An Unusual Case of Chest Pain Presenting to the Cardiology Clinic.

We present the case of a 45-year-old physically active female who presented to the cardiology clinic with subacute chest pain. Despite an extensive diagnostic cardiac workup revealing no significant findings, her chest pain persisted. The patient was finally referred to thoracic surgery for the management of severe pectus excavatum deformity, and her symptoms improved following surgical correction of the condition. Our case report details a symptomatic case of pectus excavatum, emphasizing the importance of recognizing that this condition can be associated with clinical symptoms.

Safety of i.v. administration of redback spider antivenom.

It is unclear what the risk of allergic reactions is with appropriately given dilute i.v. redback spider antivenom (RBSAV). Ninety-five i.v. administrations of RBSAV referred from January 2001 to November 2006 were reviewed. All patients had local pain, 72% radiating pain, 57% diaphoresis and 39% systemic effects. Four patients (4%) had immediate systemic hypersensitivity reactions: none were severe, one was moderate and three mild. In 32 patients followed up for 2 weeks, three (10%) developed serum sickness. RBSAV given i.v. had a low reaction rate.

Novel Case Report of a Rare Symptomatic Orofacial Burkitt's Lymphoma.

Burkitt lymphoma is a very fast-growing non-Hodgkin lymphoma (NHL) characterized by rapid tumor progression and increased lymphocyte proliferation. It often involves the jaw and facial bones. The current case report represented a 37-year-old male patient with Burkitt's lymphoma who sought dental care for persistent and dull pain and paresthesia on the left side of his mandible extending to the periauricular region. The initial diagnosis suggested a dental origin, leading to the lower left third molar (LL8) extraction. Despite this, the patient continued to experience worsening pain, which radi

Temporomandibular disorder confounders in motor vehicle accident patients.

BACKGROUND: Motor vehicle accidents (MVA) are associated with the onset of temporomandibular disorder (TMD) symptoms. However, diagnosing TMD-related pain is challenging due to various entities that can refer pain to the region. This study aims to identify prevalent radiographic confounders to pain diagnosis in MVA patients who were subsequently referred for temporomandibular joint imaging using cone-beam computed tomography (CBCT) by comparing these patients to a cohort of patients without MVA history. METHODS: CBCTs of 738 temporomandibular joints were reviewed, with cases stratified by MVA

Odontogenic Pain Mimicking Trigeminal Neuralgia: The Diagnostic Value of Cone-Beam Computed Tomography: A Case Report.

BACKGROUND: Trigeminal neuralgia (TN) is characterized by severe and often relentless pain in the trigeminal nerve distribution. It is primarily a diagnosis of exclusion, highlighting the importance of a thorough workup to rule out treatable or secondary causes of atypical facial pain. CASE REPORT: An 89-year-old woman experienced 2 years of relentless odontogenic pain that had been misdiagnosed as TN. She was referred to endodonticsfor a cone-beam computed Tomography (CBCT) scan. CBCT led to the diagnosis of an abscess and nerve impingement due to an old crown post. Following tooth extractio

[Sacroiliac joint dysfunction with groin pain after an operation for lumbar spinal disorder. A case report].

A 75-year-old male presented with groin pain after an operation to treat lumbar spondylolisthesis (L5). Groin tenderness was localized to the medial border of the anterior superior iliac spine (ASIS). Radiographical and physical examination raised the suspicion of sacroiliac joint (SIJ) dysfunction. Injection of a painkiller into the SIJ relieved symptoms, including groin tenderness. Symptoms improved gradually, and finally disappeared after five SIJ injections. Groin pain has been reported as a referred symptom of SIJ dysfunction in 9.3-23% of patients. Prior to the patient undergoing surgery

The use of single photon emission computed tomography (SPECT) in the diagnosis of low-back pain in young patients.

Single photon emission computed tomography (SPECT) is a method of scintigraphy that provides sectional and multi-planar imaging that should improve the sensitivity for diagnosis in remote areas of the spine. To test this hypothesis, the authors reviewed 15 patients with low-back pain and compared the results of the radiographs, planar bone scans (PBS), and SPECT. Four of 15 had normal examinations with all three methods of imaging. The rest had a positive test in at least one of the three methods. Single photon emission computed tomography was positive in eleven, PBS in six, and the radiograph

Coronary calcium to rule out obstructive coronary artery disease in patients with non-anginal chest pain.

