental function directly in pregnancy at the time of small-for-gestational-age diagnosis is highly needed. T2*-weighted place

From Continuous-Flow Mechanical Circulatory Support to Heart Transplantation: Hemodynamic, Immunometabolic, and Body Composition Determinants of Rehabilitation Outcomes.

Background: Continuous-flow left ventricular assist devices (LVADs) and heart transplantation (HTX) improve survival and quality of life in advanced heart failure. However, restoration of central hemodynamics does not consistently normalize exercise capacity, physical performance, or body composition. Persistent skeletal muscle dysfunction, endothelial abnormalities, metabolic disturbances, and adverse body composition changes frequently limit functional recovery. Methods: This narrative review examines determinants of rehabilitation outcomes across the transition from advanced heart failure t
