[Obstructive sleep apnea syndromes].

Snoring and excessive sleepiness are the hallmarks of obstructive sleep apnea syndrome but other clinical manifestations are present and a precise assessment of the disease involves clear definitions of the various kinds of apnea. Several pathogenetic factors (functional, anatomical, neurological, genetical) are still being discussed. However new insights of the pathophysiology of apneas allow more reliable treatments. Central nervous and cardiovascular complications as well as the traumatic morbidity, associated with excessive daytime sleepiness, make it a major public health problem.

Sleep organization and epilepsy.

Sleep is known to facilitate epileptic manifestations but can also protect the sleeper against the recurrence of seizures. This has been demonstrated in studies on sleep deprivation, and is particularly evident in alcoholic epilepsy and matutinal myoclonus epilepsy. Sleep organization in the epileptic patient is permanently altered by frequent awakenings and stage shifts. Nocturnal grand mal and repetitive partial seizures worsen the sleep disorder by reducing total sleep time and decreasing REM percentage by half. The cumulative effect of these sleep disorders may act on day-time vigilance in…

Sleep apnea in acromegaly.

OBJECTIVE: To provide information on the nature, prevalence, and severity of sleep apnea in patients with acromegaly. DESIGN: Consecutive case series. SETTING: Tertiary referral hospital. PATIENTS: Fifty-three patients with acromegaly were consecutively referred: 33 patients were referred because of clinical suspicion of sleep apnea and 20 patients were referred without suspected apnea. MEASUREMENTS: Sleep studies as well as growth hormone and insulin-like growth factor 1 (IGF-1) measurements were done. MAIN RESULTS: Thirty-one patients (93%; 95% Cl, 85% to 100%) referred because of suspi…

[Obstructive sleep apnea syndromes].

With the exception of cases where a lesion creates a clearly identifiable and surgically removable anatomical obstacle on the upper air duct (tumors, cysts, hypertrophic tonsils, vegetations,...), currently the continuous positive air pressure is the treatment of choice. One drawback is that in most cases it is no more than a symptomatic treatment. The most efficient alternative treatment is using extensive maxillofacial surgery. Another promising way is prevention through specific mouth breathing treatment from the earliest age on.

Obstructive sleep apnea syndrome.

The diagnosis and treatment of obstructive sleep apnea syndrome (OSAS) is a field of emerging interest to a wide variety of professionals, including dentists. OSAS is now widely recognized as a common clinical disorder with potentially life-threatening consequences. The syndrome is characterized by repeated episodes of upper airway obstruction during sleep. The most prominent clinical manifestations are loud snoring during sleep and daytime hypersomnelence. Bypassing the area of obstruction with a tracheostomy results in an improvement in clinical symptoms. The dentist's role may be to fit a s…

[Cardiovascular disorders during sleep].

Holter recordings of subjects apparently free from cardiovascular disease have demonstrated a moderate sinusal and nodal depression during sleep. This depression does not seem to be sufficient to create overt cardiovascular disorders in apparently healthy subjects, but it may aggravate or even reveal an underlying disorder of rhythm or conduction in elderly people or in patients taking drugs that potentiate its effects. In sleep apnea syndrome prolonged episodes of apnea may produce a paroxysmal, then permanent increase in pulmonary arterial pressure, which may lead to right heart failure. The…

[Chronobiologic organization of sleep].

Interrelations between sleep and chronobiology as been studied in isolated experimental situations. A succession of hormonal regulations has been described to explain these mechanisms. Some disruptions of these regulations might be at the beginning of a lot of sleep pathologies (jet lag syndrome, burn out syndrome, insomnia...).

[The sleep apnea syndrome].

Polygraphic nocturnal records make it possible to differentiate different forms of the sleep apnoea syndrome, depending on the predominating type of apnoea--central or obstructive. In mixed apnoea always one component predominates. On five typical cases of sleep apnoea the authors describe basic symptoms, principles of diagnosis and pathophysiological mechanisms of their development. In the clinical picture usually several factors and pathophysiological systems participate, most frequently functional disorders of respiratory centres and tonigenetic structure along with their afferentation or i…

Cardiovascular stress and sleep.

