PURPOSE: To assess potential retinal viability by electroretinography following selective ophthalmic artery chemotherapy infusion for retinoblastoma.
METHODS: Uncontrolled prospective case series. Patients with advanced retinoblastoma were offered elective ophthalmic artery chemotherapy infusion treatment under an IRB-approved protocol as an alternative to enucleation. The ophthalmic artery was cannulated under fluoroscopic control, and chemotherapeutic agents (melphalan and occasionally carboplatin) were directly infused at doses resulting in very high local tissue concentrations, but low sy

We report a case of branch retinal artery occlusion in a young woman suffering from established Takayasu arteritis. A 35-year-old woman with a previous history of Takayasu arteritis, diagnosed 5 years before, presented to the emergency department reporting sudden visual loss in her left eye and restriction of the visual field for the past hour. On ophthalmologic examination, occlusion of the superior temporal branch of the central retinal artery was revealed. However, no other classic signs of ocular ischemia associated with Takayasu arteritis were observed. Three out of the six criteria set o…

Central retinal artery occlusion (CRAO) is a rare and potentially devastating cause of acute blindness in sickle cell disease (SCD) that is unique compared to classic sickle retinopathy. Few details related to this complication in SCD are known, including its risk factors, pathogenesis, presentation, treatment and outcomes. We present three patients with SCD and retinal artery occlusion. The overall variability in clinical presentation, treatment and prognosis reported in the literature underscores the need for a greater understanding of these factors as they relate to this complication in SCD…

We present a case in which mfERG and OCT helped to make a diagnosis of an old BRAO in the setting of compound heterozygous MTHFR genotype. A 44-year-old woman presented for evaluation of a 10 month history of persistently cloudy vision OS. She had been worked up previously for MS versus BRAO, and she was on coumadin, folate, and multivitamin at the time of presentation. The patient has a fraternal twin sister who was diagnosed with MS. Dilated fundus examination OS showed subtle inferior optic atrophy with slight narrowing of the inferotemporal retinal artery, and HVF test revealed a superonas…

A 77 year old female patient was admitted to our hospital because of a rapid onset left-sided visual loss. Occlusion of the left central retinal artery was confirmed by the consultant ophthalmologist. The patient was further evaluated to exclude a thromboembolic event. Chest x-ray was consistent with pulmonary hypertension. Doppler echocardiography revealed a secundum atrial septal defect with a bi-directional shunt. In this 77 year old patient, arterio-arterial embolism from atherosclerotic plaques is the most probable cause of her central retinal artery occlusion. Yet, paradoxical embolism m…

PURPOSE: To evaluate longitudinally the optic disk morphology of nonglaucomatous optic nerve damage secondary to retinal nerve fiber damage, using experimental central retinal artery occlusion in rhesus monkey eyes as a model.
METHODS: This prospective study included 24 eyes of 16 monkeys. In eight eyes of eight animals, central retinal artery occlusion was produced by clamping the central retinal artery in the retrobulbar space. Occlusion was verified by fluorescein fundus angiography. The same eyes at baseline as well as the eight contralateral healthy eyes and eight monkey eyes with experi…

The paper describes the study carried out on a sample of 27 coronary artery disease patients, aiming to determine a correlation between coronary artery disease and anomalies in the retinal circulation. Patients underwent selective coronary arteriography and fundus fluorescein angiography that allowed us to investigate the dye dynamics and to detect abnormalities of time evolution, vessel walls, and flux. During the fluorescein angiography we measured time events as the choroidal flush (ta) the start of the laminar phase (tb), and the end of the laminar phase (tc) and we took pictures that were

BACKGROUND: A central retinal artery occlusion is a potentially blinding retinal vascular event with no effective treatment regimen available. Recently, a few reports have described improved vision in eyes with central retinal artery occlusions after selective fibrinolytic therapy followed by long-term systemic anticoagulation. Acceptance of this treatment, however, has been hampered by a paucity of confirmatory reports and its potential for producing serious hemorrhagic complications. Our report independently confirms the beneficial effects of selective thrombolysis, even with the use of only…

PURPOSE: To document a case of central retinal artery occlusion after chiropractic treatment.
METHODS: An 87-year-old man suffered a central retinal artery occlusion immediately after chiropractic neck manipulation.
RESULTS: A magnetic resonance angiogram showed an atherosclerotic plaque in the ipsilateral carotid artery, which probably shed emboli during cervical joint mobilization.
CONCLUSION: The carotid arteries should be approached carefully in patients with known or suspected atherosclerotic disease of the carotid arteries.

We describe the results of early hyperbaric oxygenation combined with nifedipine treatment for central retinal artery occlusion, and explain the results pathophysiologically. We report four cases in which hyperbaric oxygenation therapy was applied in combination with nifedipine, eyeball massage, and glycerol for the treatment of central retinal artery occlusion. In two of the cases in which therapy was started less than 100 minutes after the acute onset of visual loss and one case in which therapy was started during the course of central arterial occlusion, considerable improvement in visual a…

In extremely rare cases, after occlusion of the central retinal artery, a complete ring of peripapillary anastomotic channels develops. One such case is described and a proposed term given to these channels--'Nettleship collaterals'--after the man who first described them. These collaterals are dilated pathways within the terminal capillary networks of the posterior ciliary arteries and the branches of the central retinal artery. Formation of the channels is believed to be promoted by prelaminar obstruction of the central retinal artery.

Optic nerve injury after retrobulbar injection of anesthetic agents may damage the optic nerve and vascular supply to the eye. We describe a case of documented orbital optic nerve injury with central artery occlusion after retrobulbar injection before cataract surgery. Early computed tomography documentation of optic nerve injury associated with retrobulbar injection is suggested to allow early consideration of optic nerve sheath decompression.

A transient, ipsilateral central retinal artery occlusion and contralateral amaurosis developed following retrobulbar injection of 2% lidocaine and 0.75% bupivacaine. A computerized tomography scan obtained 1 1/2 hours following the injection demonstrated an air bubble within the ipsilateral optic nerve sheath. This case further supports the view that intranerve sheath injection can cause these two complications associated with retrobulbar anesthesia.

Central retinal-artery obstruction (CRAO) is a devastating complication after retrobulbar anesthesia, a procedure which was previously recommended routinely to immobilize the eye and reduce discomfort during laser surgery. Recent developments in treatment technique, involving scatter laser applications given over several sessions with smaller spot sizes and shorter durations, have virtually eliminated the need for retrobulbar injections and the risks of retrobulbar hemorrhage, which can cause increased intraocular pressure and culminate in closure of the arterial circulation. We present the ca
