Despite the use of intravenous and oral contrast material and antiperistaltic pharmaceuticals, abdominal computed tomography has limitations in the morphological study of the gastrointestinal tract, retroperitoneum, and pelvis. To better define these regions, positive contrast material was infused into the peritoneal space prior to abdominal CT. Results included: improved visualization of the peritoneal cavity and its contents; evident demarcation between the retroperitoneal space and intraperitoneal structures; sharp delineation of serosal surfaces of the bowel and solid viscera; and an obvio…

We report two very unusual observations in which gas and fluid effusions were transiently and unexpectedly found in the extravertebral spaces of patients presenting with painful necrotic vertebral collapse containing a vacuum cleft. We hypothesize that gas and/or fluid which progressively may replace vacuum in vertebral compression fractures could be secondarily pumped through extravertebral and retroperitoneal spaces. Although being rare, these observations may represent a potential missing link in the imaging snapshots of the cyclic and dynamic vacuum phenomenon.

Urinary ascites in adults is usually secondary to iatrogenic or traumatic injuries. A case of a 69-year-old male with abdominal pain and decreased renal function is described. Ultrasound showed bilateral hydronephrosis and ascites. Computed tomography revealed retroperitoneal fibrosis complicated by a rupture at the right ureteropelvic junction and urine extravasation in the perirenal and intraperitoneal spaces. Delayed scans showed leaking of contrast media from the anterior perirenal space to the peritoneal cavity. The urinary ascites resolved using a double-J stent.

Perforating colorectal carcinomas may be complicated by abscess formation. If unchecked, the abscess may subsequently extend to a number of sites thus requiring emergency medical attention. The cases of five such patients treated at Massachusetts General Hospital are reported. Sepsis may spread in a short period of time over a considerable area in anatomic spaces such as the retroperitoneal space. Recognition of the source of abscess may be difficult. At times, the abscess may be accompanied by subcutaneous emphysema. Prompt diagnosis and elimination of the source of sepsis may be lifesaving.

Peritoneal carcinomatosis and metastatic involvement of the retroperitoneum are relatively common manifestations of many organ-based malignancies and lymphoproliferative disorders. Primary malignancies of peritoneal and retroperitoneal origin occur much less frequently, and can be difficult to distinguish from metastatic disease. In many cases, a precise diagnosis based on imaging findings alone is not possible. However, the imaging features of these primary tumors, in combination with the clinical and demographic data, can be utilized to narrow the scope of the differential diagnosis. This ch

Retroperitoneal fibrosis may be classified as either primary or idiopathic, probably of autoimmune origin or secondary to a malignant process. Both classes are usually presented identically, and it is the responsibility of the physician to exclude a malignant process. The disease should be especially suspected in patients with signs and symptoms of irritation of the retroperitoneal space, such as pain in the lower part of the back or in the flank, a distribution of pain down the legs and, at times, a positive Patrick's sign. Gray scale ultrasonography and computerized axial tomography may now …

BACKGROUND: The retroperitoneal approach and natural orifice specimen extraction surgery are emerging minimally invasive techniques in color … ma. By performing key surgical steps (mobilization, devascularization, lymphadenectomy) through the retroperitoneal space and eliminating the need for an abdominal incision for specimen extraction, it may reduce postoperative pain and surgical site complications.

We report a case of pathologically proved actinomycosis that presented unusual computed tomographic (CT) features: a soft-tissue retroperitoneal mass encasing the adjacent ureter resulting in hydronephrosis and spreading into the intraperitoneal space with a stellate pattern. The role of CT in monitoring the response to antibiotic treatment is also discussed.

PATIENTS AN … oon rupture causes no tissue damage, and expansion to approximately 800 mL in adults is safe if the retroperitoneal space has not previously been invaded. The use of nitrous oxide for pneumo-insufflation in the retroperitoneal space is safe if proper precautions are taken.

Wünderlich syndrome is a non-traumatic retroperitoneal hemorrhage confined to the subcapsular, perirenal, and/or pararenal space. It has been previously associated with Lenk's triad of flank pain, hematuria, and hypovolemic shock; however, this triad is not always present, making early recognition in the emergency department challenging. We describe a 39-year-old male who presented with sudden-onset severe left flank pain, diaphoresis, and persistent tachycardia without hematuria or hypotension. Contrast-enhanced computed tomography of the abdomen and pelvis rapidly showed a left renal angiomy

We describe herein a case of sporadic retroperitoneal aggressive fibromatosis. A 54-year-old man with a palpable abdominal mass was referred to our hospital for investigation and treatment. Abdominal ultrasonography and computed tomography revealed a solid mass with relatively well-defined borders in the left abdominal retroperitoneum. At surgery, a large tumor (14 x 13 x 11 cm) was found, arising from the retroperitoneal space and involving the wall of jejunum. Complete removal was achieved. Histological examination of the resected specimen revealed the presence of changes consistent with agg…

For masses in retroperitoneal space and adhesion of abdominal organs, CT was more sensitive than B mode ultrasonography. However, for hydronephrosis and lesions in ureter, B mode ultrasonography was the first choice. (3) Steroid and immunosuppressant were effective during the early st …

We report a case of retroperitoneal abscess mimicking a malignant retroperitoneal tumor. A 77-yearold woman was referred to our hospital because of retroperitoneal tumor without any symptoms. The tumor was located just behind the inferior vena cava, and the tumor size was about 38 mm. The tumor showed no abnormal findings in positron emission tomography (PET) and 123I-MIBG scintigraphy. We considered the tumor as benign tumor or retroperitoneal cyst. Follow-up computed tomography (CT) 6 months after the first visit to our department showed no change in the tumor. However, 3months later, she pr

BACKGROUND: The accessory spleen is a congenital defect characterized by a separated ectopic splenic parenchyma. The size is rarely more than 4 cm. The preoperative diagnosis is prohibitive preoperatively. The aims of the present manuscript were to present the case of a patient with a rare oversize accessory spleen and a review of the literature.
CASE PRESENTATION: A 15-year-old boy was admitted to the emergency department following blunt abdominal trauma. The computed tomographic scan showed a traumatic rupture of the spleen and a 7-cm mass at the left side of the retroperitoneal space. Cons…

Retroperitoneal fibrosis (RF) is a rare disease characterized by inflammation and fibrosis of retroperitoneal soft tissues. It is classified into two types: idiopathic (iRF) and secondary (sRF). The aim of the study was to investigate the relationship between iRF and IgG4-related disease (IgG4-RD) and to eventually extend the clinicopathological features of this condition by analysis of the sample comprising six iRF and six sRF patients. The iRF patients included four males and two females, aged 12-62 years (median 55 years). Two lesions were periaortic, one was periureteral, and three cases s

BACKGROUND: Extramedullary hematopoiesis (EMH) can present as a diffuse lesion or as a solitary mass. EMH is most commonly seen in patients with hematologic disorders and rarely in asymptomatic patients. The liver, spleen and lymph nodes are the most common EMH sites. However, EMH has also been seen at unusual sites, including adrenal glands, breast, thymus, thyroid, kidney, epididymis, lung, pleura, peripheral nerves and dura mater. The diagnosis of this lesion on fine needle aspiration (FNA) smears has rarely been described.