This review summarizes briefly the present knowledge on sleep-related factors in ischaemic heart disease. A marked circadian rhythm in the frequency of onset of acute myocardial infarction has been found, but the exact mechanism is not known. The circadian variation is possibly explained by several mechanisms. The best documented is sleep apnoea syndrome, which seems to be a risk factor for ischaemic heart disease and stroke. Stressful REM-sleep seems to be potentially arrhythmogenic in patients with decreased cardiopulmonary function. The role of coronary spasm, increased thrombocyte aggregat…

Episodic sleep disorders (parasomnias).

In the vast majority of patients with parasomnias, the evaluation can be accomplished in the office setting, based on information from the patients themselves and family members or other observers of the nocturnal events. Parasomnias are episodic sleep disorders that include sleepwalking, night terrors, and nightmares. All three conditions are more prevalent in childhood, when developmental factors appear to be responsible for their occurrence; in the much less frequent cases of a delayed onset or persistence in adulthood, psychopathologic states are often primary in their cause. Based on a nu…

Sleep-related respiratory disorders.

Sleep-induced narrowing of the upper airways underlies the widespread and supposedly trivial complaint of snoring, which may not only constitute a risk factor for the cardiocirculatory system, but in predisposed individuals may lead to the OSAS. The latter is a life-threatening condition characterized by repeated episodes of cessation of respiration at night with an associated drop in SaO2. Patients frequently present with hypersomnia, systemic and pulmonary hypertension, and even heart failure. HSD is the term we use to describe the evolutive stages from snoring to OSAS. ICAH, or Ondine's cur…

Sleep apnea in narcolepsy.

Polysomnographic and multiple sleep latency studies documented recurrent apneic episodes during NREM sleep stages 1 and 2 and REM sleep in 11 of 16 narcoleptic patients. Episodes were predominantly central, but obstructive and mixed events were also observed. Apneas were accompanied by mild to moderate oxygen desaturation.

Familial lethal sleep apnea.

Three of six siblings presented with sleep apnea between 18 and 26 months of age. Twin females and a male had normal growth and development without antecedent neurologic or apparent metabolic disorder. The females presented at 25 and 27 months respectively with irregular respiration and episodes of apnea. Twin A succumbed to an apneic episode while sleeping. Central sleep apnea was diagnosed in twin B at the Stanford Sleep Clinic. She died following an apneic episode three months after evaluation. The male presented at 18 months with fatal sleep apnea. A fourth child was evaluated for sleep ap…

Breathing route during sleep.

Nasal obstruction has been associated with apneic episodes during sleep. However, the normal distribution of nasal and oral air flow while asleep has not been investigated. To determine the normal route of ventilation during sleep, we studied 7 healthy men and 7 healthy women using a sealed face mask that mechanically separated nasal and oral air flow. Standard sleep staging techniques were employed. The subjects slept 297 +/- 29 (SEM) min, with a mean of 197 +/- 15 min of ventilation recorded. Ventilation was decreased during sleep as has been previously demonstrated. However, during sleep, w…

Sleep apnoea in infancy.

Serial polygraphic sleep studies were carried out in 86 index infants (33 'symptomatic', 24 siblings of infants with the sudden infant death syndrome (SIDS), 29 'near-miss' for SIDS) and 11 healthy controls. Brief (greater than or equal to 3 less than 6 sec) or prolonged (greater than or equal to 6 sec) obstructive apnoea was observed only in index caes, and coincided with symptoms due to associated illnesses (usually respiratory). Their prevalence was comparable in 'symptomatic' and 'near-miss' groups--39% and 35% respectively. Prolonged (greater than or equal to 20 sec) central apnoea was se…
