Light-stimulus-evoked blink reflex: methods, normal values, relation to other blink reflexes, and observations in multiple sclerosis.

The blink reflex may be evoked by auditory click or light stimulation, the latter being constantly present in all healthy subjects. The latency and amplitude of the light-stimulus-evoked blink response (L-BR) is influenced by the light intensity, distance between the light stimulus and the eye, attention, and background facilitation. The L-BR was abnormal in 75% of definite and 61% of probable multiple sclerosis (MS) patients. When abnormal, the L-BR was more commonly absent than delayed. Optic nerve lesions could be identified by the pattern of abnormalities in the L-BR in some patients. The …

The endocannabinoid anandamide regulates the peristaltic reflex by reducing neuro-neuronal and neuro-muscular neurotransmission in ascending myenteric reflex pathways in rats.

BACKGROUND: Endocannabinoids (EC) and the cannabinoid-1 (CB1) receptor are involved in the regulation of motility in the gastrointestinal (G … ileum and in longitudinal rat ileum segments where the ascending myenteric part of the peristaltic reflex was studied in a newly designed organ bath. Additionally intracellular recordings were performed in ileum and colon. RESULTS: Anandamide significantly reduced cholinergic twitch contractions of ileum smooth muscle whereas SR141716A caused an increa …

The effect of opioids on the light-off pupillary reflex.

OBJECTIVE: We examined the relationship between modeled opioid concentration and quantitative pupillary measures during remifentanil infusion sequences with particular attention to the "light-off" (LO) reflex. METHODS: Ten volunteer subjects were recruited to undergo two 10-min remifentanil infusion protocols. Pupillary unrest in ambient light (PUAL) and LO were measured at baseline and every 2.5 min during the first 10-min infusion-25-min recovery sequence, and after a wash-out period, the Neurological Pupillary index (NPi) and LO were measured during an identical infusion-recovery sequence.…

Effects of pharyngolaryngeal injury on nociceptive swallowing reflexes.

OBJECTIVES: Although pharyngolaryngeal injury impairs water-evoked swallowing, its influence on nociceptive swallowing reflexes remains unclear. Therefore, this study aimed to determine the effects of pharyngolaryngeal injury on nociceptive receptor-dependent swallowing reflexes. METHODS: Adult male rats underwent endoscopic evaluation of swallowing under anesthesia. The number, latency, and swallowing intervals were quantified. Distilled water, capsaicin (TRPV1 agonist), and allyl isothiocyanate (AITC, TRPA1 agonist) were infused onto the pharyngolaryngeal surface via a syringe pump at a con…

Interaction of tracheobronchial cough and expiration reflex in the cat.

Cough and expiration reflex (ER) are two fundamental airway defense mechanisms traditionally considered distinct motor behaviors. This study examined their temporal and functional interaction using an anesthetized cat model to elucidate the integrative neural mechanisms that coordinate airway protection. Electromyographic recordings of diaphragm and abdominal muscles were obtained during mechanically induced tracheobronchial cough and laryngeal ER. The ER was evoked in three specific phases of the cough cycle: inspiratory, active expiratory phase, and passive expiratory phase. While ER provoca…

Reflex Parotitis as an Unreported Complication of Zygomatic Implants.

Reflex parotitis is a recurrent parotid inflammation caused by an imbalance in the parasympathetic and sympathetic innervation of the salivary gland. This is an unknown but real parotid pathology. However, until now, it had not been reported as a complication of the placement of zygomatic implants. This case report presents the case of a male patient who presented from 1 month postoperatively repetitive edema in the parotid region and salivary alterations.

Mandibular extension as a trigger of the proprioceptive trigeminocardiac reflex.

The trigeminocardiac reflex (TCR) is a physiological response characterized by a reduction in heart rate and arterial blood pressure following stimulation of any branch of the fifth cranial nerve. This review presents a novel mechanism of TCR induction through proprioceptive activation of the trigeminal nerve via mandibular extension (ME). Accordingly, it is referred to as the "proprioceptive trigeminocardiac reflex" (pTCR). An overview of the existing literature on ME-evoked TCR in humans and animals is presented, with particular emphasis on evidence of cerebral microcirculatory alterations. …

Changes of the cough reflex sensitivity following the septoplasty.

BACKGROUND: Doctors need information about cough reflex sensitivity influenced by symptomatic nasal septum deviation-the most common structural abnormality of the nasal cavity. OBJECTIVES: We assessed the relationship between cough reflex sensitivity and increased nasal patency caused by the deviation of nasal septum. MATERIALS AND METHODS: Eighteen adult patients (8 women and 10 men, age range 20-59 years, mean age 40.1 years) were included in the study. Capsaicin aerosol in doubling concentrations (from .49 to 1000 μmol/L) was inhaled by a single-breath method. We measured cough reflex sen…

Targets for deprescribing in patients with hypertension and reflex syncope.