BACKGROUND: The use of coronary artery calcium score (CACS) to rule out obstructive coronary artery disease (CAD) remains debated, with performance differing across symptomatic patient subgroups. However, in patients with non-anginal chest pain, CACS may be particularly effective in identifying those who can be deferred from further testing. OBJECTIVES: To investigate the ability of zero CACS to rule out obstructive CAD in a large cohort of patients presenting with non-anginal chest pain. METHODS: Consecutive patients with non-anginal chest pain who were referred for coronary computed tomogr

Diagnostic approach to patients with suspected motility disorders: one size does not fit all.

BACKGROUND: Dysphagia and retrosternal chest pain are considered typical manifestations of major esophageal motility disorders (mEMD). High-resolution manometry (HRM) is the gold standard for mEMD diagnosis, while endoscopy and barium swallow are ancillary tools. We aimed to investigate the frequency of mEMD among patients referred for HRM with typical compared to non-typical symptoms. We also evaluated endoscopic and barium swallow data from patients with mEMD who underwent HRM. METHODS: We retrospectively collected epidemiological, endoscopic, barium swallow, and HRM data from 302 patients.

Familial Mediterranean fever with colonic lesions: A case report.

A 26-year-old man with a history of ulcerative colitis treatment presented to our clinic with abdominal pain and fever. He had a history of bloody stools and abdominal pain at 19 years of age. A thorough examination by a medical practitioner, including lower gastrointestinal endoscopy, resulted in the diagnosis of ulcerative colitis. After induction of remission with prednisolone (PSL), the patient was treated with 5-aminosalicylate. One year ago in September, his symptoms flared up again, and he was administered 30 mg/day of PSL until November of the same year. However, he was transferred to

Chronic itch induced by thalamic deep brain stimulation: a case for a central itch centre.

BACKGROUND: Central itch syndrome has been previously described in conditions such as stroke. The neurophysiology of central itch syndrome has been investigated in non-human primates but remains incompletely understood. METHODS: We report an observational study of a rare case of severe central itch following thalamic deep brain stimulation and postulate the location of the central itch centre in humans. RESULTS: The patient was a 47-year-old female, with congenital spinal malformations, multiple previous corrective spinal surgeries and a 30-year history of refractory neuropathic pain in her

Pitfalls in diagnosing geriatric general pain: coexistence of polymyalgia rheumatism and fibromyalgia.

Polymyalgia rheumatism and fibromyalgia are clinically similar. As there are no specific diagnostic markers, they are difficult to distinguish. A 92-year-old woman with a 3-month history of general pain, including left hip pain, was referred to our hospital. Her medical history included polymyalgia rheumatism that had been treated with prednisolone for 18 years. We found 14 of 18 symmetrical tender points. We diagnosed fibromyalgia based on two physical examinations and her personal history of general pain. Pregabalin was prescribed, and her pain gradually disappeared. About a month later, she

Dorsal root ganglion stimulation provides functional improvement from debilitating abdominal pain in Crohn's disease: A 12-month follow-up.

BACKGROUND: Crohn's disease (CD) is a chronic relapsing-remitting, immunological, inflammatory bowel disease involving any part of the gastrointestinal tract, most commonly, the terminal ileum. Abdominal pain is a prominent debilitating symptom of CD due to continuous intestinal inflammation, associated with disease severity and complications. However, abdominal pain has shown to occur even with disease remission. CASE PRESENTATION: A female college student with a history of Crohn's Disease was referred for severe, chronic abdominal pain, with frequent flare-ups and hospitalizations. Due to h

Differential effect of compression-ischaemia block on warm sensation and heat-induced pain.

The effect of compression-ischaemia nerve block on psychophysical thresholds for warm sensation and heat-induced pain was studied on 19 normal human volunteers. Although those two sensory submodalities should be predicted to block simultaneously, based on the fact that both are served by unmyelinated primary afferents, it was actually found that warm sensation was much more vulnerable to compression-ischaemia than heat-induced pain. This is interpreted as resulting from different summation requirements for each of the two sensory modalities; sensation of warmth depends on spatial summation to
