Long-Term Neurocognitive, Mental Health Consequences of Contact Sports.

This article presents a brief history and literature review of chronic traumatic encephalopathy (CTE) in professional athletes that played contact sports. The hypothesis that CTE results from concussion or sub-concussive blows is based largely on several case series investigations with considerable bias. Evidence of CTE in its clinical presentation has not been generally noted in studies of living retired athletes. However, these studies also demonstrated limitation in research methodology. This paper aims to present a balanced perspective amidst a politically charged subject matter.

Economic costs of functional dyspepsia.

Dyspepsia is defined as chronic or recurrent symptoms believed to originate in the upper gastrointestinal tract. When routine investigation results in no identifiable explanation for those symptoms patients are labelled as having functional dyspepsia. In community-based surveys, approximately 30% of the otherwise apparently healthy population report dyspeptic symptoms and the majority are believed to have functional dyspepsia. Although only 1 in 4 or 5 patients make use of healthcare resources, this patient category is one of the largest in ambulatory care (1.6 to 5% of all consultations in ge

Medical malpractice in diagnostic radiology: claims, compensation, and patient injury.

The purpose of this study is to provide more information to diagnostic radiologists regarding claims, compensation, and patient injury in medical malpractice cases. Malpractice cases filed against the U.S. government were reviewed. The most common claim was misdiagnosis of a malignancy (30% of the cases); in these cases the claimants received relatively high compensation. Most of these cases involved failure to diagnose lung carcinomas on chest radiographs and failure to diagnose colon carcinomas on barium enema studies. The findings emphasize the importance of having the members of a radiolog

Legal considerations when making a practice change.

Changes in medical practice, such as retiring, selling a practice, and switching employment, have significant legal impacts on physicians. These decisions should be carefully analyzed prior to being made. Physicians who do not make decisions in a well-considered manner may face economic penalties, licensure sanctions, and/or other legal issues. This article explores some key legal issues including (1) timing, (2) patient care continuity, (3) medical records retention, (4) licensure and board certification, (5) professional liability insurance, and (6) postemployment restrictions on practice. Patients avoiding surgery. Pathology and one-year life status follow-up.

Patients were selected for a research study to determine the outcome of patients who had demonstrated enough pathology to have been viewed as surgical candidates and had chosen to avoid surgery. The purpose of the study was to identify the diagnostic categories and to obtain follow-up data on these patients. The patient population consisted of 66 patients with the following pathologies: disc disruption (one and two levels), disc disruption (three levels), stenosis, spondylolisthesis, instability, and herniated nucleus pulposus. The patients were followed for a period of 1 1/2 years and rated o

A cost analysis of reductions in work productivity for MG patients and their caregivers by symptom severity.

INTRODUCTION: Myasthenia Gravis (MG) is a debilitating autoimmune disorder associated with fatigue and weakness in the ocular, respiratory, bulbar and limb muscles. This study evaluates productivity losses for MG patients and their caregivers by MG symptom severity. METHODS: In the multinational MyRealWorld-MG study, 1,049 MG patients and caregivers reported on work productivity (sick leave, reduced working hours, early retirement). Productivity losses were calculated using the average wage per hour. A UK perspective was adopted for the whole sample, and country-specific analyses were conduct…

NZ-RugbyHealth Study: Current Postural Control Ability of Former Rugby Union and Non-contact Sport Players.

BACKGROUND: Players in contact sports frequently experience mild traumatic brain (concussion) injuries (TBI). While there are known disruptions to balance following acute head trauma, it is uncertain if sport-related concussion injuries have a lasting impact on postural control. AIM: To assess postural control in retired rugby players in comparison to retired non-contact sport players, and to evaluate any association with self-reported sport-related concussion history. METHODS: Using a cross-sectional design, 75 players in the NZ-RugbyHealth study from three sports groups (44 ± 8 years; 24 e

The Impact of Work Status and Age in Choosing Type of Abdominal Aortic Aneurysm Repair.

INTRODUCTION: The PReferences for Open Versus Endovascular Repair of Abdominal Aortic Aneurysm (PROVE-AAA) trial aimed to determine the efficacy of a validated decision aid to enable better alignment between patient preference and their ultimate repair. We sought to determine the key factors influencing the decision-making of veterans for endovascular repair of abdominal aortic aneurysm (EVAR) or open surgical repair (OSR). METHODS: A total of 235 veterans in the PROVE-AAA trial were asked their information sources regarding repairs, employment status, and preferred intervention. Answers were

Rugby Player's Aorta: Alarming Prevalence of Ascending Aortic Dilatation and Effacement in Elite Rugby Players.

BACKGROUND: Prompted by a cluster of observations concerning ascending aortic pathology in elite rugby players, we assessed over 150 asymptomatic predominantly retired players with echocardiography, aiming to document the prevalence and severity of ascending aortic dilatation and/or anterior aortic effacement, both 'risk factors' for potentially catastrophic aortic complications. METHODS: Rugby players (at least 5 years of high level competitive rugby) were classified as elite (national, state or first grade representatives) or non-elite. A total of 152 asymptomatic players with a mean age of

Long-term consequences of recurrent sports concussion.

BACKGROUND: Recurrent concussions are suspected to promote the development of long-term neurological disorders. The study was designed to assess the prevalence of major depressive disorder, mild cognitive disorders and headache in a population of retired high-level sportsmen and rugby players and to study the link between scores evaluating these disorders and the number of reported concussions (RCs). METHODS: A total of 239 retired rugby players (RRPs) and 138 other retired sportsmen (ORSs) who had reached the French national or international championship level between 1985 and 1990 filled in

Dementia in former amateur and professional contact sports participants: population-based cohort study, systematic review, and meta-analysis.

BACKGROUND: Although there is growing evidence that former professional athletes from sports characterised by repetitive head impact subsequently experience an elevated risk of dementia, the occurrence of this disorder in retired amateurs, who represent a larger population, is uncertain. The present meta-analysis integrates new results from individual-participant analyses of a cohort study of former amateur contact sports participants into a systematic review of existing studies of retired professionals and amateurs. METHODS: The cohort study comprised 2005 male retired amateur athletes who h

Prognostic Indicators after Musculoskeletal Injuries in Standardbred Racehorses in Italy.

The objective of this study was to report recovery time, retirement rate, racing performance, and presence of compensation pattern after musculoskeletal injuries (MSIs) in Standardbred racehorses (STBRs). This is a retrospective single open-cohort study, enrolling 356 STBRs in training in a single racetrack. Musculoskeletal injury was defined as any training-related injury after which the horse did not train for at least 15 days. The first and second MSIs encountered during the horse's racing career were considered. Medical records, training logbooks, and racing data were reviewed to determine…

Trajectories of mobility limitations over 24 years and their characterization by shift work and leisure-time physical activity in midlife.

BACKGROUND: We aimed to investigate trajectories of mobility limitations (MLs) over a period of 24 years. In addition, we aimed to study how shift work and leisure-time physical activity (LTPA) in midlife predict assignment to MLs trajectories separately for those retired on statutory pensions (SPs) and on disability pensions (DPs). METHODS: Subjects who responded MLs questionnaires (1985-2009, N = 3048) in Finnish Longitudinal Study on Aging Municipal Employees were included in this prospective cohort study. LTPA and shift work were measured during baseline. International Classification of F
