AIMS: The aim of this study was to report the incidence of implant-related complications, further operations, and their influence on the outcome in a series of patients who underwent primary reverse total shoulder arthroplasty (RTSA).
METHODS: The prospectively collected clinical and radiological data of 797 patients who underwent 854 primary RTSAs between January 2005 and August 2018 were analyzed. The hypothesis was that the presence of complications would adversely affect the outcome. Further procedures were defined as all necessary operations, including reoperations without change of comp

After a short abstract about history and principles in the operative treatment of recurrent dislocation of the shoulder joint and after an illustration of disadvantages and complications in the operative technique described by Eden-Hybinette-Lange, which is favoured in the German speaking area, we are introducing a modification of this technique by use of an implant made from cancellous metal "Lübeck" instead of a bone block. In addition to the introduction of this new technique, first results in 14 patients undergoing this operation are shown up.

BACKGROUND: Machine learning techniques can identify complex relationships in large healthcare datasets and build prediction models that better inform physicians in ways that can assist in patient treatment decision-making. In the domain of shoulder arthroplasty, machine learning appears to have the potential to anticipate patients' results after surgery, but this has not been well explored.
QUESTIONS/PURPOSES: (1) What is the accuracy of machine learning to predict the American Shoulder and Elbow Surgery (ASES), University of California Los Angeles (UCLA), Constant, global shoulder function,

Patients with peripheral vascular disease and lower extremity amputation frequently undergo arterial bypass surgery to salvage the opposite leg. Compression of axillary-femoral bypass grafts can occur from a variety of causes. The primary physician should monitor graft pulses closely. Compression should be avoided by using forearm crutches instead of axillary crutches. Waist belts for prosthetic support should not be used, and alternatives such as a thigh corset or shoulder suspension should be used. The patient should avoid sleeping on the graft, using a wrist restraint when necessary. If the

Axillary artery transection after recurrent anterior shoulder dislocation is extremely rare. We present 2 such patients. The first, a 62-year-old man, presented with acute ischemia and a large hematoma in the axilla and chest wall. The second, a 63-year-old man, had a pseudoaneurysm and palpable peripheral pulses. Both underwent urgent computed tomography, which confirmed the clinical diagnosis, and the patients were taken to the operating room. In the first patient, intraoperative angiogram through both the brachial and the femoral route showed complete disruption of the axillary artery rende

Tissue augmentation has been used to reinforce repairs of massive chronic rotator cuff tears. This report describes a new technique for arthroscopic insertion of a biologic rotator cuff tissue augmentation. Double-loaded suture anchors are needed for the technique. One suture is used for the rotator cuff repair and 1 suture is used for the tissue augmentation. The sutures for the tissue augmentation are left exiting through accessory portals. Two free sutures are then passed through the repaired rotator cuff, 1 anteromedially and 1 posteromedially via the anterior and posterior cannulae, respe

The restoration of pain-free stable function in gleno-humeral arthritic cases in various situations such as rotator cuff deficiency, old trauma and failed total shoulder arthroplasty is a challenging clinical dilemma. The Bayley-Walker shoulder has been designed specifically for very difficult cases where surface replacement devices do not provide sufficient stability. This device is a fixed-fulcrum reverse anatomy implant where the centre of rotation is placed medially and distally with respect to the normal shoulder, to increase the lever arm of the abductor muscles. An important problem in

A case of axillary artery disruption following an anterior shoulder dislocation is reported, the eleventh such case in the past 25 years. As an alternative to dividing the pectoralis major tendon, two incisions, infraclavicular and transaxillary incisions, were used to repair the artery with an interposition Dacron graft. An absent radial pulse following a shoulder dislocation suggests the possibility of axillary artery injury, usually a dissected intimal flap but occasionally a ruptured vessel. Early arterial repair is indicated, preceded by arteriography, if available. Brachial plexus injury

