Microsurgical replantation of two totally avulsed scalps.

We describe our experience with two patients in whom total avulsion of the scalp occurred. Successful replantation was carried out by microvascular surgery, with 90 and 95% survival of the scalp. Normal hair regrowth and ability to raise the eyebrows was achieved in both cases. A single superficial temporal artery was anastomosed, providing excellent perfusion of the entire scalp. Microvascular replantation should be attempted with all scalp avulsions, regardless of appearance and mechanism of injury. The patient must be transferred to a medical center with comprehensive microvascular surgery …

Pustular dermatosis of the scalp associated with autoimmune diseases.

A 36-year-old woman visited our hospital with a five month history of persistent pustulation, crusting, and alopecia on the vertex of the scalp. No pathological organisms were isolated from the lesions. Histological examination revealed non-specific changes of chronic inflammation with destroyed follicles. Antibiotic therapy produced no response, but steroid therapy was effective. From these observations, a diagnosis of erosive pustular dermatosis of the scalp (EPDS), as described by Pye et al., was made. The patient also had Hashimoto's thyroiditis, autoimmune hepatitis, and Takayasu's aortit…

Congenital arteriovenous malformation of the scalp--case report.

A case of congenital arteriovenous malformation (AVM) of the scalp containing a complex vascular network is described. The AVM was totally removed, together with its feeding artery and draining veins. Congenital AVM of the scalp is rare; only 21 other cases have been reported since 1973. All involved complex vascular networks, and about one half of the patients had red or purple birthmarks. These clinical features clearly differ from those of traumatic arteriovenous fistulae of the scalp. Although many treatment approaches have been tried, total excision appears to be the most appropriate.

[Temporal arteritis with hypotension syndrome and necrosis of the scalp].

An 80-year-old male patient with histologically confirmed temporal arteritis presented with two very rare complications: ocular hypotony and bilateral scalp necrosis. The intraocular pressure gradually normalized on high-dose cortisone therapy. Scalp necrosis was surgically removed and covered with a split-skin graft with good incorporation of the transplant. Scalp necrosis as well as ocular hypotony are serious complications of temporal arteritis, indicating a poor prognosis. To date three similar cases have been reported in the literature.

Folded skin with lipomatous nevus in the forehead and scalp.

An 18-year old Japanese woman with folded skin in the forehead and scalp was found to have an underly Brachycephaly with peculiar hair lines, broad and sparse eyebrows, bilateral epicanthi, and decreased neck-shoulder angles were found, as well as reticular pigmentation of fingers and toes. Routine laboratory data and endocrine analyses were within normal limits. Combined longitudinal and coronal incisions in the forehead and scalp initially, followed by gull-wing incisions along the eyebrows, resulted in satisfactory improvement of the clinical appearance.

Scalp vein sign in Paget's disease of bone.

Prominent scalp vein dilatation was observed in 55% of 71 patients with Paget's disease of bone involving the skull. None of 137 pagetic patients without skull involvement showed visible dilatation of their scalp veins. Following disease suppression with oral etidronate disodium therapy, 21 of 30 patients showed diminution in their scalp vein distension. This "scalp vein sign" may serve as 1 marker of pagetic skull disease and provides another clinical feature of disease activity.

Scalp abscess: a complication of the spiral fetal electrode.

During the period from September, 1974, to January, 1975, we performed 276 deliveries, of which 54% were internally monitored with a spiral type of fetal electrode. Eight of the infants monitored (incidence, 5.4%) developed scalp abscesses. We had had no previous complications associated with the electrodes. An intensive investigation was conducted to detect the causative factor(s) of the fetal scalp infections. Case histories of the mothers and infants were scrutinized, instruments were examined, the environment and techniques were analyzed. A survey of maternal and infant complications durin…
