Rotator cuff supraspinatus tendon injuries are clinically challenging due to the high rates of failure after surgical repair. One key limitation to functional healing is the failure to regenerate the enthesis transition between tendon and bone, which heals by disorganized scar formation. Using two models of supraspinatus tendon injury in mouse (partial tear and full detachment/repair), the purpose of the study was to determine functional gait outcomes and identify the origin of the cells that mediate healing. Consistent with previous reports, enthesis injuries did not regenerate; partial tear

BACKGROUND: This study compared the incidence and pattern of potential nerve injuries between reverse shoulder (RSA) and total shoulder arthroplasty (TSA) using intraoperative neuromonitoring. Our hypothesis was that RSA has a greater risk of nerve injury than TSA due to arm lengthening.
METHODS: We reviewed 36 consecutive patients who underwent RSA (n = 12) or TSA (n = 24) with intraoperative neuromonitoring. The number of nerve alerts was recorded for each stage of surgery. Neurologic function was assessed preoperatively and postoperatively at routine follow-up visits. Predictive factors fo

BACKGROUND: Rotator cuff tear is a common musculoskeletal injury, negatively affecting shoulder function. Rotator cuff tear severity ranges from small to massive tears, but it is unclear how tear severity affects glenohumeral joint loading and how changes contribute to secondary injury. This study's objective was to determine how glenohumeral joint contact force changes with tear severity during functional task performance using computational models.
METHODS: Eight models of increasing tear severity were developed, ranging from no rotator cuff tear to massive three-tendon tears. Simulations w

UNLABELLED: In a prospective study we examined the dominant shoulder of 11 athletes from the 1992 German Olympic water polo team. Their mean age was 25.4 +/- 3.5 years. Water polo had been practised generally for 14.1 +/- 4.3 years and professionally for 11.6 +/- 3.9 years. The weekly training amounted to between 12.5 and 19.5 hours. All athletes were submitted to standardised clinical, sonographical and MRI tests. In 5 cases we found anamnestic and/or clinical indications of rotator cuff pathology. In all 5 cases the supraspinatus tendon was affected, and in three cases both the supra- and th

BACKGROUND: Arthroscopic repair of rotator cuff tears, although commonly performed, carries the risk of retears. Therefore, bioremediation techniques such as platelet-rich plasma injections have been used as adjuvant therapies. The clinical efficacy of platelet-rich plasma in the arthroscopic repair of full-thickness rotator cuff injury is controversial. We performed a meta-analysis to evaluate the clinical effectiveness and safety of platelet-rich plasma and provide evidence-based medical recommendations for selecting the proper clinical treatment plan for full-thickness rotator cuff injuries…

INTRODUCTION: Rotator cuff repair (RCR) is commonly performed before reverse shoulder arthroplasty (RSA) with conflicting evidence on the effect on arthroplasty outcomes. The purpose of this investigation was to evaluate the effect of a prior RCR on the outcomes and complications of primary RSA.
METHODS: Between 2007 and 2017, 438 RSAs performed in patients with a prior RCR and 876 case-matched controls were identified from a multicenter database. Patients were grouped based on a preoperative diagnosis of glenohumeral osteoarthritis (GHOA) and rotator cuff tear arthropathy (CTA). Data collect

OBJECTIVE: Rotator cuff injury often disrupts shoulder proprioception. However, early postoperative recovery of external rotation proprioception after arthroscopic repair lacks quantification via ecologically valid tools. This study aimed to quantify early improvement in shoulder external rotation proprioception following arthroscopic rotator cuff repair using the Active Movement Extent Discrimination Apparatus (AMEDA), and to identify preoperative factors associated with postoperative proprioceptive improvement.
METHODS: We included thirty-one patients with unilateral rotator cuff injury who

Full-thickness subscapularis tears with advanced fatty infiltration represent one of the most challenging problems in shoulder surgery, in large part because every available treatment option is imperfect. Reverse total shoulder arthroplasty in older patients and tendon transfer in younger individuals without glenohumeral arthritis are effective but often perceived by patients as extreme, while nonoperative management frequently leaves persistent symptoms. New literature suggests that perhaps a modification of a traditional open approach, in the form of open antegrade muscle advancement, is a v

There has been a growing body of evidence that 1- and 2-year patient-reported outcomes (PROs) have little variance after many orthopaedic interventions, including lower extremity arthroplasty, shoulder arthroplasty, anterior cruciate ligament reconstruction, and, of course, rotator cuff repair. The upsides of this are huge: better ability to achieve high follow-up rates and faster iterative research progressing our field. This study shows an increase in the retear rate after , rotator cuff repair between 1 and 2 years, but in only 4% of patients. This corresponds roughly to the number of patie

Bacterial contamination as a result of suture contamination may be associated with rotator cuff retear or impaired tendon healing after rotator cuff repair. This represents a potential new area of focus and intervention. Despite an array of rotator cuff repair techniques, from varying suture configurations and double-row repairs to biologic adjuncts and patches, a substantial proportion of repairs do not heal or go on to retear. Decades of research have been dedicated to identifying risk factors for these failures and mitigating their influence. Yet, as we transitioned from open to arthroscopi

BACKGROUND: Plasma is a collection of active particles generated by dissociating molecules and ionizing atoms through applying high energy to a gas, such as high-sound heating or electrical shock. Recently, many reports have been published on the effectiveness of non-thermal atmospheric pressure gas discharge plasma (NTAPP) on living organisms. Furthermore, we have reported on the promotion of bone and tendon repair by NTAPP irradiation. We hypothesized that irradiation of NTAPP would promote the repair of the tendon-bone junction in a rotator cuff repair. This study investigated the effect of