BACKGROUND AND OBJECTIVE: After chemotherapy was used to treat patients with malignant bone tumors in 1970s, amputation, which was the typical intervention in the 1980s, has been substituted with limb-sparing surgery. This article reported the surgical indications, operative methods, operative effects, and complications of prosthetic replacement of the proximal humerus after the resection of bone tumors.
METHODS: From April 2004 and December 2008, prosthetic replacement was performed in 18 patients with proximal humerus tumors, including 7 patients with osteosarcoma, 5 patients with chondrosa

BACKGROUND: This study aimed to investigate the biomechanical effect of subacromial spacer (physically depressing the humeral head) and magnets (concavity compression restoration) in massive rotator cuff tear (MRCT). We assessed the influence on superior stability by employing the subacromial spacer (the spacer effect) and magnets (concavity compression effect) separately or in combination in MRCT.
METHODS: A customized shoulder testing system tested 7 fresh-frozen cadaveric shoulders. The prosthesis implantation was performed prior to all experimental conditions. Each specimen underwent the

Background: Wrist arthroplasties have not achieved clinical outcomes comparable to those of shoulders and knees, being offered low-demand patients due to a high failure rate. In the 90s, there were no wrist arthroplasties available for high-demand patients. An experimental setup for the development of a new wrist arthroplasty intended for all wrist patients were done. A long-term final follow-up to evaluate the performance of the experimental arthroplasty was performed. Methods: A novel uncemented modular wrist prosthesis with conical threaded fixation, metal-on-metal coupling and ball-and-soc

BACKGROUND: Extraction of well-fixed humeral implants during revision shoulder arthroplasty is difficult and prone to complications, especially humeral fracture (up to 12% in anatomic stem revision and 30% in reverse stem revision). To prevent this complication, we perform a vertical incomplete humerotomy (VIH) and use cerclages with suture loops and Nice knots for fixation. The purpose of this study is to describe the VIH technique with suture cerclage fixation, report the perioperative complications, and evaluate the radiological and clinical results outcomes.
METHODS: We performed a retros

BACKGROUND: As the average age of patients undergoing shoulder arthroplasty (SA) increases, the frequency of SA patients with osteoporosis is expected to rise. While the effects of osteoporosis have been described in the broader orthopedic literature, it is presently unclear how osteoporosis affects SA postoperative medical and implant-related outcomes.
METHODS: A multicenter database TriNetX was queried for patients between 2011 and 2021 who underwent SA with and without osteoporosis. Patients with less than 2 years of follow-up and those with a prior shoulder hemiarthroplasty were excluded.

PURPOSE: Reverse shoulder arthroplasty (RSA) in fractures is especially indicated in patients of advanced age, although the influence of age on functional outcomes, health-related quality of life (HRQoL) and complication rate is unknown. We hypothesized that complication rate would decrease in the geriatric population because of their reduced activity, improving the perceived HRQoL.
METHODS: Retrospective study of 42 patients with proximal humeral fractures treated with RSA, divided into two groups: <80 years (<80y)(n = 16) and ≥80 years (≥80y)(n = 26). The assessment tools used were radiolog

Complex proximal humerus fractures with an avascular head fragment and unreconstructable fracture types represent indications for humeral head replacement. Special prosthetic designs allow modular anatomical restoration of the centre of rotation and alignment and stable fixation of the tuberosities. These play a key role with respect to the functional outcome and are often prone to complete or partial osteolysis with secondary rotator cuff deficiency. Because the operational procedure is technically demanding, attention must be paid to correct implantation. The functional results which can be

This prospective randomized study compared the immediate postoperative periglenoid radiolucencies among 3 glenoid-drying techniques used in total shoulder arthroplasty. Seventy-one consecutive patients with primary osteoarthritis underwent total shoulder arthroplasty by use of 1 prosthetic system with convex-back, keeled, polyethylene glenoid components; the same modern, instrumented pressurization technique was used to cement all glenoids. Of the shoulders, 21 had glenoid implants cemented after bony preparation with thrombin-soaked gel foam, 24 after compressed gas lavage, and 26 after salin