BACKGROUND: We aimed to identify the target of deprescribing, i.e. the 24-hour SBP increase needed to achieve the greatest reduction of SBP drops. METHOD: Forty hypertensive patients (mean age 73.6 ± 9.3 years, 26 females) with reflex syncope and SBP drops on a screening ABPM were advised to withdraw or to reduce their therapy. The study objective was the reduction of SBP drops <90 mmHg and <100 mmHg on a second ABPM performed within 3 months. RESULTS: Out of a total of 98 drugs taken during ABPM 1, 44 were withdrawn, 16 had a dose reduction and 38 remained unchanged at the time of ABPM 2. 2…

The Influence of Cough Reflex Testing on Patient Management.

PURPOSE: Cough reflex testing (CRT) is an adjunct to the clinical swallowing evaluation (CSE), providing information on patients' risk of silent aspiration. CRT has been shown to influence diet recommendations, but in previous work, the many varied patient characteristics are not controlled. Therefore, the specific role of CRT results in these decisions remains unclear as this relationship has not been directly assessed. METHOD: An online survey was sent to speech language therapists working in dysphagia. Two patient cases were presented that differed only by the presence of risk factors for …

[Clinical reflexes in the face of an atypical neuropsychiatric picture].

A 59-year-old man who had been presenting with a variety of neuropsychiatric symptoms for several weeks. Despite repeated visits to somatic emergencies, as well as a thorough work-up including complementary examinations and specialist opinions, no organic diagnosis was established. The patient was treated symptomatically with neuroleptics and benzodiazepines, which led to a significant improvement in symptoms.

The Rate of Asystolic Reflex Syncope Is Not Influenced by Age.

BACKGROUND: The head-up tilt test (HUT) and other evidence suggest that the vagal effect on the heart decreases with age. OBJECTIVES: The main aim of the study was to assess whether this age effect also affects the rate of asystole in spontaneous reflex syncope (RS). METHOD: We performed an analysis of pooled individual data from 4 studies that recruited patients ≥40 years of age affected by certain or suspected RS who received an implantable loop recorder (ILR) and reported follow-up data on syncope recurrence. We assessed the presence of asystolic syncope of >3 seconds or nonsyncopal asyst…

Primitive Reflexes and Dementia in Adults With Down Syndrome.

BACKGROUND AND OBJECTIVES: To determine whether primitive reflexes serve as an indicator of dementia in adults with Down syndrome (DS), we collected neurologic examination data, cognitive and behavioral assessments, and clinical consensus diagnoses of dementia from 92 adults with DS. METHODS: In a cross-sectional, observational study of a regional cohort, χ2 and Fisher exact tests examined individual reflexes across the diagnostic group (no, possible, or probable dementia). In 64 participants with all 8 reflexes assessed, the number of primitive reflexes was assessed as a predictor of diagnos…

Alprazolam and lorazepam overdose and the absence of brainstem reflexes.

Benzodiazepines (BZDs) rarely cause respiratory depression and death. On the other hand, high-dose BZDs may lead to profound sedation and diminished brainstem functions that mimic other structural brain lesions as described in our case: a 70-year-old unresponsive woman. She was hypothermic and had rapid shallow breathing. Her Glasgow Coma Scale score was E1V1M4, with pinpoint pupils and absent corneal, oculocephalic and oculovestibular reflexes. Other physical exams, laboratory testing and brain imaging were unremarkable. After two doses of 0.4 mg naloxone and intravenous thrombolytics were gi…

Reflexive and Intentional Saccadic Eye Movements in Migraineurs.

Background: Migraine has been postulated to lead to structural and functional changes of different cortical and subcortical areas, including the frontal lobe, the brainstem, and cerebellum. The (sub-)clinical impact of these changes is a matter of debate. The spectrum of possible clinical differences include domains such as cognition but also coordination. The present study investigated the oculomotor performance of patients with migraine with and without aura compared to control subjects without migraine in reflexive saccades, but also in intentional saccades, which involve cerebellar as well…

Effects of sagittal split osteotomy on brainstem reflexes.

OBJECTIVES: This prospective study was designed to assess whether patients with skeletal deformities show characteristic masseter inhibitory reflex (MIR) and blink reflex (BR) patterns. A secondary aim was to investigate whether these reflexes change following bilateral sagittal split osteotomy (BSSO). MATERIALS AND METHODS: Fourteen consecutive patients who underwent single-jaw BSSO and 14 class I subjects who constituted the control group were enrolled into the study. MIR and BR, obtained by the stimulation of supraorbital (SBR) and mental nerves (MBR), were electrophysiologically recorded.…
