Cross-over comparative study of cold-induced brown adipose tissue activity in Greenlandic Inuit and Danes: rationale, design, and methodology.

Brown adipose tissue (BAT) is essential for non-shivering thermogenesis, a key survival mechanism for Arctic populations exposed to chronic cold. As BAT dissipates energy as heat, it presents a potential target for improving cardiometabolic health and treating obesity. The Arctic Inuit represents a unique metabolic model due to distinct genetic and environmental adaptations. This study advances methods in cardiometabolic research by investigating BAT activation in Arctic Inuit and ethnic Danes under cold exposure. A comparative crossover study of 20 Inuit and Danes includes two sessions: (A) t

[Common skin and soft tissue infections-current recommendations].

Abscesses, erysipelas, or uncomplicated skin and soft tissue infections (phlegmons) are common soft tissue infections encountered by dermatologists. Their definition, diagnosis, and treatment are summarized here based on the updated German-language guidelines. Cutaneous abscesses are encapsulated, pus-filled cavities which, depending on their location in deep or superficial skin-layers, cause solid to fluctuating, more or less erythematous, painful swelling under usually intact epidermis. Superficial and mature abscesses reveal visible collection of pus. Primary and often sufficient therapy co

Effect of Neck-Deep Immersion in Cool or Thermoneutral Water on Blood Glucose Levels in Individuals With Type 1 Diabetes.

CONTEXT: It is unclear whether immersion in cool water, typical of many beaches, increases the concentration of blood glucose in individuals with type 1 diabetes mellitus (T1DM). OBJECTIVE: To test the hypothesis in individuals with T1DM that immersion neck-deep in cool water (COOL) causes an increase in blood glucose concentration, but not exposure to thermoneutral water (THERMO) or thermoneutral air. METHODS: Eight overnight-fasted participants with T1DM were exposed for 60 minutes on separate days to 3 experimental conditions: cool water (COOL, 23 °C); thermoneutral water (THERMO, 33.5 °C

Caffeine: A potential strategy to improve survival of neonatal pigs and sheep.

Caffeine is commonly used to treat pre-and postnatal injuries, including apnoea in premature infants, as well as neurological impairment caused by hypoxia or asphyxiation often associated with difficult birthing. As an adenosine antagonist, caffeine is metabolised rapidly and transported into many tissues. Caffeine stimulates the brain respiratory centre, improving respiratory function in immature infants or neonates, provides neuroprotection to the fetal brain, and initiates non-shivering thermoregulation increasing metabolic rates. Recently, potential benefits of caffeine for animal producti

Serotonin Syndrome in the Perioperative Setting.

BACKGROUND Serotonin syndrome is a life-threatening condition that can lead to neurologic complications and is associated with the use of serotonergic medications. As the use of antidepressant medications has increased, the incidence of perioperative serotonin syndrome has transitioned from a rare diagnosis to one that should be considered as a differential diagnosis for any patient displaying signs of neuroexcitation. CASE REPORT A 70-year-old man (ASA 2) with a history of vestibular migraines (treated with venlafaxine), gastroesophageal reflux disease, and benign prostatic hyperplasia presen

Fungaemia caused by obstructive renal candida bezoars leads to bilateral chorioretinitis: a case report.

BACKGROUND: Renal fungal bezoars are remarkably rare and mostly occur in immunodeficient patients. Only a small number of cases with immunocompetent patients have been published so far. The published treatment approaches comprised systemic antimycotic therapy and surgical or minimal invasive removal of the fungal balls. In some cases irrigation of the renal duct system with amphotericin B was performed. By obstruction of the urinary tract bezoars can lead to infected hydronephrosis and severe urosepsis with high lethality. Fungaemia can cause fungal colonization in different distant organs. Fu

Successful Ultrasound-Guided Percutaneous Drainage of Multiple Splenic Abscesses in a Kidney Transplant Patient With Encapsulated Sclerosing Peritonitis: A Case Report.

Differential diagnosis of post-transplant infections should include rare/uncommon foci and pathogens. We present a rare case of life-threatening infection, a splenic abscess in a 53-year-old woman who was transplanted with a cadaveric kidney 5 months previously. The patient was admitted to our clinic with chills, shivering, and fever. She required a kidney transplant because of end-stage renal disease secondary to systemic lupus erythematosus, which had previously been treated by means of peritoneal dialysis for 7 years, until encapsulated sclerosing peritonitis developed, at which time therap

Intracarotid etomidate is a safe alternative to sodium amobarbital for the Wada test.

BACKGROUND: The Wada procedure (the intracarotid amobarbital procedure) has been used widely to evaluate the hemispheric dominance of language and memory before temporal lobe surgery in patients with medically refractory seizures. Because of repeated shortage of sodium amobarbital, attempts have been made to find a suitable alternative to sodium amobarbital. The aim of our study was to review our experience with the use of etomidate as an alternative to sodium amobarbital for Wada testing in patients with medically refractory seizures. METHODS: After the ethics approval, we retrospectively re

Spinal anesthesia in children: no longer an anathema!

BACKGROUND: Spinal anesthesia is underutilized for surgery in children in Nigeria. Until recently, only caudal and general anesthesia are employed in the pediatric age group, and the thought of spinal anesthesia was an anathema. This study evaluated cardiovascular changes, duration of spinal block, the length of surgery, length of spinal needle to establish lumbar puncture, and the incidence of complications associated with this procedure. METHODS: Thirty-two children ASA 1 and 2 between the ages of 2 and 12 had spinal block from August 1, 2008 to February 28, 2010 for orthopedic and plastic

Effects of anti-histaminic and anti-cholinergic substances on human thermoregulation during cold provocation.

The roles of histaminergic and cholinergic neuron systems in the regulation of body temperature have been studied almost exclusively in animals. Recently, we have found that motion sickness, i.e. a condition where hippocampal cholinergic mismatch signals induce a release of histamine in the vomiting centre, accelerates the decline in body temperature in men during exposure to cold. In the present study we measured the thermoregulatory effects of two substances commonly used against motion sickness, i.e. the histamine (H1) receptor blocker dimenhydrinate (DMH) and the muscarine receptor blocker
